Paraesophageal hernia (type II hiatal hernia) accounts for less than 5-10% of all herniation through the esophageal hiatus. Anatomically, it is distinguished from the more common sliding variety (type I hiatal hernia) by the position of the gastroesophageal junction. The management of paraesophageal hiatal hernia is influenced by the tendency of the herniated stomach to develop volvulus, which potentiates life-threatening complications that include complete gastric obstruction, bleeding, infarction and perforation. In a cases with gastroesophageal reflux one of the antireflux procedures had to

Hepatodiaphragmatic interposition of the intestine is a rare anomaly (0.025 to 0.28 percent of the general population) described by Chilaiditi in 1911 and often believed to be of irrelevant clinical interest. To the contrary, recent studies stated that this syndrome is a potential source of abdominal problems requiring emergency or elective operation. From a retrospective analysis of records since 1976, four instances of Chilaiditi's syndrome have been found (three males and one female). Interposition of the proximal transverse colon was found in three patients and the small intestine in one p

We describe a case of a newborn being treated for encephalopathy and seizures, whose radiographs since the first day of life demonstrate a persistent ovoid lucency over the central lower chest. A CT performed confirmed a type IV hiatal hernia, which is defined as a paraesophageal type hernia containing a portion of the abdominal viscera. This infant's hernia included the distal stomach, pylorus, and proximal duodenum. There was no volvulus or ischemic change at surgery. The patient underwent successful reduction, fundoplication, and gastrostomy placement with hospital discharge after further s

OBJECTIVE: We report clinical, radiograpic and instrumental data about 18 cases of gastric chronic idiopathic volvulus observed since 1997 to june 2000.
METHOD: All cases diagnosed by barium meal, performed for vomiting or after a pHmetric esophageal monitoring, in cases of atipical symptoms of gastroesophageal reflux disease.
RESULTS: All patients underwent a conservative treatment (antireflux position and procinectics): 5 recovered after 1 year of treatment and 13 were operated on because they did not improve during medical therapy or relapsed after the end of conservative treatment.

Wandering spleen is a rare condition, characterized by a mobile spleen that is attached only by an elongated vascular pedicle, allowing it to migrate to any part of the abdomen or pelvis. Mesenteroaxial gastric volvulus usually occurs in children and may be associated with wandering spleen. Both entities result from abnormal laxity or absence of the peritoneal attachments due to abnormal fusion of the peritoneal mesenteries. Pancreatic volvulus is a very rare anomaly, with only a few isolated case reports described in association with wandering spleen. Anomalous right sided descending and sigm

Gastric ischemia is uncommon because the stomach has multiple collateral blood supplies. The etiology of gastric ischemia is vascular insufficiency caused by systemic hypotension, vasculitis, or disseminated thromboembolism. Mechanical causes include gastric volvulus and acute gastric distention. Uncommon as gastric ischemia is, we are the first to report a 65-year-old male who developed gastric ischemia leading to gastric pneumatosis 26 days after initial treatment for severe acute respiratory syndrome coronavirus-2 infection (SARS-CoV-2), via laparoscopic imaging. We conclude that physicians

INTRODUCTION: Paraesophageal hernia with intrathoracic mesentericoaxial type of gastric volvulus is a rare clinical entity. The rotation occurs because of the idiopathic relaxation of the gastric ligaments and ascent of the stomach adjacent to the oesophagus through the hiatus defect, while the gastroesophageal junction remains in the abdomen. The open approach remains the gold standard therapy for most patients. Here we report the case of a patient with such a condition who underwent a successful laparoscopic surgery. A literature search revealed that this is the first case report from Southe

Although volvulus of a gastric pull-up may acutely result from iatrogenic twist at the time of gastric transposition, we have recently encountered a patient in whom a gastric volvulus developed in the late postoperative period, resulting in gastric outlet obstruction. The perioperative evaluation and operative management are discussed.

Until 1980, 300 cases of gastric volvulus had been reported in the literature. Of these only 50 had presented in children. The disease is considered rare. We reviewed our experience at Sainte-Justine, in the last 30 years and found 10 cases, all of which diagnosed since 1980. Four patients were under one year of age. The other six were between 3 and 17 years old. In five the presentation was acute and three had had similar previous episodes. In 5 patients the presentation was chronic and in four of these the symptoms dated from birth. Seven associated anomalies were present in 6 cases. Diagnos

Gastric dilatation-volvulus is a dramatic and life-threatening disease of large-breed dogs. The cause is unknown, but ingestion of dry cereal-based food is frequently incriminated as a predisposing factor. The purpose of this study was to examine the effect of commercial diets on gastric motility and emptying in the dog. Four large-breed dogs were fed 3 different diets (diet A = canned meat-based; B = cereal-based with 77% added water; and C = dry cereal-based) in a randomized block design. Each experiment was done in triplicate. Motility was assessed, using 5 AgAgCl electrodes and 2 strain ga

Gastric ischemia is a relatively rare condition that can lead to severe or life-threatening outcomes. It can be caused by various etiological factors, including vascular occlusion, atherosclerosis, vasculitis, hypovolemic shock, cardiac failure, mesenteric ischemia, splanchnic vasoconstriction, and abdominal compartment syndrome. Furthermore, gastric dilation can be caused by volvulus and acute necrotizing gastritis. This condition may go unnoticed in the setting of intestinal obstruction. In this case report, we describe a 43-year-old female who presented with signs, symptoms, and radiologica

The case was a 66-year-old female with a diagnosis of bilateral ovarian carcinoma with peritoneal carcinomatosis and liver metastases. She had only undergone one cycle of chemotherapy and could not be given other cycles because of her poor general condition. She was admitted due to intermittent vomiting for a week and severe heartburn, with the last stool passed two days before, presenting abdominal distension and lower abdominal pain, without signs of peritonism and no peristaltic sounds. After performing several complementary tests a computed tomography scan showed a large amount of intra-ab

The sliding hiatal hernias are the most usual diaphragmatic hernias in adults. Their recognition and the survey of their complications are performed with esophagoscopy. Antisecretory drugs are efficient in case of esophagitis. Indications for surgery are discussed in young patients with a long-term medical treatment. The paraesophagal hernias even with a total intrathoracic volvulus of the stomach are operated through abdominal approach. The Morgagni hernias are uncommon and rarely symptomatic. The Bochdaleck hernias are exceptionally revealed at adult age. Traumatic hernias occur after penetr

OBJECTIVES: Congenital paraesophageal hernia is a rare condition in the pediatric age group. The symptomatology of these patients is usually non-specific in the form of repeated attacks of chest infection and/or recurrent attacks of vomiting but can be associated with serious complications such as intrathoracic gastric volvulus.
METHODS: Between 1989 and 1997, 6 children with paraesophageal hernia were treated at our hospital.
RESULTS: Six children (4 males and 2 females) were treated for congenital paraesophageal hernia. Their age at presentation ranged from 2 days to 21/2 years (mean 1.3 y

Hiatal hernias are a common condition, particularly in the elderly. While often asymptomatic, they can lead to life-threatening complications such as incarceration, strangulation, and gastric volvulus. Obstructive shock due to cardiac compression is a rare but critical presentation. We present the case of a 77-year-old Japanese man with a history of renal transplantation who was admitted for conservative management of an incarcerated hiatal hernia. On the second day of admission, he developed profound shock, requiring ICU admission. A malpositioned nasogastric tube had failed to decompress the

AIM: The aim of this study was to report on laparoscopy-assisted gastropexy in a child with situs inversus, asplenia, and major cardiac anomaly.
CASE: A 15-month-old boy presented with a sudden onset of epigastralgia, nonbilious vomiting, and severe abdominal distention. After a nasogastric tube decompression of the stomach, symptoms resolved and an upper gastrointestinal contrast study confirmed situs inversus and asplenia. Computed tomography showed hepatic symmetry. Major cardiac anomalies (e.g., single atrium, single ventricle, common atrioventricular valve, and pulmonary atresia) were al

HISTORY AND ADMISSION FINDINGS: A 67-year-old man with schizophrenia, well controlled by drugs, had undergone a left hemicolectomy with a terminal transverse colostomy and rectal stump closure (Hartmann's operation) for perforation of the sigmoid colon with diffuse peritonitis. 3 months later a large invagination of the transverse colon necessitated relaparotomy with further extensive resection of the colon and a new colostomy. Subsequent mild mucosal erosions were treated conservatively. He was referred to our hospital after another irreducible colon invagination through the colostomy. On adm

OBJECTIVE--To investigate any potential structural differences in hepatogastric ligaments between clinically normal dogs and dogs with gastric dilatation-volvulus (GDV). DESIGN--Case-control study. ANIMALS--Hepatogastric ligaments were examined in 13 large-breed control dogs and in 13 large-breed dogs referred for surgical treatment of GDV. PROCEDURE--Measurements and biopsies of hepatogastric ligaments were performed at the time of surgery, circumcostal gastropexy, for correction of GDV. Serial sections from each ligament were stained with H&E, Masson's trichrome, and elastin stains to assess
