Small-angle strabismus detection by telemedicine in an experimental model.

PURPOSE: To evaluate the ability of observers to accurately detect strabismus using an alternate cover testing approach via telemedicine and to assess the effect of various factors related to video conditions on the accuracy rate. METHODS: Videos were made by the authors in which different angles of strabismus, up to 12Δ, were induced by prism and recorded using alternate cover testing. The videos were made under a variety of conditions that incorporated various head postures, lighting, fixation target position and motion, and viewing angle. The videos were shown to observers of varying level…

The blinq™ Vision Screener in Detection of Amblyopia and Strabismus.

PURPOSE: The blinq (Rebion Inc) is a new screening device designed to directly detect amblyopia and strabismus rather than amblyopia risk factors. We performed an independent assessment of the effectiveness of the blinq in detecting amblyopia and strabismus. DESIGN: Prospective clinical validity analysis of a screening device based on sensitivity and specificity. METHODS: Children presenting for examination in the pediatric ophthalmology clinic underwent screening with the blinq before examination by a pediatric ophthalmologist blinded to the screening results. Results of the blinq and exami…

Golden Indications and an Overview on the Use of Botulinum Toxin in Strabismus.

Botulinum toxin A (BTXA) is considered a pharmacological agent that may provide an alternative treatment to surgery in strabismus. It can be used as both a diagnostic and therapeutic agent in various concomitant, paralytic, and restrictive disorders. The major advantage of BTXA treatment is that it is non-invasive and does not impact the patient's chance for future surgery in case of an unfavorable response. In some selected disorders, BTXA has become the primary choice of treatment, whereas surgery is found to be more effective in others. Accumulated knowledge and experience have demonstrated…

The Swan incision: an underutilized conjunctival approach to strabismus surgery.

BACKGROUND: The Swan incision is an easy-to-learn yet underutilized approach to strabismus surgery. We compare the Swan approach to the limbal and the fornix approaches and report the results of a survey of surgeons who had previously learned the technique. METHODS: We distributed a survey to former fellows of the senior author (NBM) to determine which approaches to strabismus surgery they have continued to use. For comparison, we also distributed our survey to other strabismus surgeons practicing in the greater New York area. RESULTS: Surgeons in both groups reported using all three approac…

Applications of bupivacaine in the non-surgical treatments of strabismus: a review.

Bupivacaine (BUP) is an anesthetic from the family of aminoacyl anesthetics and has the highest myotoxicity among other groups of anesthetic … There are numerous studies according to BUP injection for the non-surgical management of horizontal strabismus. To intensify the effects of the injection, botulinum toxin injection can also be used simultaneously in the antagonist muscles. In general, although the rate of improvement in strabismus varied among different studies, BUP injection alone corrects …

Scleral Perforation as a Complication of Strabismus Surgery: A Literature Review.

The aim of this review was to report the existing literature on the incidence, risk factors, treatment, and outcomes of scleral perforation as a vision-threatening complication of strabismus surgery. The current literature reported an incidence range of scleral perforation/penetration between 3 in 1,000 and 7.8%. Rectus muscle recession was the most commonly mentioned risk factor among the studies. Other risk factors were myopia, previous extraocular muscle surgery, surgeon experience, S-24 needle use, muscle reattachment posterior to the original insertion site, operating on a horizontal rect…

Causes of concomitant strabismus surgery delay in teenagers and adults.

To reveal the reasons for strabismus surgery delay and motivations for seeking surgical treatment in adulthood. Prospective survey among 91 adult patients, suffering from concomitant strabismus acquired in the childhood that underwent delayed strabismus surgery. The mean age of participants was 24 years (from 16 to 58 years); 48 females (52.7%), and 43 males (47.3%). Strabismus surgery has been delayed for about 20 years in adult patients who could potentially benefit of it in childhood. The most common reasons for strabismus surgery delay, reported by patients, included the following: lack of…

Epileptic ictal strabismus: a case report and review of the literature.

Ictal strabismus, sometimes associated with epileptic nystagmus, is an extremely rare epileptic phenomenon, suggestive of cortical involvement in monocular eye movement control. We describe a patient with ictal disconjugate contraversive horizontal eye deviation of cortical origin as the main clinical feature of a focal seizure. A 17-year-old, previously healthy woman had a seizure characterized by initial rightward conjugate eye deviation, followed by convergent strabismus due to adduction of the right eye towards the nose without conjugate left eye abduction (esotropia), forced leftward head…

Neural mechanisms of oculomotor abnormalities in the infantile strabismus syndrome.

Infantile strabismus is characterized by numerous visual and oculomotor abnormalities. Recently nonhuman primate models of infantile strabismus have been established, with characteristics that closely match those observed in human patients. This has made it possible to study the neural basis for visual and oculomotor symptoms in infantile strabismus. In this review, we consider the available evidence for neural abnormalities in structures related to oculomotor pathways ranging from visual cortex to oculomotor nuclei. These studies provide compelling evidence that a disturbance of binocular vis…

The psychological impact of strabismus: does the angle really matter?

INTRODUCTION: The psychological impact of strabismus is well recognized. Patients with strabismus have lower levels of psychological well being compared to normal controls. Few studies have evaluated the relationship between levels of psychological distress and the angle of deviation, age, sex, presence of diplopia, visual acuity and direction of deviation. METHODS: 50 patients with strabismus who attended the adult strabismus clinic were included in this questionnaire based prospective study. The Psychological Impact questionnaire was used to assess the psychological impact of noticeable str…

Postural control in children with strabismus: effect of eye surgery.

The purpose of this study was to examine the postural control in children with strabismus before and after eye surgery. Control of posture is a complex multi-sensorial process relying on visual, vestibular and proprioceptive systems. Reduced influence of one of such systems leads to postural adaptation due to a compensation of one of the other systems [3]. Nine children with strabismus (4-8 years old) participated in the study. Ophthalmologic, orthoptic, vestibular and postural tests were done before and twice (2 and 8 weeks) after eye surgery. Postural stability was measured by a platform (Te…

Utility of adjustable sutures in primary strabismus surgery and reoperations.

PURPOSE: To evaluate the success of adjustable suture (AS) and nonadjustable suture (NAS) strabismus surgery in primary procedures and reoperations. DESIGN: Retrospective case series. PARTICIPANTS: Four hundred four patients older than 12 years who underwent strabismus surgery over a 13-year period. METHODS: All eyes underwent the same hang-back suturing technique by a single surgeon (S.P.K.) in both the AS and NAS groups. Success was defined as alignment within 10 prism diopters (PD) for horizontal recti and within 5 PD of orthophoria for vertical recti without diplopia or further surgery.…

Prevalence of amblyopia and strabismus in young singaporean chinese children.

PURPOSE. To determine the prevalence of amblyopia and strabismus in young Singaporean Chinese children. METHODS. Enrolled in the study were 3009 Singaporean children, aged 6 to 72 months. All underwent complete eye examinations and cycloplegic refraction. Visual acuity (VA) was measured with a logMAR chart when possible and the Sheridan-Gardner test when not. Strabismus was defined as any manifest tropia. Unilateral amblyopia was defined as a 2-line difference between eyes with VA < 20/30 in the worse eye and with coexisting anisometropia (> or =1.00 D for hyperopia, > or =3.00 D for myopia, a…
