Clinical outcomes of cervical spinal cord injuries without radiographic evidence of trauma.

We investigated 33 cervical spinal cord injury patients (25 males and eight females) without bony injury. Patients whose neurologic recovery had reached a plateau and who had evidence on imaging of persistent spinal cord compression were considered candidates for surgical decompression. When imaging did not show spinal cord compression or patients were maintaining a good neurologic recovery from the early days after injury, we pursued conservative treatment. Age at injury varied from 20 to 76 years (mean, 55.6). Average follow-up was 31 months. Twelve patients were treated conservatively (Grou

[Injuries of the spinal cord and vertebral column in children and adolescents].

A review of 127 pediatric patients admitted to the Department of Neurosurgery between 1965 and 1993 because of spinal cord and vertebral column injuries is presented. The aim of this paper is to relate clinical appearance and age of the patient. Patients were subdivided into two age groups on the grounds of skeletal development features. The first included 22 patients (3-14 years of age), the second one included 105 patients (15-18 years of age). Accident type, injury mechanism, level of the spinal injury and type of treatment differed in these groups as was found clinically and statistically.

Occupational spinal cord injury: demographic and etiologic differences from non-occupational injuries.

Most spinal cord injuries (SCIs) are the result of motor vehicle accidents (MVAs). There are no published reports specifically addressing SCIs that occur in the workplace. We report a cohort survey study examining the frequency and etiology of SCI in the workplace. Through a cooperative program, an evaluation of all SCIs seen in Colorado during a 5 1/2-year period (January 1, 1986 through June 6, 1991) is presented. We report a detailed analysis of specific data of all SCIs occurring in the workplace. There were a total of 566, with 74 (13.1%) due to injuries that occurred during the course of…

Esophagitis and esophageal motor abnormalities in patients with chronic spinal cord injuries.

To investigate the frequency and severity of esophagitis and esophageal dysmotility in patients with chronic spinal cord injury (SCI), 46 males with chronic SCI completed a questionnaire regarding gastrointestinal symptomatology. Eleven of these patients subsequently underwent upper gastrointestinal (GI) endoscopy with esophageal biopsies and 10 of the 11 also had esophageal motility studies. A significantly higher percentage of SCI patients experienced heartburn (SCI 61%; controls (C) 40%), esophageal chest pain (SCI 33%; C 6.4%), and intermittent dysphagia (SCI 30%; C 8.5%). Forty-five perce

Neglected spinal cord, brain stem and musculoskeletal injuries stemming from birth trauma.

OBJECTIVE: A review of the medical literature was undertaken to determine cause, diagnosis, prognosis, treatment and prevention of injuries resulting from birth trauma. The primary focus was the neonate, though infant, child and adult were also considered because the effects of birth trauma can be life-long. DATA SOURCE: A compilation of case studies and review articles were extracted from numerous "MEDLINE" literature searches. Key Terms included: Birth Trauma, Central Nervous System Injuries, Musculoskeletal Injuries, Stillbirth, Sudden Infant Death Syndrome (SIDS), Cerebral Palsy, Brachial

[25 years Heidelberg Rehabilitation Center for Spinal Cord Injuries--retrospect and prospects].

In the framework of a commemorative gathering and scientific symposium in November 1991, an account was given of the 25 years of service of the Rehabilitation Centre for Spinal Cord Injured Persons of the Heidelberg Orthopaedic University Hospital Foundation. Tribute was at the same time paid to the lifework of Kurt Lindemann, former director of the Orthopaedical University Hospital and rector magnificus of the Heidelberg Ruperto Carola University, in memory of the 25th anniversary of his death in 1966. Encouraged by the father of SCI rehabilitation Sir Ludwig Guttmann, of Stoke Mandevillehosp

Neurologic recovery following injuries to the cervical spinal cord and nerve roots.

From this literature, we have made the following conclusions: (1) Complete cord lesions do not recover cord functional motor control. (2) Complete lesions recover one nerve root level of function at the fracture site. (3) Partial lesions recover partially. (4) The less the injury, the greater the recovery. (5) Brown-Sequard lesions recover more than central cord syndromes, which recover more than anterior cord syndromes. (6) Reduction of dislocated facets facilitates nerve root recovery. (7) Better documentation of specific pathology and recovery rates are necessary to determine the surgical b

Problems in the management of combined brachial plexus and spinal cord injuries.

Despite the fact that spinal cord injury is rarely seen without other injuries, the association with brachial plexus lesions has been very infrequently reported. Eleven cases of such injuries are reported, six as a result of motorcycle accidents. The brachial plexus lesion is often overlooked because clinical attention is naturally directed to the spinal cord. The patient often is unable to give a coherent history due to head injury, and the paralysis may be thought to be due to the spinal cord lesion or the head injury. The brachial plexus lesion greatly complicates rehabilitation and require

[Critical notes on the transport of patients with cervical spinal cord injuries].

Patients with a cervical spine injury who cannot be taken to a special centre immediately after the accident should be transferred to such a centre only when one is sure that no additional transportation trauma will be produced. The fracture cervical spine must be adequately stabilized during transportation. The partial oxygen pressure must not be lower than 60 mm Hg and one should wait at least 1 week post removal of tracheostomy tube. An acute stress ulcer (melaena) also has to be excluded.

Targeting Spinal Interneurons for Respiratory Recovery After Spinal Cord Injury.

Spinal interneurons (SpINs) are pivotal to the function of neural circuits, orchestrating motor, sensory, and autonomic functions in the healthy, intact central nervous system. These interneurons (INs) are heterogeneous, with diverse types contributing to various neural systems, including those that control respiratory function. Research in the last few decades has highlighted the complex involvement of SpINs in modulating motor control. SpINs also partake in motor plasticity by aiding in adapting and rewiring neural circuits in response to injury or disease. This plasticity is crucial in the

Charcot Spinal Arthropathy Secondary to Spinal Cord Injury - A Case Report.

INTRODUCTION: Pressure ulcers are common complications in immobilized patients, typically occurring over bony prominences. However, in rare instances, particularly in high-risk individuals with hematologic disorders such as sickle cell disease, ulcers may develop at unusual anatomical sites due to external medical devices. This case highlights a rare presentation of a vulvar pressure ulcer caused by prolonged application of a Thomas splint in a patient with homozygous sickle cell disease (HbSS). CASE REPORT: A 24-year-old woman with HbSS was admitted with a closed distal femoral shaft fractur

Tophaceous spinal gout in a young man with complete spinal cord injury.

Gout is a common inflammatory arthritis, caused by monosodium urate crystal deposition in the joints. Gout typically affects peripheral joints but less commonly it affects the spine. We report a case of a man in his 30s with chronic gout who presented acutely with thoracic back pain and flaccid paralysis of his lower limbs. CT showed abnormal facet joints at T8/9 with lucency suggestive of septic arthritis and MRI showed signal changes suggestive of epidural collection. The patient underwent emergency spinal surgery to evacuate the collection, followed by broad-spectrum antibiotic treatment. C

Developing a predictive model for spinal shock in dogs with spinal cord injury.

BACKGROUND: Reduced pelvic limb reflexes in dogs with spinal cord injury typically suggests a lesion of the L4-S3 spinal cord segments. However, pelvic limb reflexes might also be reduced in dogs with a T3-L3 myelopathy and concurrent spinal shock. HYPOTHESIS/OBJECTIVES: We hypothesized that statistical models could be used to identify clinical variables associated with spinal shock in dogs with spinal cord injuries. ANIMALS: Cohort of 59 dogs with T3-L3 myelopathies and spinal shock and 13 dogs with L4-S3 myelopathies. METHODS: Data used for this study were prospectively entered by partner…

Spinal Cord Injury From Spinal Tumors: Prevalence, Management, and Outcomes.

BACKGROUND: With the aging population in the United States, it can be anticipated that the prevalence of spinal cord injuries (SCIs) and cancer will increase. Primary or metastatic spine tumors sit at a unique intersection of these 2 realms. Our objective was to evaluate the prevalence, outcomes, and complications after the management of SCI arising from spinal tumors. METHODS: In the present retrospective evaluation, all patients with SCI and a diagnosis of primary or metastatic spinal cancer who had been admitted to the inpatient rehabilitation unit at a level 1 trauma center from January 2…

[Spontaneous spinal epidural hematoma as cause of incomplete spinal cord injury].

Spontaneous spinal epidural hematomas are rare but are of differential diagnostic importance due to the potentially dramatic progression through to irreversible neurological deficits. At the beginning the clinical symptoms are non-specific and the development of neurological deficits leads to the diagnosis. We present the case of a 73-year-old female patient who initially reported uncharacteristic neck pain and developed incomplete quadriplegia during the next day as well as the case of a 78-year-old male patient, who complained of acute back pain and developed paraplegia a short time afterwar

Influence of spinal reflexes on the locomotor pattern after spinal cord injury.

In complete spinal cord injured (cSCI) subjects a shift from dominant early (60-120ms latency) to dominant late (120-450ms latency) spinal reflex (SR) components occurs over time after injury. This shift is assumed to reflect a spinal neuronal dysfunction below the level of a spinal lesion. The neuronal pathways of SR are suggested to be closely connected with spinal locomotor circuits. The aim of this study was to explore the influence of the two SR components on the electromyographic (EMG) pattern induced by assisted locomotion in cSCI subjects. Leg muscle EMG activity was analysed during as
