Underutilization of the MSLT in sleepy patients with a short onset REM period (SOREMP) in the sleep clinic.

OBJECTIVE/BACKGROUND: A nocturnal sleep onset REM period (defined as REM onset latency ≤ 15 min; SOREMP) occurs rarely and research has shown that the phenomenon is specific for type 1 and 2 narcolepsy. However, little is known about the meaningfulness of the phenotype in general sleep clinic patients because those that exhibit the phenomenon often present with few traditional narcolepsy symptoms. As such, this study aimed to (1) evaluate the rate of eventual MSLT testing for those with a SOREMP on routine PSG when the phenomenon occurred in the absence of potential explanatory factors and (2)

Microinjection of the melanin-concentrating hormone into the sublaterodorsal tegmental nucleus inhibits REM sleep in the rat.

A study was performed on the effects of local microinjection of melanin-concentrating hormone (MCH) into the right sublaterodorsal tegmental nucleus (SLD) on sleep and wakefulness in rats prepared for chronic sleep recordings. MCH 200ng significantly decreased rapid-eye-movement sleep (REMS) time during the first and second 2-h of the recording period which was related to the reduction of the number of REMS periods and the increase of REMS latency. It is proposed that REMS inhibition was related to the direct deactivation of SLD glutamatergic neurons by the peptide.

Hypocretinergic and non-hypocretinergic projections from the hypothalamus to the REM sleep executive area of the pons.

Within the postero-lateral hypothalamus neurons that utilize hypocretin or melanin-concentrating hormone (MCH) as neuromodulators are co-distributed. These neurons have been involved in the control of behavioral states, and a deficit in the hypocretinergic system is the pathogenic basis of narcolepsy with cataplexy. In this report, utilizing immunohistochemistry and retrograde tracing techniques, we examined the hypocretinergic innervation of the nucleus pontis oralis (NPO), which is the executive site that is responsible for the generation of REM sleep in the cat. The retrograde tracer choler

The frequency of multiple sleep onset REM periods among subjects with no excessive daytime sleepiness.

The multiple sleep latency test (MSLT) is a valuable tool in the assessment of excessive daytime sleepiness (EDS). Additionally, multiple sleep onset rapid eye movement periods (SOREMPs) are a frequent occurrence in patients with narcolepsy. To date, however, few studies have evaluated the frequency of SOREMPs in a population of healthy control subjects. Subjects participating in a variety of sleep studies were screened with a nocturnal clinical polysomnogram, followed by the MSLT. Subjects were required to be drug free and have no sleep-related symptoms or medical or psychiatric conditions. O

Cholinergic and noradrenergic neurotransmission: impact on REM sleep regulation in healthy subjects and depressed patients.

The reciprocal interaction model of NonREM- and REM sleep regulation suggests that the cycling alternating pattern of Non-REM- and REM sleep is under the control of noradrenergic/serotonergic and cholinergic neuronal networks. This model was tested in healthy humans by administration of cholinergic agonists/antagonists and noradrenergic antagonists prior to or during sleep. Cholinomimetics like physostigmine, RS 86 and galanthamine provoked an earlier onset of REM sleep, whereas subchronic treatment with scopolamine, a cholinergic antagonist, only led to a heightening of REM density. Simultane

Distribution of choline acetyltransferase immunoreactive somata in the feline brainstem: implications for REM sleep generation.

In the present study we examined the distribution of cholinergic and catecholaminergic neurons, in the feline brainstem, as defined by choline acetyltransferase (ChAT) and tyrosine hydroxylase (TH) immunohistochemistry. In the dorsal tegmentum, ChAT immunoreactive neurons were distributed in the parabrachial area [the pedunculopontine group (PPG)] and along the medial adjacent central gray [the lateral dorsal tegmental group (LDT)]. The cholinergic neurons in the LDT area were larger than those in the PPG. When adjacent tissue sections were labeled with TH we noted extensive overlap between ca

The role of hypoxia in sleep, and its function as a precursory etiologic mechanism for the REM state: a hypothesis.

Sleep is a restorative hypoxic process annuling the effects of the potentially toxic products of aerobic metabolism. The REM state is characterized by the desynchronized homeostasis resulting from the hypoxia (hypoxemia and possible localized anoxia) which alerts the organism. Compensatory increase in respiration occurs, and the organism recovers from the threat of possible anoxic asphyxiation. The cyclic hypoxia (deep sleep) and desynchrony (REM state) and recovery from hypoxia repeat as the different stages of sleep till a homeostatically acceptable recovery from the metabolic burden of aero

Sleep apnoea occurring only in REM (possible relevance of the case for some cot deaths).

A case of sleep apnoea occurring only within REM sleep in a 57-year-old man is described. It was successfully treated with clomipramine for a period. The patient probably had persistent phrenic nerve damage which accounted for, or contributed to, the phenomenon. It is suggested that this mechanism may operate in some cot deaths.

Multimodal PET-MRI profiling predicts dementia with Lewy bodies in isolated REM sleep behaviour disorder.

Despite isolated REM sleep behaviour disorder (iRBD) reliably identifying individuals at high risk of developing Parkinson's disease (PD) or dementia with Lewy bodies (DLB), with more than 90% converting, the neurobiological markers of the conversion phenotype are not fully understood. Most previous studies have found univariate imaging biomarkers of phenoconversion, and most of these lack specificity for PD and DLB. Using a well-characterised iRBD cohort (N=21), receiving multiple PET and MRI scans, we aimed to assess if the inter-modality relationship can specifically predict conversion to P

Actigraphy Meets AI: A Digital Biomarker for Parkinson's Disease and Isolated REM Sleep Behaviour Disorder.

Isolated rapid eye movement (REM) sleep behaviour disorder (RBD) is an early stage of alpha-synucleinopathies, such as Parkinson's disease (PD). Actigraphy is a promising tool for assessing rest-activity patterns, however its diagnostic or predictive value in these disorders is understudied. This study evaluated AI-based models applied to actigraphy data to improve the detection of PD and RBD, additionally exploring their capability to capture early neurodegenerative patterns in isolated RBD. We trained and evaluated machine learning models with extracted actigraphy features, a convolutional n

Imperatorin restores behavioral and recognition memory deficits induced by REM sleep deprivation in male rats.

Many studies have shown the deleterious effects of rapid-eye movement (REM) sleep deprivation (SD) on cognitive functions and synaptic plasticity markers. On the other hand, imperatorin (IMP, extracted from the fruits of Angelica dahurica and Peucedanum praeruptorum) may exert significant pro-cognitive and neuroprotective effects. The present study aimed to explore the potential therapeutic effects of IMP on behavioral and molecular changes induced by REM SD, before or after training in the novel object recognition (NOR) memory test. REM SD was done for 24 h and IMP was orally administered at

Altered topological properties in PD patients with probable REM sleep behavior disorder through fuctional MRI.

PURPOSE: Rapid eye movement (REM) behavior disorder (RBD) is a common non-motor symptom in Parkinson’s disease (PD) patients. However, its pathogenesis remains unclear. Our study aimed to investigate the topological properties of the functional brain network in PD patients with probable REM sleep behavior disorder (PD-pRBD). METHOD: Using the RBD screening questionnaire, 32 PD patients without RBD (PD-nRBD) and 17 PD-pRBD patients were included. All participants underwent resting-state functional Magnetic Resonance Imaging (rs-fMRI) scan and clinical assessment. We compared the differences bet

Characterization of patients with isolated REM sleep behavior disorder and cerebrospinal fluid α-synuclein negativity.

Isolated REM sleep behavior disorder (IRBD) represents the prodromal phase of synucleinopathies. We investigated the characteristics of IRBD patients who tested negative for misfolded α-synuclein (αsyn) in CSF. CSF αsyn was analyzed by seed amplification assay (SAA) in 191 consecutive patients. Using logistic regression, demographic and clinical variables were compared between subjects with negative (αsyn-) and positive (αsyn+) αsyn. αsyn- occurred in 47 (24.61%) patients who were younger (p = 0.018), had less electromyographic REM sleep activity (p < 0.001), and more frequent normal DAT-SPECT

Speech Biomarkers From Smartphone Calls Track Progression in REM Sleep Behavior Disorder and Parkinson's Disease.

OBJECTIVE: This 24-month longitudinal study involving isolated rapid eye movement sleep behavior disorder (iRBD), early-stage Parkinson's disease (PD), and matched healthy control subjects aimed to assess whether acoustic speech features from real-world smartphone calls provide passive progressive biomarkers in synucleinopathies. METHODS: Participants underwent clinical assessments at baseline, 1, and 2 years. Speech was continuously captured during phone calls via a standardized smartphone application, segmented, and analyzed for speech impairment severity end points and key acoustic feature

The REM effect: How sleep stages influence learning strategies in complex reward-based decision-making.

Model-free (MF) and model-based (MB) learning strategies are complementary decision-making processes used in evaluating choices with potential rewards. Disorders involving compulsive behaviours (e.g., substance use, gambling) are suggested to emerge from an overreliance on MF learning, though the reasons for this bias remain unclear. Sleep disruptions, common in these disorders, could be a contributing factor, however no study has examined the impact of sleep and/or sleep loss on an individual's engagement of each strategy. Thus, this study examined the influence of sleep on MF/MB learning in

Plasma pTau181 and amyloid markers predict conversion to dementia in idiopathic REM sleep behaviour disorder.

Blood-based biomarkers for Alzheimer's disease (AD) pathology have been investigated intensively as markers for AD-related neurodegeneration. Comorbid AD pathology is common in dementia with Lewy bodies (DLB). Accordingly, we hypothesized that plasma biomarkers associated with AD pathology might be useful to predict DLB in a cohort of idiopathic/isolated REM sleep behaviour disorder (iRBD), an incipient synucleinopathy. The aim of this study was to determine whether plasma amyloid-β and pTau181 biomarkers can predict DLB. This longitudinal single-centre (Canada) cohort study included 158 indiv
