Psychological factors in dyspepsia of unknown cause: a comparison with peptic ulcer disease.

Patients with dyspepsia of unknown origin (DUO) and those with similar upper abdominal symptoms but with an organic cause (peptic ulcer) were assessed on personality and psychological symptom measures. The DUO patients had significantly more symptoms of anxiety and tension and higher scores for trait tension and hostility than the organic group. The two groups did not differ significantly in terms of depressive symptoms, neuroticism, psychoticism, or suppression of negative affects. The implications of these findings for the aetiology and diagnosis of DUO are discussed.

Goal-directed behavior in children with spina bifida.

A group of school-age children with spina bifida (n = 15) between the ages of 6 and 12 years were compared with an age- and IQ-matched control group of normal children (n = 15). Goal-directed behavior in these two groups was measured by videotaped observational measures of the number of different task-oriented play activities the children performed and the amount of time they spent in independent task-oriented activities. Measures of time spent off-task and in simple manipulation of the play materials were taken, as well as the number of social initiations the child directed to the examiner. T

[Psychosocial problems in HIV infected families--possibilities for coping].

The psychosocial problems of 49 HIV-exposed children (data given as of September 1989) and 19 HIV-infected hemophiliacs cared for in the pediatric immuno-deficiency clinic of the Munich university hospital are described in detail. Drug addiction, financial needs, housing problems and delinquency play an important role in the first group. Hemophiliacs, however, live in more stable family conditions. Both groups are offered counseling, home and hospital visits, and legal, administrative and financial support. Furthermore we help with coordinating regular meetings, obtaining short-time housekeepe

Illness representations and matching labels with symptoms.

Three studies are reported that show that health-relevant information (e.g., blood pressure [BP] or symptoms) initiates an active cognitive search process that results in the construction of an illness representation. Study 1 showed that informing subjects that their BP was elevated affected two attributes of illness representation: identity (label and symptoms), and time line or expected chronology of the health threat. Subjects given a high-BP reading reported symptoms commonly associated with high BP, especially if they attributed the high-BP reading to stress. Study 2 showed that the activ

Psychopathology and temporal lobe epilepsy in adolescents.

Although high rates of psychopathology in children and adolescents suffering from temporal lobe epilepsy (TLE) have been reported in the literature, the subject remains controversial. In this investigation we made a psychiatric study of 26 adolescents with TLE using a detailed structure interview and rigorous diagnostic criteria. These were compared to 26 matched controls suffering from chronic bronchial asthma (BA). In neither group was there a high rate of specific psychiatric illness, although many adolescents in both samples showed moderate to severe depression. We conclude that TLE may no

Nonpsychotic patients referred to a consultation service.

In order to examine characteristics of nondangerous and nonpsychotic patients referred to a consultation service, the authors administered self-rating scales and questionnaires to fifty referred medical patients and nonreferred controls. Recalled abuse in childhood appeared a stronger predictor for referral than some other commonly researched ones such as parental death, divorce or separation in childhood or recent losses. Anxiety, hostility and depression were significantly higher in referred patients, whereas hypochondriacal attitudes did not differ between the two groups. The findings appea

Disability among home-nursed patients and home-help clients.

218 chronically ill or elderly (65 years or over) home-nursed patients and home-help clients were studied to obtain information about the degree of disability, factors associated with disability, and predictors of disability. About 9% of the patients and clients needed help to a moderate extent in daily and self-care activities, but none was totally dependent. Poorly self-perceived health status, dementia and neurological disease were associated with serious disability. Male sex and living alone were correlated with high functional capacities. Only one-tenth of the patients and clients alive a

[Psychosomatic diseases of the eye--a review].

After remarks on the uncritical use of the term "hysteria" in some investigations on psychosomatic phenomena in ophthalmology some recent reviews are presented, followed by the results of psychosomatic research on specific ophthalmological diseases. Dealing with glaucoma controlled studies are emphasized which attempt to determine the structure of neuroses and the importance of environmental situations. Using the MMPI high values on the scales 'depression', 'hypochondria' and 'hysteria' were observed. In previous attempts to influence glaucoma psychotherapeutically mainly autosuggestive and hy

Motion sickness is linked to nystagmus-related trigeminal brain stem input: a new hypothesis.

Motion sickness is a common and distressing but poorly understood syndrome associated with nausea/vomiting and autonomic nervous system accompaniments that develops in the air or space as well as on sea or land. A bidirectional aetiologic link prevails between migraine and motion-sickness. Motion sickness provokes jerk nystagmus induced by both optokinetic and vestibular stimulation. Fixation of gaze or closure of eyes generally prevents motion sickness while vestibular otolithic function is eliminated in microgravity of space, indicating a predominant pathogenetic role for visuo-sensory input

Attentional and mnemonic deficits associated with infectious disease in humans.

BACKGROUND: Infectious diseases are accompanied by behavioural and psychological changes that suggest the implication of the central nervous system. Among them, cognitive alterations have been reported, but their specificity and implication in everyday life are still largely unclear. The purpose of the present study was to evaluate and specify the everyday memory disturbances in sick human subjects and to determinate the role of fever in the appearance of these alterations. METHODS: The study was carried out in a military training centre for naval recruits. Ninety-one volunteer subjects, heal

Atrial pacing: who do we pace and what do we expect? Experiences with 100 atrial pacemakers.

The records of 100 patients with permanent atrial pacemakers implanted over a 7-year period were reviewed to assess the role and results of this mode of pacing. Indications for pacing were sick sinus syndrome in 91, carotid sinus hypersensitivity in 3, and use of an antitachycardia device in 6 patients. The mean follow-up period was 32.9 months. Symptomatic relief was good. Lead dislodgment occurred in 11 patients (usually in the first week). Threshold rises not amenable to reprogramming occurred in three patients and loss of sensing occurred in seven patients but only one required interventio

Case Report: Relief of long-standing severe motion sickness following stellate ganglion block.

BACKGROUND: Motion sickness is a common and often debilitating condition arising from sensory conflict within the vestibular-visual-proprioceptive system, with downstream activation of autonomic and emetic pathways. Despite its prevalence in travel, maritime operations, aviation, spaceflight, and virtual environments, effective prophylaxis remains limited by adverse side effects. Experimental and clinical evidence implicates autonomic imbalance and sympathetic nervous system (SNS) hyperactivity in motion sickness susceptibility and symptom severity. CASE PRESENTATION: We report the incidental

beta-Endorphin and arginine vasopressin following stressful sensory stimuli in man.

This experimentation partially defines, for the first time, the response of beta-endorphin (ENDO) in man during tests designed to elicit nausea and motion sickness. These responses are similar to those associated with arginine vasopressin (AVP) and adrenocorticotropin (ACTH) to the extent that all hormones rise in response to motion sickness (p < 0.003). Repeated exposure diminished motion-induced release of ENDO (p < 0.005) and AVP (p < 0.004) despite a three-fold increase in resistance to motion stimuli. Higher post-stress levels of AVP (p < 0.04) and ACTH (p < 0.02) were correlated with gre

The brief illness perception questionnaire.

OBJECTIVE: This study evaluates the Brief Illness Perception Questionnaire (Brief IPQ), a nine-item scale designed to rapidly assess the cognitive and emotional representations of illness. METHODS: We assessed the test-retest reliability of the scale in 132 renal outpatients. We assessed concurrent validity by comparing the Brief IPQ with the Illness Perception Questionnaire-Revised (IPQ-R) and other relevant measures in 309 asthma, 132 renal, and 119 diabetes outpatients. Predictive validity was established by examining the relationship of Brief IPQ scores to outcomes in a sample of 103 myoc

Sensory conflict theory of space motion sickness: an anatomical location for the neuroconflict.

Most investigators understand sensory conflict to mean a discontinuity between either visual, proprioceptive, and somatosensory input, or semicircular canal and otolith input. Few hypotheses have attempted to define specific physiological mechanisms linking the conflict with the sickness. Suggestions that the theory be renamed the neural mismatch theory allow for the possibility that central integrative mechanisms are involved in interpreting the significance of the sensory environment and that the conflict between visual or vestibular input systems or between separate components of the vestib

[Spatial distribution and bloodmeal preferences of Glossina palpalis palpalis in the forest focus of Zoukougbeu (Ivory Coast). Epidemiological consequences].

In the sleeping sickness focus of Zoukougbeu (Côte d'Ivoire), in the cropping areas which are favourable for disease transmission, more than a quarter of the flies collected were found to have fed on domestic pigs. The sites where Glossina palpalis palpalis was caught fed on these animals were concordant with the sites where the patients were present. These results might indicate that in Zoukougbeu, but perhaps also in other sleeping sickness foci, the pig could play an active role in disease transmission, allowing the parasite to spread widely via the tsetse.

Prior chronic stress induces persistent polyI:C-induced allodynia and depressive-like behavior in rats: Possible involvement of glucocorticoids and microglia.

When animals suffer from viral infections, they develop a set of symptoms known as the "sickness response." Recent studies suggest that psychological stress can modulate the sickness response. However, it remains uncertain whether acute and chronic psychosocial stresses have the same effect on viral infection-induced sickness responses. To address this question, we compared changes in polyI:C-induced sickness responses, such as fever, change of body weight and food intake, mechanical allodynia, and depressive-like behavior, in rats that had been pre-exposed to single and repeated social defeat

[Shaping the interaction between nurses and patients in the rehabilitation process--the example of hospital care for paraplegic persons].

This study analyses the interaction between nurses and paraplegic patients. The way how nurses perceived, interpreted and dealt with important themes for patients during the process of care determines the quality of interaction. Based on these findings we developed a model for interaction in the nursing process that is characterised by its orientation towards a mutual understanding. This model enables nurses to systematise nursing negotiation processes, from identifying problems to determining options for interventions to decision-making.

[Reduction of pain perception of chronically ill children by intermittent infusion therapy].

Chronically ill children who require life-long i.v. infusions in regular intervals (i.e. Agammaglobulinemia, HIV-infection) can be traumatized by such painful procedures. Some children gradually accept such infusions, whereas other children become sensitized and react with increasing fear and resistance. We report on our experience with a combined method of local anaesthesia and active role-plays enacting the i.v. infusion on a puppet. In more than 230 applications (approximately 11 per child) in our outpatient clinic, 19/20 children reported significantly less pain sensations after the 2nd or
