Endovascular treatment of an intraosseous dural arteriovenous fistula of the greater wing of the sphenoid: A case report and literature review.

Intraosseous dural arteriovenous fistula (DAVF) of the greater wing of the sphenoid is extremely rare, and its endovascular treatment faces challenges due to numerous transosseous feeders and inaccessible dural sinuses. A successful case of transarterial embolization (TAE) is reported. A 63-year-old female presented with headache but no positive neurological signs. Computed tomography and computed tomography angiography revealed left temporal lobe edema and dilated intracranial veins. Digital subtraction angiography confirmed a left intraosseous DAVF of the greater wing of the sphenoid (Cognar

Management of Isolated sphenoidal aspergillosis: Case report and review of literature.

INTRODUCTION AND IMPORTANCE: Sphenoidal aspergillosis is an uncommon serious condition that could lead to a fatal outcome. There is dilemma in its management. Published data concerning this pathology are limited so we find interesting in adding our experience to the literature mass. In fact, this work focused on the description of the clinical features of this entity as well as its management by presenting a case report and reviewing literature. CASE PRESENTATION: We report a case of 53 years old male who presented to the outpatient department of our university teaching Hospital with isolated

Bilateral nasoseptal flaps for endoscopic endonasal transsphenoidal approach.

A nasoseptal flap is used to reconstruct defects in the skull base when cerebrospinal fluid (CSF) leaks after the endoscopic endonasal transsphenoidal approach (EETSA). We evaluated the usefulness of elevating bilateral nasoseptal flaps with the EETSA. Sixty-seven patients (71 procedures, including 4 revisions) underwent the EETSA with bilateral nasoseptal flap elevation. We retrospectively reviewed patients' medical records, including demographic data, surgical procedures, outcomes, and complications. The entire sellar floor was exposed after elevating bilateral nasoseptal flaps. We reconstru

Recurrent pleomorphic adenoma: unusual cause of isolated sphenoid sinus lesion.

Pleomorphic adenoma is the most common benign salivary gland tumour of the head and neck region, most commonly seen in parotid glands. These may arise also from minor salivary glands of the upper aerodigestive tract comprises the upper lip, cheek, floor of the mouth and rarely from mucoserous glands in the nasal cavity and paranasal sinuses. A middle-aged man, who had undergone surgery for a nasal mass 14 years ago, presented with headache and visual complaints from a recurrent mass originating from the sphenoid sinus. Isolated sphenoid sinus space-occupying lesions have always been a diagnost

A rare case of giant cell tumor involving the clivus resected through Le Fort I Osteotomy and median maxillotomy.

BACKGROUND: Giant cell tumors (GCTs) are bone tumors that seldom involve the skull. Skull GCTs preferentially occur in the sphenoid and temporal bones with few reported cases involving the clivus. Due to the rarity and complex location, surgical management is not well established for clival GCTs. CASE DESCRIPTION: A 49-year-old male presented with headaches and blurred vision in the right eye for 2 weeks. Computed tomography (CT) with contrast revealed a sellar mass eroding through the sphenoid sinuses with compression of optic chiasm. Biopsy was consistent with GCT. Patient underwent tumor r

[Endoscopic endonasal treatment of cerebrospinal fluid leak caused by clival fracture--a case report].

INTRODUCTION: Clivus is a central structure of the skull base located in the vicinity of the brainstem and vital brain vessels. Clival fractures are usually caused by a high-energy trauma. Cerebrospinal fluid leak is one of the most common complications. CASE REPORT: A middle-aged male sustained a mild head trauma, followed by a nasal cerebrospinal fluid leak. CT scan revealed the massive pneumocephalus and the fracture of the clivus in the posterior wall of the sphenoidal sinus. We performed an endoscopic endonasal surgery to seal the defect. DISCUSSION: Pneumatization of a sphenoidal sinus

[Functional endoscopic sinus surgery].

Treatment of chronic paranasal sinusitis (CPS) by functional endoscopic sinus surgery refers to removal of the pathologic tissue which interferes with the physiologic function of the sinuses in the ostiomeatal complex under direct vision by use of endoscopes to restore the ventilation and drainage of the sinuses. While the ventilation and drainage of the sinuses become normal, the lesions of sinus mucosa will recover spontaneously and the function of the sinuses will be restored. From May to August 1988, functional endoscopic sinus surgery was performed in 47 cases based on the operative metho

[Infectious aneurysms of the cavernous carotid artery].

We describe 3 patients who developed infectious aneurysms of the cavernous carotid artery. The aneurysms were due to sphenoidal sinusitis in two patients and due to endocarditis in one. The acute and septic onset of the cavernous sinus syndrome, suggested thrombophlebitis of the cavernous sinus in all 3 patients. The diagnosis was established by magnetic resonance imaging and magnetic resonance angiography. Therapeutic internal carotid artery occlusion was indicated for a fissuration of their aneurysm manifested (n=3) by an episode of epistaxis (n=2) and blood in sphenoid sinus (depicted by MR

The sphenoid sinus, the neglected nasal sinus.

O. E. Van Alyea stated that "The sphenoid sinus is the most neglected of the nasal sinuses." It is neglected by disease because of its location, by the physician because of its subtle symptoms when diseased, and the surgeon because of its inaccessability. An accurate knowledge and understanding of the anatomy of the area help the physician diagnose and manage problems in this area. Eleven cases are used to illustrate the wide spectrum of pathologic conditions in and about the sphenoid sinus. Only two of the patients were asymptomatic. All of the others had deep-seated, retrobulbar headaches. E

Assessment of Anatomical Uniqueness of Maxillary Sinuses through 3D-3D Superimposition: An Additional Help to Personal Identification.

Paranasal sinuses represent one of the most individualizing structures of the human body and some of them have been already analyzed for possible applications to personal identification, such as the frontal and sphenoid sinuses. This study explores the application of 3D-3D superimposition to maxillary sinuses in personal identification. One hundred head CT-scans of adult subjects (equally divided among males and females) were extracted from a hospital database. Maxillary sinuses were segmented twice from each subject through ITK-SNAP software and the correspondent 3D models were automatically

Isolated sphenoid sinus disease.

Isolated sphenoid sinus disease is an unusual entity that is encountered infrequently. With advances in antibiotic therapy and imaging techniques, the nature of isolated sphenoid sinus disease has significantly changed. More tumors and less inflammatory disorders are being encountered. We have retrospectively reviewed the medical records of patients who had undergone sublabial transseptal sphenoidotomy for isolated sphenoid sinus disease at Allegheny General Hospital for Pittsburgh between January 1985 and July 1989. Thirteen patients were identified with isolated sphenoid sinus disease who we

Paranasal sinus bony structures and sinus functioning during viral colds in subjects with and without a history of recurrent sinusitis.

OBJECTIVES/HYPOTHESIS: The objective was to assess the impact of ostial obstruction and anatomical variations on paranasal sinus functioning during viral colds with computed tomography (CT) in subjects with and without a history of sinusitis. STUDY DESIGN: Cross-sectional study. METHODS: Twenty-three volunteers with a history of recurrent sinusitis and 25 subjects without such history who had an early (symptoms for 2-4 d) natural cold were examined by taking viral specimens and CT scans and recording symptoms. The pathological sinus changes in the CT scans were scored, and several paranasal

Surgical reconstruction for spheno-orbital meningioma extending into the sphenoid sinus with hyperostosis.

Spheno-orbital meningiomas (SOMs) are complex tumors that grow and extend into nearby structures. SOM tumor growth is often associated with hyperostosis caused by tumor cell infiltration and bone alteration. We describe the case of a 64-year-old man with SOM that extended into the sphenoid sinus without a direct connection between the intracranial and extracranial lesions. This report emphasizes the importance of identifying the growth patterns of SOMs and assessing the paranasal sinuses adjacent to the hyperostotic orbit walls from preoperative images.

Maxillary sinus osteoma: two cases and review of the literature.

Osteomas are benign tumours characterized by proliferation of compact or cancellous bone. The most common site is the mandible, followed by the sinuses. These tumours are slow-growing, usually asymptomatic, and are generally discovered as incidental radiological findings. Osteomas occur commonly in frontal sinus, followed by the ethmoid and maxillary sinus, and very rarely in the sphenoid sinus. Symptoms arise when osteomas obstruct the ostium of the sinus or impinge on adjacent orbital or intracranial structures. Two cases of maxillary sinus osteoma are presented along with a review of the li

Solitary fibrous tumor arising from the sphenoid sinus.

Solitary fibrous tumor (SFT) is an uncommon neoplasm that usually arises from the pleura. To our knowledge, only 30 cases of SFTs in the nasal cavity and paranasal sinuses have been reported in the literature. We describe an SFT that arose from the right sphenoid sinus and extended to the nasal cavity and epipharynx. The tumor was completely removed by endoscopic sinus surgery without complication. The patient is taking an uneventful course without any evidence of recurrence of the disease 8 months after surgery now.

Maxillary osteonecrosis after radiotherapy for nasopharyngeal carcinoma.

A case of extensive osteoradionecrosis of the posterior maxillae, pterygoid plates and floor of the sphenoid sinuses is reported. The patient received more than one full course of radical radiotherapy for nasopharyngeal carcinoma. Although there was complete remission of the tumour, she suffered from repeated episodes of gum bleeding which was found to be due to oroantral fistulas and severe dental decay. The large fraction dose and total dose, severe dental caries and xerostomia are the most likely contributory causes to this unusual complication in our patient.

Updating nasalisation: From concept to technique and results.

Nasalisation is a surgical technique proposed for the treatment of nasal polyposis in 1995. The technique as initially described was based on large opening of the maxillary, sphenoidal and frontal ostia and resection of the middle turbinates, with the aim of resecting the mucosa of the lateral masses of the ethmoid as completely and safely as possible. Recent findings on the evolution and development of the nose and sinuses and sinus physiology allow both the concept and the technique of nasalisation to be updated. According to evo-devo theory, the ethmoid is not a paranasal sinus but the skul

[Pneumosinus dilatans--general considerations and a case report].

The term "pneumosinus dilatans" was coined by Benjamin in 1918 in his description of ballooning hyperpneumatization of the frontal sinus. The term was use later on for excessive pneumatization of individual or several paranasal sinuses, with or without local compressive signs. Inspite of numerous publications, the phenomenon of pneumosinus dilatans of paranasal sinuses never became particularly popular. The diagnosis of sphenoid pneumosinus dilatans in particular was difficult to establish by conventional radiological methods and demonstration of the indirect signs of a compressive mechanism w
