SLEEP AND NEURODEGENERATION: EXAMINING POTENTIAL PHYSIOLOGICAL MECHANISMS.

PURPOSE OF THE REVIEW: The purpose of this review is to provide an overview of potential mechanisms mediating the bi-directional relationship between sleep and neurodegenerative diseases such as Alzheimer's disease. We provide updates on previously proposed mechanisms and identify new mechanisms particularly concerning how sleep disturbances affect memory-related neural circuits. RECENT FINDINGS: In this review, we focus on the multiple mechanisms that potentially mediate the relationship between sleep and Alzheimer's disease. We present updates for previously hypothesized mechanisms such as

[Effects of continuous positive airway pressure on Cheyne-Stokes breathing in congestive heart failure].

To investigate the effects of a continuous positive airway pressure (CPAP) of 5 cmH2O for three months on sleep disordered breathing and cardiac function in congestive heart failure (CHF), we performed physiological tests before and after the CPAP treatment in six male patients with stable CHF, whose echocardiographic left ventricular ejection fraction (LVEF) was below 30%. The control polysomnography revealed that all six had Cheyne-Stokes breathing (with central sleep apnea in four, and central sleep hypopnea in two). Prior to the CPAP, the number of episodes of apnea/hypopnea per hour of sl

[Disorders of wakefulness and sleep in blind patients].

The goal of this paper was to summarize three studies focused on sleep/wake disorders in blind subjects. The first study was an epidemiology survey performed in 1073 blind subjects in comparison with non-blind controls. The blind had more episodes of insomnia and free running rhythms. They also took more sleeping pills and complained of more daytime somnolence. The seriousness of the sleep disorders was related to the seriousness of the blindness. In the second study, 78 blind children were compared with seeing children. They had more insomnia and more parasomnias but there was not any more fr

Urodynamic function during sleep-like brain states in urethane anesthetized rats.

The aim was to investigate urodynamic parameters and functional excitability of the periaqueductal gray matter (PAG) during changes in sleep-like brain states in urethane anesthetized rats. Simultaneous recordings of detrusor pressure, external urethral sphincter (EUS) electromyogram (EMG), cortical electroencephalogram (EEG), and single-unit activity in the PAG were made during repeated voiding induced by continuous infusion of saline into the bladder. The EEG cycled between synchronized, high-amplitude slow wave activity (SWA) and desynchronized low-amplitude fast activity similar to slow wa

Narcolepsy and the hypocretins.

Narcolepsy is a chronic neurologic disease characterized by excessive daytime sleepiness and one or more of three additional symptoms (cataplexy, or sudden loss of muscle tone; vivid hallucinations; and brief periods of total paralysis) related to the occurrence of rapid eye movement (REM) sleep at inappropriate times. The daytime sleepiness typically presents as a sudden overwhelming urge to sleep, followed by periods of sleep that last for seconds or minutes, or even longer. During daytime sleep episodes, patients may exhibit "automatic behavior," performing conventionalized functions (eg, t

Respiratory pauses and apnea during daytime sleep in normal infants during the first year of life: longitudinal observations.

Respiratory pauses (3-10 sec in duration), apnea (less than 10 sex in duration), and periodic respiration observed in thoracic respirograms were measured in 226 polysomnograms obtained on 17 normal infants during the first year of life. All subjects had one or more respiratory pauses in a majority of recordings; 35% had respiratory pauses in all recordings; 75% of respiratory pauses were associated with body movement. There is marked intersubject and even intrasubject variability in respiratory pause rates. The range of mean respiratory pause rates among subjects was 2.0 - 14.4/h, and for sing

Auditory Tones and Foot-Shock Recapitulate Spontaneous Sub-Threshold Activity in Basolateral Amygdala Principal Neurons and Interneurons.

In quiescent states such as anesthesia and slow wave sleep, cortical networks show slow rhythmic synchronized activity. In sensory cortices this rhythmic activity shows a stereotypical pattern that is recapitulated by stimulation of the appropriate sensory modality. The amygdala receives sensory input from a variety of sources, and in anesthetized animals, neurons in the basolateral amygdala (BLA) show slow rhythmic synchronized activity. Extracellular field potential recordings show that these oscillations are synchronized with sensory cortex and the thalamus, with both the thalamus and corte

The Impact of Sound on Electroencephalographic Waves during Sleep in Patients Suffering from Tinnitus.

Based on the knowledge that sensory processing continues during sleep and that a relationship exists between sleep and learning, a new strategy for treatment of idiopathic subjective tinnitus, consisted of customized sound stimulation presented during sleep, was tested. It has been previously shown that this treatment induces a sustained decrease in tinnitus intensity; however, its effect on brain activity has not yet been studied. In this work, we compared the impact of sound stimulation in tinnitus patients in the different sleep stages. Ten patients with idiopathic tinnitus were treated wit

[The role of opioid system in the regulation of the sleep-wakefulness cycle].

In chronic experiments influence of single administration of various doses of morphine (0.5, 2.0 and 3.0 mg/kg) on general behavior, structural organization of the sleep-waking cycle and some EEG and somato-vegetative parameters was studied in the cats. Intraperitoneal administration of morphine elicits numerous autonomic disorders, psychic derangement and complete deterioration of the sleep-waking cycle structure. The result certify that single administration of morphine induces activation of the opiate receptors in various structures of the brain, which entails deviations of behavioral and E

Sleep whistling and replaying a melody during N3 sleep: a new form of disorder of arousal.

Vocalizations are common during arousal disorders, but producing a recognizable whistled melody has not been described. A 30-year-old man with a history of childhood sleepwalking, occasional sleep talking and confusional arousals exhibited an unusual musical behavior during a spontaneous slow-wave arousal from N3 sleep. This behavior consisted of rhythmic upper-body movements that kept time with the whistling of the recognizable melody "Take Me Home, Country Roads." He had complete amnesia for the event but later recognized the melody as one he had recently learned. This observation expands th

Neuronal activity in the cholinoceptive basal forebrain of freely moving narcoleptic dobermans.

Cholinergic stimulation in the basal forebrain (BF) triggers cataplexy in canine narcolepsy. Extracellular single unit recordings in the BF were carried out in freely moving narcoleptic dogs to study the neuronal mechanisms mediating cataplexy induction in the BF. Among the 64 recorded neurons, 12 were wake-active, three were slow wave sleep (SWS)-active, 17 were wake-/REM-active, 11 were REM sleep-active, three were cataplexy-active, and the other 18 were state-independent. Systemic administration of physostigmine, a cholinesterase inhibitor, induces status cataplecticus, decreases SWS and in

Ambulatory 24 hour sleep-wake monitoring in narcolepsy-cataplexy compared to matched controls.

Twenty-four hour ambulatory sleep-wake recordings were made in 10 untreated patients with narcolepsy-cataplexy and 10 matched controls. Nocturnal sleep of narcoleptics was similar to descriptions of laboratory based recordings and was characterized by frequent sleep onset REM periods, high variability of REM latency, increased amounts of wakefulness after sleep onset, and low sleep efficiencies. Daytime portions in narcoleptics showed greater drowsiness and sleep than in controls with significantly higher amounts of stages 1A, 1B, 3, 4 and REM, more sleep episodes longer than 1 and longer than

Primary role of respiratory afferents in sustaining breathing rhythm.

We studied the effects on breathing rhythm of suppressing the major respiratory stimuli (wakefulness, vagal, peripheral and central chemoreceptors) in healthy, unanesthetized dogs. Respiratory frequency (f) was obtained with a pneumotachograph; the state of wakefulness (W) or sleep was determined by EEG and behavioral criteria. During quiet W, f averaged 17 breaths/min and minute volume of ventilation (VI), 8.4 l/min. In slow-wave sleep (SWS), f slowed to 14 breaths/min, and VI decreased to 6.8 l/min. Afferent vagal blockade during SWS slowed f to 4 breaths/min, due primarily to prolongation o

Sleep restoration by optogenetic targeting of GABAergic neurons reprograms microglia and ameliorates pathological phenotypes in an Alzheimer's disease model.

BACKGROUND: Alzheimer's disease (AD) patients exhibit memory disruptions and profound sleep disturbances, including disruption of deep non-rapid eye movement (NREM) sleep. Slow-wave activity (SWA) is a major restorative feature of NREM sleep and is important for memory consolidation. METHODS: We generated a mouse model where GABAergic interneurons could be targeted in the presence of APPswe/PS1dE9 (APP) amyloidosis, APP-GAD-Cre mice. An electroencephalography (EEG) / electromyography (EMG) telemetry system was used to monitor sleep disruptions in these animals. Optogenetic stimulation of GABA

Sleep as a Potential Biomarker of Tau and β-Amyloid Burden in the Human Brain.

Recent proposals suggest that sleep may be a factor associated with accumulation of two core pathological features of Alzheimer's disease (AD): tau and β-amyloid (Aβ). Here we combined PET measures of Aβ and tau, electroencephalogram sleep recordings, and retrospective sleep evaluations to investigate the potential utility of sleep measures in predicting in vivo AD pathology in male and female older adults. Regression analyses revealed that the severity of impaired slow oscillation-sleep spindle coupling predicted greater medial temporal lobe tau burden. Aβ burden was not associated with coupl

Sleep disturbances and Alzheimer's disease: a multiscale approach from exposome to neurobiology and precision medicine.

Sleep disturbances and Alzheimer's disease (AD) are interconnected public health challenges. However, the underlying mechanisms of their complex relationships remain elusive. Here, we propose a hypothetical integrative, stream-like model outlining how external and internal exposome factors accelerate brain aging, thereby exacerbating circadian dysregulation, orexin-mediated hyperexcitability, metabolic imbalance, and inflammaging. These changes can lead to increased sleep fragmentation and reduced slow-wave sleep, triggering widespread neuroinflammation, glymphatic dysfunction, and the accumul

A case of central sleep apnea strictly dependent upon REM-sleep.

Central sleep apnea (CSA) is characterized by the inability to generate regular breathing patterns as a result of the loss of metabolic drive and failure of respiratory muscle control. We present the case of a 54-year-old woman with a severe CSA strictly dependent upon REM-sleep. Extensive diagnostic workup excluded typical underlying causes and serological analysis revealed acute borreliosis infection. The severity of sleep apnea decreased after repeated polysomnographic studies without a specific therapy. CSA is usually associated with non-REM stages of slow-wave-sleep. This report illustrat

Treatment of narcolepsy with gamma-hydroxybutyrate. A review of clinical and sleep laboratory findings.

Previous studies on the effects of gamma-hydroxybutyrate (GHB) on the sleep and clinical response of patients with narcolepsy are reviewed. New information on 48 patients treated with GHB for as long as 9 years is presented. These studies indicate that 2.25 to 3.00 g of GHB, taken in conjunction with a low dose of a stimulant during the day, rapidly alleviate the symptoms of narcolepsy in most patients. Tolerance does not develop to this treatment regimen; neither have any patients discontinued the treatment because of side effects. In poor responders, daytime drowsiness and not cataplexy has

Acquired auditory agnosia in childhood and normal sleep electroencephalography subsequently diagnosed as Landau-Kleffner syndrome: a report of three cases.

AIM: We report three cases of Landau-Kleffner syndrome (LKS) in children (two females, one male) in whom diagnosis was delayed because the sleep electroencephalography (EEG) was initially normal. METHOD: Case histories including EEG, positron emission tomography findings, and long-term outcome were reviewed. RESULTS: Auditory agnosia occurred between the age of 2 years and 3 years 6 months, after a period of normal language development. Initial awake and sleep EEG, recorded weeks to months after the onset of language regression, during a nap period in two cases and during a full night of sle

Modulation and subcortical sleep signatures in Parkinson's disease under chronic STN-DBS using synchronized sensing and videopolysomnography.

Deep brain stimulation (DBS) of the subthalamic nucleus (STN) influences the sleep-wake cycle, yet in vivo evidence remains limited. We conducted a longitudinal, multimodal study in 38 patients with Parkinson's disease undergoing STN-DBS, integrating full-night video-polysomnography with synchronized subthalamic local field potentials (LFPs) 6 months post-surgery. Twenty patients completed pre/post video-polysomnography (19 with LFPs data). Slow-wave sleep increased after DBS, with no change of efficiency and fragmentation indices. Mean heart rate decreased in wake and sleep. REM-related motor
