Predicting Liver Cancer Risk Using Comprehensive Medical History.

BACKGROUND AND OBJECTIVES: Liver cancer mortality is rising faster than any other cancer, significantly impacting life expectancy due to its … in diseases were strongly linked to a higher risk of liver cancer, such as liver cirrhosis, chronic steatorrhea, and neoplasms of unknown behavior in the genitourinary organs, each with an OR greater than 8. Digestive disorders, including pancreatic disorders and chronic hepatitis B and C, were also associated with an increased risk of liver cancer (OR > 4). …

High prevalence of gastrointestinal symptoms in patients with primary Sjögren's syndrome cannot be attributed to pancreatic exocrine insufficiency.

INTRODUCTION: Pancreatic exocrine insufficiency (PEI) results in maldigestion of fat, leading to steatorrhea, malabsorption and weight loss. Sjögren's syndrome (SS) is a chronic autoimmune rheumatic disease with unknown etiology. The exocrine pancreas and the salivary glands are functionally and histologically comparable, and pancreatic dysfunction in SS has been hypothesized. METHODS: Patients were recruited from the Department for Rheumatology at the Karolinska University Hospital in Stockholm, Sweden, between June and December 2019. PEI was assessed by fecal elastase-1 (FE-1) and 13C-mixed…

Bruising as the first sign of exocrine pancreatic insufficiency in infancy.

Exocrine pancreatic insufficiency is an important cause of chronic malnutrition, secondary to maldigestion-malabsorption, which can be caused in children especially by cystic fibrosis, but also by other much rarer diseases. The case of a 6 months and 3 weeks old male pediatric patient is reported, who was admitted to the clinic for head and forearms bruising. Laboratory findings identified vitamin K deficiency as the cause of the cutaneous hemorrhagic syndrome. Further investigations revealed association of steatorrhea (which is a marker of fat malabsorption), iron-deficiency anemia and hypovi…

Clinical manifestations and enzymatic activities of mitochondrial respiratory chain complexes in Pearson marrow-pancreas syndrome with 3-methylglutaconic aciduria: a case report and literature review.

UNLABELLED: Pearson marrow-pancreas syndrome (PS) is a rare mitochondrial disorder. Impaired mitochondrial respiratory chain complexes (MRCC) differ among individuals and organs, which accounts for variable clinical pictures. A subset of PS patients develop 3-methylglutaconic aciduria (3-MGA-uria), but the characteristic symptoms and impaired MRCC remain unknown. Our patient, a girl, developed pancytopenia, hyperlactatemia, steatorrhea, insulin-dependent diabetes mellitus, liver dysfunction, Fanconi syndrome, and 3-MGA-uria. She died from cerebral hemorrhage at 3 years of age. We identified a …

[Rapid adaptation response after extensive resection of the small intestine: case report].

INTRODUCTION: Short-gut syndrome is defined as any of the malabsorptive conditions resulting from massive resection of small bowel. Clinically the syndrome is manifested by malnutrition, weight loss; steatorrhea, and diarrhea that occur because of decreased absorptive capacity. Intestinal failure is the end result of several complex interacting mechanisms relating in: reduced enterocyte mass, short small bowel length (8-15% of normal) with consequent reduced mucosal contact time for absorption, massive proximal loop dilatation with poor propulsion. Intraluminal stasis and bacterial overgrowth …

OBJECTIVES: To investigate the intestinal absorptive processes in children with HIV infection before and after treatment with combination th … virologic and immunologic responses. RESULTS: In all, 9 children had carbohydrate malabsorption, 3 steatorrhea, 2 protein loss, and 7 iron deficiency. Most tests produced normal results after 3 months of therapy, and all abnormalities were abolished 6 months after institution of combination therapy. Mean results of each of four absorption tests were signific …

Utility of Fecal Elastase-1 in Estimating Exocrine Pancreatic Function in Cystic Fibrosis: A Scoping Review.

BACKGROUND: In people with cystic fibrosis (pwCF), identification of exocrine pancreatic insufficiency (EPI) is essential to prevent steatorrhea and, if not managed actively, can lead to catastrophic consequences. Fecal elastase-1 (FE-1) is a widely used test to screen for EPI in cystic fibrosis (CF). Once thought permanent, some patients with EPI on CF transmembrane conductance regulator (CFTR)-directed therapies (modulators) have been noted to show improvement in EPI. Here, we evaluated the utility of FE-1 in pwCF. METHODOLOGY: We performed a scoping review and searched several databases fo…

BACKGROUNDS/AIMS: Groove pancreatitis (GP) is a seldom encountered form of chronic pancreatitis characterized by inflammation of the pancrea … creas, with ongoing alcohol consumption (n = 3) or smoking (n = 4). New-onset diabetes mellitus and steatorrhea were observed in 42.85% (6/14) and 21.42% (3/14) of patients, respectively. Furthermore, 71.42% (10/14) reported weight gain, with a median increase of 13.5 kg (range 5.00-22.75 kg). CONCLUSIONS: PD for GP offers substantial long-term pain relief w …

The fat-enriched diets were well tolerated with no manifestations of steatorrhea in evidence. Update on Bile Acid Malabsorption: Finally Ready for Prime Time?

PURPOSE OF REVIEW: To provide an update on the prevalence, pathophysiology, disease associations, and treatment options for bile acid malabsorption (BAM). RECENT FINDINGS: •Molecular mechanisms-BAs prevent water reabsorption and increase water secretion by intracellular mediators, increasing aquaporin channels and intracellular permeability. •Inflammatory bowel disease-new molecular mechanisms of BAM are identified in patients without ileal disease, including changes in expression of ileal BA transporter and nuclear receptors involved in BA homeostasis. •Microscopic colitis-BAM is one of the

Management of bile acid malabsorption using low-fat dietary interventions: a useful strategy applicable to some patients with diarrhoea-predominant irritable bowel syndrome?

This study evaluates the efficacy of low-fat dietary interventions in the management of gastrointestinal (GI) symptoms due to bile acid malabsorption. In total, 40 patients with GI symptoms and a 7-day (75)selenium homocholic acid taurine (SeHCAT) scan result of <20%, were prospectively recruited and then advised regarding a low-fat dietary intervention. Before and after dietary intervention, patients rated their GI symptoms using a 10-point numerical scale, and recorded their intake in 7-day dietary diaries. After dietary intervention, the median scores for all GI symptoms decreased, with a s

Characterization of a PNLIP variant identified in Amish pediatric patients with congenital pancreatic lipase deficiency.

Congenital pancreatic lipase deficiency (CPLD, OMIM #614338) is a rare exocrine pancreatic disorder presenting in late infancy with steatorrhea, fat-soluble vitamin deficiency, and low pancreatic lipase activity. Variants of the pancreatic triglyceride lipase (PNLIP) gene have been linked to CPLD. Six children from four Amish families exhibited CPLD symptoms, and two had decreased fecal elastase levels when tested. A novel homozygous PNLIP variant, c.869G>A (p.S290N), was identified in these children. This study aimed to characterize the PNLIP variant to understand its mechanism underlying CPL…
