Cutaneous silent period in patients with obstructive sleep apnea.

PURPOSE: Empirical evidence suggests that the duration of the Cutaneous Silent Period (CuSP) is significantly extended in pathological states such as Idiopathic Parkinson's Disease (IPD), Restless Legs Syndrome (RLS), and dystonia, all of which are correlated with the degeneration of dopaminergic neurons. This neurodegenerative process is a critical factor in the etiology of Obstructive Sleep Apnea (OSA). In light of this active participation, the present study sought to assess the CuSP in patients with OSA. METHODS: A cohort comprising forty individuals diagnosed with OSA and forty healthy c

Airway Anatomy and Surgical Management in Obstructive Sleep Apnea.

Obstructive sleep apnea (OSA) is a sleep-related breathing disorder characterized by recurrent collapse of a structurally vulnerable upper airway during sleep. Airway caliber and stability are influenced by multiple anatomical structures, including the nasal cavity, soft palate, lateral pharyngeal walls, tongue base, hyoid complex, and craniofacial skeleton. Detailed knowledge of these structures is therefore essential for rational surgical planning. This narrative review summarizes key anatomical features relevant to upper pharyngeal surgery, including nasal obstruction and the internal nasal

Obstructive Sleep Apnea and Type 2 Diabetes: An Update.

Obstructive sleep apnea (OSA) syndrome is a severe, debilitating, and pervasive sleep disorder. OSA mainly affects people with obesity, type 2 diabetes mellitus (T2DM), hypertension, and dyslipidemia and is strongly associated with cardiovascular complications. Based on the bidirectional relationship between T2DM and OSA, the latter represents a risk factor for the former, and, vice versa, people with T2DM have a high risk of OSA. Mechanical and hormonal factors, inflammatory mediators, and a dysregulated autonomic nervous system contribute to the mechanisms underlying the disease. Treatment o

Circadian Biology in Obstructive Sleep Apnea-Associated Cardiovascular Disease.

A dysregulated circadian system is independently associated with both Obstructive Sleep Apnea (OSA) and cardiovascular disease (CVD). OSA and CVD coexistence is often seen in patients with prolonged untreated OSA. However, the role of circadian dysregulation in their relationship is unclear. Half of the human genes, associated biological pathways, and physiological functions exhibit circadian rhythms, including blood pressure and heart rate regulation. Mechanisms related to circadian dysregulation and heart function are potentially involved in the coexistence of OSA and CVD. In this article, w

Integrative treatment of obstructive sleep apnea: principles and practice.

UNLABELLED: In the management of obstructive sleep apnea, the integrative concept of care is gathering momentum. This is an encouraging development to prevent deficiencies or errors due to a lack of professional collaboration since risks, benefits and alternatives of various techniques can be discussed candidly. However, while the deployment of a multidisciplinary team is encouraged, a scientific, evidence-based approach needs to be rigorously engendered to encompass the phenotypic heterogeneity of obstructive sleep apnea, including neurologic, metabolic, craniofacial, and myopathic endotypes.

The Interplay between Asthma, Obesity, and Obstructive Sleep Apnea.

The interrelationship between asthma, obesity, and obstructive sleep apnea (OSA) presents a critical area of investigation within sleep medicine, given the rising prevalence of these conditions globally. This article explores the multifactorial interactions among these three disorders that contribute to significant morbidity. Asthma, a chronic inflammatory condition of the airways, is one of the most common chronic respiratory conditions globally. Asthma in people with obesity is associated with poor asthma control, increased asthma severity, and an increased frequency of exacerbations. Obesit

Community awareness of obstructive sleep apnea (OSA): Saudi Arabia.

PURPOSE: OSA is a potentially serious medical condition associated with metabolic and cardiovascular hazards. The Saudi population remains inadequately informed, despite its increasing prevalence. The purpose of this study is to evaluate Saudi Arabian public awareness about OSA. METHODS: 992 people from all around Saudi Arabia participated in a descriptive cross-sectional study using a validated online questionnaire. Descriptive statistics were used to assess the demographic, health, and OSA knowledge data, and Pearson's Chi-Square test was used to look for correlations with the demographic c

Synchronous airway lesions in children with obstructive sleep apnea.

OBJECTIVE: Drug-induced sleep endoscopy is established for evaluating the upper airway in pediatric obstructive sleep apnea (OSA), but the role of comprehensive lower airway assessment remains uncertain. We aimed to determine whether synchronous airway lesions identified during microlaryngoscopy and bronchoscopy (MLB) are associated with OSA severity in children undergoing adenotonsillectomy (AT). STUDY DESIGN: Retrospective cohort study. SETTING: Tertiary care academic children's hospital. METHODS: We reviewed MLB findings documented by three fellowship-trained pediatric otolaryngologists

Roles of vitamins and nutrition in obstructive sleep apnea.

INTRODUCTION: Obstructive sleep apnea (OSA) is the most common sleep-related breathing disorder, with a prevalence that rises alongside with the increasing prevalence of obesity. OSA is characterized by a low-inflammatory state and is followed by cardiovascular and metabolic sequelae. Continuous positive airway pressure (CPAP) represents the cornerstone of treatment for severe OSA. However, poor compliance with CPAP treatment renders OSA treatment a challenging venture. Weight loss and exercise are recommended as adjunctive treatment options for OSA. Several diets have proven to facilitate wei

Pregnancy-onset obstructive sleep apnea and ambulatory blood pressure.

STUDY OBJECTIVE: Elevated nocturnal blood pressure (BP) increases the risk for hypertensive disorders of pregnancy (HDP). Though obstructive sleep apnea (OSA) increases the risk for HDP, data on OSA, and 24-hour ambulatory blood pressure monitoring (ABPM) in pregnancy are scarce. We aim to examine the BP profile of women with pregnancy-onset OSA. METHODS: Pregnant women with overweight/obesity and snoring were recruited at <13 weeks gestation and underwent level III home sleep apnea testing and 24-hour ABPM at enrollment and at 31-34 weeks' gestation. Women with OSA at enrollment were exclude

Incretins for management of obesity and obstructive sleep apnea.

INTRODUCTION: Recent trials have demonstrated the efficacy of incretin-based pharmacotherapy in obstructive sleep apnea (OSA) in obese patients. In addition to improving sleep indices, these agents also led to sustained weight loss and improvements in cardiometabolic profile. Given cardiovascular mortality remains the most common cause of death among patients with OSA, these agents represent a potential paradigm shift toward more holist management approach toward patients with OSA. AREAS COVERED: This article provides an overview of OSA pathophysiology, in particular, its overlapping risk fac

Regional brain iron mapping in obstructive sleep apnea adults.

PURPOSE: Obstructive sleep apnea (OSA) subjects show significant white matter injury, including myelin changes in several brain areas, potentially from impaired glial cells, contributing to increased iron levels that escalate neurodegeneration, but brain iron loads are unclear. Our aim was to examine regional brain iron load, using T2∗-relaxometry, in OSA adults before and after continuous positive airway pressure (CPAP) treatment over controls. METHODS: We performed T2∗-weighted imaging using a 3.0-T MRI scanner on 35 OSA adults, who were followed after 3- and 9- mo CPAP treatment, and 67 co
