Recurrent sternal thymoma after thymectomy by video-assisted thoracic surgery.

An 81-year-old man underwent thymectomy by video-assisted thoracic surgery for a mediastinal tumor. The pathological diagnosis was Masaoka stage II type B3 thymoma. Thirty-six months later, he presented with a growing mass on his anterior chest wall. Computed tomography showed a lobulated tumor in the sternum with bone destruction. Positron-emission tomography-computed tomography showed a maximal standardized uptake of 12.3 in the tumor. Core needle biopsy confirmed a metastatic sternal tumor from a type B3 thymoma. We partially resected the sternum and reconstructed the defect using an expand…

Quantification of cephalocaudal progression of jaundice in preterm infants.

BACKGROUND: The cephalocaudal progression (CCP) of neonatal jaundice is a well-known phenomenon, but quantitative information on CCP in preterm infants is absent. In this study, CCP was quantified in preterm infants as a function of postnatal age and body location. METHODS: 5.693 transcutaneous bilirubin (TcB) measurements were performed in 101 preterm infants from birth until postnatal day seven at five body locations (forehead, sternum, hipbone, tibia, ankle). Multi-level linear regression analysis was performed to evaluate the CCP as a function of body location and postnatal age. TcB measu…

Pattern and distribution of bone metastases in common malignant tumors.

BACKGROUND: Bone scan is a sensitive but not specific method for evaluation of bone metastases. However, the clinical data and the pattern o … cancers. Except for the spine, common locations of bone metastases from breast cancer were ribs and sternum. In prostate cancer, the most frequent site were spine and pelvis, with similar incidences. In lung cancer, ribs followed by spine were most frequent sites of bone metastases. 97 (60.6%) of the cancer patients studied had symptoms of bone pain. The …

Model Comparison for Defining the Trunk for Dynamic Testing in a Motion Capture Lab.

Evaluation of trunk stability is common during return-to-activity assessments. While the use of motion capture for assessment of movement quality is also becoming more popular, it is critical that the trunk is modeled similarly across participants. The purpose of this study was to determine how adjusting marker placement at the sternum, cervical, and thorax locations for purposes of defining the trunk segment influences trunk kinematics. Sagittal plane trunk angles of 18 participants were computed for a comprehensive set of trunk model variations during standing, gait, and a drop vertical jump…

Correction of a postpneumonectomy syndrome with congenital pectus excavatum using Ravitch's procedure and silicone breast implants. Report of a case.

INTRODUCTION: Mediastinal repositioning and the use of allogenic implants to obliterate the postpneumonectomy space is the main principle of postpneumonectomy syndrome (PPS) correction. We present a rare case with a PPS in combination with a congenital pectus excavatum. As a pectus excavatus deformity reduces retrosternal space, simple repositioning of the heart is impossible unless combined with a sternum elevation. PRESENTATION OF CASE: Two years after left sided pneumonectomy a 30 year old female was admitted with progressive exertional dyspnea and stridor and not able to do her basic acti…

Parental infant jaundice colour card design successfully validated by comparing it with total serum bilirubin.

AIM: We designed a jaundice colour card that could be used by the parents of neonates and validated it by comparing it with total serum bili … rements were taken at 11-20 days (range 3-30). The measurements at the infant's forehead, cheek and sternum showed strong correlations with total serum bilirubin. The mean differences between the total serum bilirubin and the jaundice colour card measurements from the forehead, cheek and sternum were 1.9 mg/dL, 0.3 mg/dL and 1.5 mg/dL, respectively. When …

Four forms of Jeune syndrome have been … rom each lateral intercostal space at the midaxillary line and pulled out over the lower end of the sternum. The ends of inserted bars were fixed to soft tissue over the sternum with nonabsorbable sutures. Conjoined costal cartilage around the sternum restricts the number of bars that can be positioned. Preliminary study on the effects of bar placement on the thorax after the nuss procedure for pectus excavatum using bone scintigraphy.

AIM: Bone scintigraphy was performed to elucidate the effects of the Nuss procedure for pectus excavatum on the bony thorax. METHODS: Eight boys and 6 girls (5 - 24 years of age) underwent bone scintigraphy, using (99m)Tc-HMDP. Eleven patients were studied 5 to 21 days after the Nuss procedure; 6 were studied 20 to 24 months after the operation before bar removal. Three of 14 were studied twice after the Nuss procedure and before bar removal. RESULTS: In the early postoperative phase, RI accumulation was found at the sternum and ribs in only 1 of 6 patients under 9 years of age, whereas in a…

Pectus excavatum correction enhanced by pectoralis muscle transposition: A new approach.

BACKGROUND: Indications for the surgical correction of pectus excavatum include functional/physiological, cosmetic, and psychosocial reasons. The most popular open technique for pectus excavatum repair was proposed by Ravitch in 1949 as an open approach that requires partial resection of the costal cartilage, xiphoid excision, and osteotomy of the sternum. The goal is to remove abnormal rib cartilage while preserving the perichondrium, allowing regrowth of the rib cartilage to the sternum in a more anatomic manner. OPERATIVE TECHNIQUE: We present a case of bilateral pectoralis muscle flap tra…

Hybrid zone zero debranching thoracic endovascular aortic repair of ascending aortic injury after surgery and radiotherapy for breast cancer.

Thoracic endovascular aortic repair of the ascending aorta remains challenging. We have reported the case of an 81-year-old woman with ascending aortic injury who underwent a life-saving hybrid repair. The patient had previously undergone extended radical mastectomy and postoperative radiotherapy for breast cancer, which had resulted in a right thoracic wall defect and bone exposure and osteonecrosis of the sternum. Therefore, the ascending aorta was directly compressed by the sternum at the level of the brachiocephalic artery bifurcation, causing persistent bleeding from the thoracic wall. Hy…

A mathematical analysis of alternative sternal wound closures--an aid to reducing cardiac surgical mortality.

Sternal dehiscence is an important complication of cardiac surgery. A simple mathematical analysis is described for determining the relative strength (S) of different types of sternal closure. This is based upon resolving the tensions within the segments of a sternal wire to derive a dehiscence force. The strength of the closure (S) is the ratio of the volume of sternum which the dehiscence force acts upon to the dehiscence force itself. The derived S values varied from 4.3 for a closure using 6 transverse wires to 11.4 for a closure using the Modified Technique. The analysis suggested that cl…

An incomplete pentalogy of Cantrell.

Pentalogy of Cantrell is a rare entity of congenital defects involving the abdominal wall, sternum, diaphragm, pericardium and heart. The complete pentalogy and its variants have been described in the literature. We report a 47-day-old girl with an incomplete pentalogy of Cantrell presenting with congenital absence of the sternum, congenital heart defects, and an epigastric hernia. Two even rarer expressions of this disorder, tricuspid atresia type IIc and asymmetric kidneys, were also found. The patient was successfully managed with palliative pulmonary artery banding as preparation for a mod…

Sternal dehiscence after cardiac surgery and ACE inhibitors [correction of ACE type 1 inhibition].

We report two cases, which underwent surgery through Median sternotomy. They were on ACE inhibitors [corrected] pre-operatively. Both of these patients developed persistent dry cough post-operatively, which resulted in sternal wound dehiscence. They had no clinical or bacteriological evidence of sternal wound infection. Although one patient was overweight and had moderately impaired left ventricular function, there were no other associated risk factors. Both patients underwent rewiring of the sternum. Type II receptors inhibitor were introduced post-rewiring, which cured the persistent dry cou…

A cretaceous fly trap? remarkable abdominal modification in a fossil wasp.

BACKGROUND: Carnivorous insects have evolved a range of prey and host capture mechanisms. However, insect predation strategies in the fossil record remain poorly understood. RESULTS: Here, we describe †Sirenobethylus charybdis n. gen. & sp., based on sixteen adult female wasps in Kachin amber from the mid-Cretaceous, 99 Mya (million years ago), and place it in Chrysidoidea: †Sirenobethylidae n. fam. The fossils display unique morphological modifications on the tip of the abdomen consisting of three flaps from the modified abdominal sternum 6 and tergum and sternum 7; the lower flap formed fro…

Separation of the seventh costal-sternal junction-A new technique to improve outcomes for the Nuss procedure for pectus excavatum.

BACKGROUND: In the standard Nuss procedure for pectus excavatum, the costal arch is often elevated together with the sternum, resulting in unevenness of the lower part of the thorax. This complication is commonly called rib flaring. This paper presents a technique to avoid rib flaring and evaluates its effectiveness. MATERIALS AND METHODS: In our technique, a part of the seventh costal cartilage is removed, disconnecting the costal arch from the sternum. The effectiveness of this technique was evaluated in a retrospective clinical study of 63 pectus excavatum patients who were randomly collec…

Primary Sternal Cleft Repair in First Week of Life.

We present a full-term neonate with a postnatal diagnosis of a sternal cleft. Cardiac evaluation was otherwise remarkable for a patent ductus arteriosus and patent foramen ovale. Computed tomography scan confirmed the absence of a sternum except for a small inferior portion, including the xyphoid process. The patient underwent primary repair at five days of age through a midline incision where an absence of pericardium anteriorly was noted. She was extubated successfully the next day and discharged home on postoperative day 3. On follow-up examination, her sternum has been healing well without…

An alternative reinforced closure technique with external Kirschner wires in sternal dehiscence.

OBJECTIVE: We present an alternative closure technique that is effective in length stabilization of the dehisced sternum, technically easy, and less time consuming. METHODS: Between June 2000 and June 2003, 850 patients underwent open heart surgery in the participating clinics. In all of the patients, the sternotomies were primarily closed with No. 5 steel wires in a figure 8 manner or with single sutures. Eleven patients with sterile sternal dehiscence underwent operations with Kirschner wires. RESULTS: During the postoperative period, no complications related to the implants were observed …
