Cystic lesions of the spinal cord (syringomyelia) may occur after spinal cord injury. Posttraumatic syringomyelia may result in a myelopathy causing symptoms of sensory and motor loss, as well as worsening spasticity, pain, hyperhidrosis, and autonomic dysreflexia. Shunting of the cyst cavity along with untethering of the scarred spinal cord is widely accepted as the treatment of choice. However, the long-term stabilization of the progressive myelopathy caused by a posttraumatic cyst is suboptimal because of arachnoidal rescarring, shunt tube blockage, and cyst reexpansion. A new neurosurgical…

These experiments were designed to determine whether transplants of fetal spinal cord tissue into lesioned spinal cord in newborn rats provide a terrain that supports the growth of serotonergic (5-HT) axons across the site of the lesion. Although descending serotonergic axons can regenerate after chemical lesions in adult animals, they show little regrowth after surgical lesions. In newborn animals, 5-HT axons do not regrow after either chemical or mechanical lesions since the axotomized raphe-spinal neurons die. After partial spinal cord lesions made in developing animals, immature axons can …

CONTEXT: Spinal Cord Injuries (SCI) greatly affect physical independence, especially in older Veterans. While the Functional Independence Measure (FIM) is widely used in rehabilitation, it lacks consideration of SCI-specific needs such as ventilation, wheelchair use, and bowel/bladder management. The Spinal Cord Independence Measure-Version III (SCIM-III) was developed to address these gaps, but its effectiveness has not been studied in the veteran population, which is typically older, predominantly male, and has higher comorbidity rates. This study examined the concurrent validity, responsive…

OBJECTIVES: To observe the effect of moxibustion on urinary dynamics and lipid oxidation in spinal cord tissue of rats with urinary retention after spinal cord injury (SCI), so as to explore its possible mechanism in improving urinary retention after SCI.
METHODS: Female Wistar rats were randomly divided into sham operation, model and moxibustion groups, with 10 rats in each group. SCI induced urinary retention model was established by using Allen's method. Moxibustion was applied to "Shenque" (CV8), "Guanyuan"(CV4) and "Zhongji" (CV3) for 20 min, once daily for 14 d. The maximum bladder volu…

The effectiveness of spinal cord stimulation for control of spasticity was studied in 59 spinal cord injury patients. SCS was markedly or moderately effective in reducing spasticity in 63% of the patients. We found that control of spasticity by SCS was not correlated with the severity of spasticity, the type of spasticity (flexor or extensor), or the ability to ambulate. However, stimulation was more effective in patients with incomplete cervical lesions than in complete cervical lesions. Stimulation below the lesion was more effective than above. We conclude that SCS was effective when electr…

The fifty-six spinal cords, which were randomly removed from the patients at the First Department of Pathology, National Defence Medical College, were histopathologically examined, to detect masked spinal lesions and some age-related changes. Pathological lesions were found in 55.4% of the cases, about half of which were opportunistic findings and had not caused any clinical symptoms. In these opportunistic lesions, benign neurogenic tumors (one cellular ependymoma from the central canal and ten schwannomas arose from anterior or/and posterior rootlets in 7 cases) were found predominantly in t…

It has been established that coordinated arm and leg (A&L) cycling facilitates corticospinal drive and modulation of cervico-lumbar connectivity and ultimately improves overground walking in people with incomplete spinal cord injury or stroke. This study examined the effect of noninvasive transcutaneous spinal cord stimulation (tSCS) on the modulation of cervico-lumbar connectivity. Thirteen neurologically intact adults participated in the study. The excitability of the Hoffmann (H) reflex elicited in the soleus muscle was examined under multiple conditions involving either the arms held in a …

BACKGROUND: Spinal cord stimulation (SCS) is an efficacious, safe, and well-documented procedure for treating chronic refractory pain syndromes. We report a rare case of spinal cord injury (SCI) after SCS implantation in a 54-year-old woman.
CASE REPORT: A 54-year-old woman underwent reimplantation of a new SCS following an acute L2 compression fracture that led to significant pain and dysesthesia. The intraoperative course was uncomplicated. Immediately postoperatively, the patient experienced an inability to move her right leg, with magnetic resonance imaging showing epidural fluid collecti…

Spinal cord injury (SCI) is a severe central nervous system injury that can cause sensory or motor dysfunction. Although mortality rates for people with spinal cord injuries have dropped dramatically with advances in medicine, chronic long-term sequelae after SCI persist. The most bothersome problems reported by patients include pain, spasticity, urinary dysfunction, and loss of motor function. Thus, quality of life (QoL) is an essential issue in chronic SCI. Spinal cord stimulation (SCS) applies an adjustable, nondamaging electrical pulse that can reduce uncomfortable comorbidities and improv…

STUDY DESIGN: Retrospective study.
OBJECTIVES: The objective of this study was to examine whether the morphology of the compressed spinal cord reflects spinal cord dysfunction in cervical spondylotic myelopathy (CSM).
SUMMARY OF BACKGROUND: Segmental levels responsible for clinical symptoms in CSM are usually diagnosed by neurological examination and imaging studies such as magnetic resonance imaging (MRI). Electrophysiological examination makes it possible to assess the spinal cord function quantitatively. There are few detailed studies about correlation between intraoperative spinal cord-e…

BACKGROUND: The Walking Index for Spinal Cord Injury II (WISCI II) is a hierarchical scale that measures improvements in walking following spinal cord injury (SCI). The WISCI II has good face validity, concurrent validity, and reliability following acute SCI; however, psychometric properties need to be determined for chronic SCI. Because prior studies have demonstrated a relationship between lower-extremity motor scores (LEMS) and walking, outcome measures for walking should demonstrate a linkage between the underlying impairment (weakness) and walking-convergent validity.
OBJECTIVE: To deter…

The possibility that nitric oxide is somehow involved in the early bioelectrical disturbances following spinal cord injury in relation to the later pathophysiology of the spinal cord was examined in a rat model of spinal cord trauma. A focal trauma to the rat spinal cord was produced by an incision of the right dorsal horn of the T 10-11 segments under urethane anaesthesia. The spinal cord evoked potentials (SCEP) were recorded using epidural electrodes placed over the T9 and T12 segments of the cord following supramaximal stimulation of the right tibial and sural nerves in the hind leg. Traum…

This report describes a technique for exposing the ventrolateral quadrant of the spinal cord through an extended posterolateral approach that can be used in both cervical and thoracic regions. The surgical technique includes the following: 1) a midline skin incision with a transverse extension at the level of pathology; 2) unilateral division and retraction of the paraspinous muscles; 3) laminectomy and unilateral removal of facets and pedicles; 4) dural incision over the dorsal root entry zone; 5) multilevel division of the ipsilateral dentate ligaments; and 6) elevation and rotation of the s…

Idiopathic ventral spinal cord herniation is a rare condition that has been increasingly reported in the last decade. The natural history and optimal management have yet to be defined. Therefore, debate exists regarding the pathogenesis and surgical management of this condition. The purpose of this review article is to further educate neurosurgeons about the surgical techniques and outcomes associated with treating this rare and often misdiagnosed condition.

The clinical sign of ataxia is related to several neurological diseases and is seen in conjunction with paresis in dogs with spinal cord injury (SCI). Endeavours to objectify canine spinal ataxia in SCI remain limited. The aim of this clinical study was to determine and quantify differences between gait characteristics of ataxic dogs with thoracolumbar myelopathy and healthy control dogs using a computer-and treadmill-based gait analysis system. Five dogs with spinal ataxia and six healthy dogs underwent video-and computer-assisted gait analysis while walking on a four-ground reaction force pl…

Primary malignant melanoma of the intramedullary region of the spinal cord has rarely been reported in the literature. These tumors can have variable appearance on magnetic resonance imaging (MRI) due to different extents of melanin and hemorrhage. Histopathologic confirmation and a comprehensive workup to rule out extra-spinal melanoma are required to make definitive diagnosis. We present a case of a patient diagnosed with primary intramedullary spinal melanoma in his lower thoracic spinal cord who was effectively treated with surgical resection, adjuvant radiation, and adjuvant immunotherapy…

BACKGROUND: Our study shows that a membrane sealant/fiber fusogen polyethylene glycol (PEG) applied immediately on a sharp section of the spinal cord can mend the cord and lead to exceptional levels of motor recovery, with some animals almost normal.
MATERIALS AND METHODS: Before deploying such technology in man, long-term data in large mammals that exclude delayed complications (e.g., central pain), confirm the stability of motor recovery, and provide histological evidence of fiber regrowth are necessary. Here, we provide such evidence in dogs followed up over 6 months and in 2 cases up to 1…

Cervical spinal cord lesions in the anterior spinal artery syndrome were delineated on magnetic resonance images (MRI) in four patients. The lesion was always seen anteriorly in the cervical cord. On T2-weighted images, the lesions appeared hyperintense relative to the normal spinal cord, while on T1-weighted images, two chronic lesions appeared hypointense, with local atrophy of the cord. In one case, repeated T1-weighted images showed no signal abnormality 4 days after the ictus, but the lesion became hypointense 18 days later, when contrast enhancement was also recognized after injection of…
