Pancreatic resection in patients with synchronous extra-pancreatic malignancy: outcomes and complications.

BACKGROUND: Patients may present with a resectable pancreatic tumour in the context of a concurrent primary extra-pancreatic malignancy. These patients pose a dilemma regarding their suitability for surgery. We evaluated our experience with such patients who underwent pancreatic resection with curative intent and detailed their outcomes and rationale for surgical decision-making. METHODS: A retrospective review of patients with pancreatic concurrent extra-pancreatic primary malignancy who underwent pancreatic resection at our institution over a 12-year period (2005-2016) was performed. Percutaneous endoscopic sigmoidopexy using T-fasteners for management of sigmoid volvulus.

BACKGROUND: Sigmoid volvulus (SV) is the third leading cause of large bowel obstruction after colonic neoplasms and diverticular disease. SV has traditionally been managed via endoscopic detorsion and decompression followed by non-emergent surgical resection owing to the high risk of recurrence. Most cases of SV occur in elderly debilitated patients with multiple comorbidities and may not be candidates for surgical resection. Endoscopic sigmoidopexy has been described as an alternative to surgical resection. Here, we describe our experience with endoscopic sigmoidopexy using T-fasteners. METH

Ureterosigmoidostomy associated signet ring colon cancer presenting as hip pain.

PURPOSE: We present a case of a young male with a history of ureterosigmoidostomy who presented with hip pain, the subsequent workup of which revealed metastatic bone lesions from a primary sigmoid signet ring cell adenocarcinoma. METHODS: Review of literature was conducted using databases PubMed Medline (1966-current), PubMed Central (1970-current), and EMBASE (1988-2009) to explore previous studies on the topic; used the Surveillance, Epidemiology and End Results Program for epidemiological data; and used the National Comprehensive Cancer Network guidelines for management recommendations.

Multiple extragenital adenomatoid tumors in the mesocolon and omentum.

Adenomatoid tumors are benign mesothelial neoplasms most commonly found in the male and female genital tracts. Extragenital adenomatoid tumors are rare, most of them being solitary tumors. To our knowledge, only one case of multiple extragenital adenomatoid tumors, involving the liver and peritoneum, has been reported to date. Here we report another case of multiple extragenital adenomatoid tumors involving the mesocolon and omentum. A 47-year-old woman presented with a delayed menstrual period. Ultrasonography revealed a left adnexal mass, and surgical resection was attempted at a local hospi

Port site metastasis of sigmoid colon cancer after a laparoscopic sigmoidectomy: report of a case.

A 75-year-old woman with TNM stage I (T2, N0, M0) sigmoid colon adenocarcinoma underwent a laparoscopy-assisted sigmoidectomy. Five months later, the patient developed two subcutaneous metastases at two trocar sites. A second operation was performed and two abdominal wall masses were resected, but the operation revealed no evidence of peritoneal dissemination, liver metastasis, or lymph node metastasis. The histological findings confirmed the diagnosis of metastatic carcinoma of the sigmoid colon. Large-scale studies have shown the actual rate of port site metastasis to be similar to that obse

Adenocarcinoma of the colon and rectum.

The site and the sex incidence of 555 carcinomas of the colon surgically resected at The Hospital of the Good Samaritan in Los Angeles were reviewed. Most of the carcinomas of the right and transverse colon were in female patients, and slightly more than half of the cancers of the left and rectosigmoid were in male patients. The largest tumors were found in the right and transverse colon. The incidence of carcinomas in the proximal colonic segments, especially the cecum, appears to be greater in patients of middle or upper socioeconomic groups, a phenomenon possibly related to diet.

Study on eight patients with malignant tumors after renal transplantation.

The high incidence of cancer after renal transplantation is now a critical concern since the graft survival rate has been improved extensively. We experienced 9 malignancies in 8 patients out of 168 recipients up to December 31, 1999 in our hospital, consisting of a case of gastric plasmacytoma and cases of cancer in the liver (2), thyroid (2), prostate (1), breast (1), sigmoid colon (1) and gall-bladder (1). Two patients were diagnosed as having tumors within 3 months after transplantation, suggesting post-transplant acceleration of growth of the latent tumors. The other patients were diagnos

Case report: Giant cystic ileal gastrointestinal stromal tumor with an atypical intratumoral abscess.

BACKGROUND: Gastrointestinal stromal tumors (GISTs) are typically solid, sometimes with small cystic areas, but rarely manifest as predominantly cystic neoplasms. In addition, cystic intestinal GISTs with intratumoral abscess formation are rare. CASE PRESENTATION: We present the case of a 49-year-old male patient with a history of frequent and urgent urination for 2 weeks. Radiologic studies revealed a large cystic mass in the lower abdomen. The patient underwent abdominal laparotomy, which revealed a large cystic mass arising from the distal ileum invading the sigmoid mesocolon and apex vesi

Partial duplication of MSH2 spanning exons 7 through 14 in Lynch syndrome.

BACKGROUND: Lynch syndrome, also referred to as hereditary nonpolyposis colorectal cancer, is the most common form of hereditary colorectal cancer, and is associated with a high incidence of multiple primary neoplasms in various organs. METHODS: A 79-year-old woman (patient 1) diagnosed with ascending colon cancer had a history of previous carcinomas of the uterus, stomach, uroepithelial tract, and colon. One year later, she developed a brain tumor (glioblastoma). A 54-year-old female (patient 2) was diagnosed with endometrial cancer and sigmoid colon cancer. Both patients underwent genetic e

PDR or ADR as a quality indicator for colonoscopy.

Interval (missed) cancers and lower-than-expected mortality reduction of proximal colon cancers after screening colonoscopy drew attention to quality indicators. Small proximal polyps (prone to be advanced neoplasms) missed by colonoscopy are possible contributing factors. In this issue of AJG, the subject of polyp detection rates (PDRs) and adenoma detection rates (ADRs) in the proximal and distal colon is discussed by one group of investigators to address the issue of monitoring performance and to achieve improvement. The authors observed that these two parameters correlated well in segments

[Primary lymphomas of the large intestine].

Primary colorectal lymphomas are rare tumors that comprise 0.5 to 2% of large bowel malignancies and 10 to 20% of gastrointestinal lymphomas. Between 1982-1993 ten patients were treated in the Colorectal Unit of the "Hospital das Clínicas" of the University of São Paulo, representing 2% of the neoplasms in this period. Age varied from 22 to 76 years, with median around 50 years, and male/female ratio 8:2. Weight loss, right iliac palpable mass and bowel habit alterations were the most common symptoms. Definitive diagnostic was only set with histological evaluation of the surgical specimen in a

Symposium on outpatient anorectal procedures. Flexible fiberoptic sigmoidoscopy: an office procedure.

Patients who had benign polyps and cancers were studied retrospectively to define whether use of the 60-cm flexible sigmoidoscope would markedly improve the diagnostic yield over the 25-cm rigid sigmoidoscope. Of the polyps found, 48% could have been seen by the rigid sigmoidoscope and 85% by the flexible sigmoidoscope. Cancers were within reach of digital examination in 21%, of the rigid sigmoidoscope in 38% and of the flexible sigmoidoscope in 60%. This verifies the greater diagnostic yield by the flexible sigmoidoscope. In 211 asymptomatic patients who underwent flexible fiberoptic sigmoido

Resectable adenocarcinoma of the rectosigmoid and rectum. II. The influence of blood vessel invasion.

Several series have examined the influence of blood vessel invasion (BVI) by tumor on survival of patients with colorectal cancer; however, little data are available regarding its influence on patterns of failure. In an effort to determine the influence of BVI on the patterns of failure and survival in rectosigmoid and rectal cancer, a retrospective review of 168 patients who underwent potentially curative surgery at the New England Deaconess Hospital was performed. In patients who had tumors with extramural BVI, there was a significant decrease in five-year actuarial survival compared with pa

[Coincidence of colonic lymphoma and gallstone ileus - case report].

INTRODUCTION: Primary colonic lymphoma is a very rare malignant disease of the gastrointestinal tract, accounting for 14% of all malignant diseases in this location. It is classified in the group of extranodal lymphomas; its long-term asymptomatic progression makes it different from common colorectal carcinomas making its diagnosis very difficult, more often accidental. Gallstone ileus is quite an uncommon complication of cholecystolithiasis diagnosed with difficulty. Up to 50% of cases are diagnosed during surgery. The obturated location depends on the size of the stone, location of the conju

[Scrotal pain as the presenting symptom of metastatic sigmoid cancer].

Colorectal cancer is one of the most common malignancies globally, and typical sites of metastasis include the liver and lungs; however, metastasis to the testes is exceedingly rare. We report a case where a 54-year-old male presented with acute scrotal pain and swelling. Scrotal ultrasound revealed a solid mass, and further investigations identified metastatic adenocarcinoma originating from the sigmoid colon. This case highlights the importance of considering malignancy in older patients with scrotal symptoms, especially when common causes have been ruled out.

[A Case of Sigmoid Colon Cancer with Liver Metastasis and Gastric Infiltration for Which Ileus Surgery Was the Impetus for Surgery].

The case is a woman in her 60s. Sigmoid colon cancer surgery, liver metastasis surgery, and adjuvant chemotherapy were performed at another hospital 2 years ago. Later, she developed a metastasis in her liver and was recommended surgery, but she refused treatment and was transferred. Her liver metastasis had invaded the stomach and formed a giant gastric ulcer. This time she had an adhesive ileus and underwent laparoscopic surgery at our hospital. At that time, we observed the state of liver metastasis and gastric infiltration by laparoscopy, so we thought that palliative surgery was possible
