Stepwise reduction of bone mineral density increases the risk of cage subsidence in oblique lumbar interbody fusion patients biomechanically: an in-silico study.

BACKGROUND: Cage subsidence causes poor prognoses in patients treated by oblique lumbar interbody fusion (OLIF). Deterioration of the biomechanical environment initially triggers cage subsidence, and patients with low bone mineral density (BMD) suffer a higher risk of cage subsidence. However, whether low BMD increases the risk of cage subsidence by deteriorating the local biomechanical environment has not been clearly identified. METHODS: OLIF without additional fixation (stand-alone, S-A) and with different additional fixation devices (AFDs), including anterolateral single rod screws (ALSRs

Trend and Economic Implications of Implant Selection in the Treatment of Intertrochanteric Hip Fractures: A Review of the American Board of Orthopaedic Surgery Database From 2007 to 2017.

INTRODUCTION: Intertrochanteric (IT) fractures are estimated to burden the healthcare system six billion dollars annually. Previous studies have documented a trend of early-career orthopaedic surgeons favoring intramedullary nails (IMNs) for these fractures, despite multiple randomized controlled trials demonstrating no evidence for improved patient outcomes when compared with sliding hip screws (SHSs). The purpose of this study was to report the trend of implant utilization for IT fracture fixation from 2007 to 2017. METHODS: The American Board of Orthopaedic Surgery (ABOS) Part II (oral) da

Examining the Relationship Between Wound Complications and the Use of Resorbable Plates in Cranial Vault Reconstruction.

BACKGROUND: Resorbable plates are commonly used in cranial vault reconstruction surgery. There are few published papers examining their safety profile. The authors examined the prevalence of wound complications associated with the use of resorbable plates (Inion CPS Fixation System) in pediatric patients undergoing cranial vault reconstruction. METHODS: A retrospective review of patients (n = 182) who underwent cranial vault reconstruction using resorbable plate fixation was undertaken. All procedures were performed by a single Craniofacial Surgeon at the National Pediatric Craniofacial Cente

Rod Migration to the Thoracic Subarachnoid Space after C1-2 Instrumentation: A Case Report and Literature Review.

Posterior instrumented fusion of the cervical spine is a common surgical procedure in the treatment of cervical subluxation, fractures, and stenosis. Although malpositions are commonly seen, it is rare to observe the malposition of the rod or interconnection because of hardware failure. A 62-year-old woman with spastic tetraparesis as a sequel to pediatric meningitis with C1 -C2 cervical subluxation and myelomalacia had undergone laminectomy of C1 and C1 lateral mass and C2 bilateral pedicular screw fixation. Three years after the stabilization, she presented with complaints of headache, neck

Functional and Radiological Outcomes Following Volar Locking Plate Fixation for Distal Radius Fractures: A Prospective Analytical Study.

INTRODUCTION: Most frequent injuries of the Upper limb are a distal radius fracture (DRF). If they aren't handled well, they might cause problems for a long time. There are several techniques to treat this issue, but volar locking plate fixation has become increasingly prevalent since it may restore anatomical alignment, provide stable fixation, and allow for early movement. This study aimed to evaluate the functional and radiological results of distal end radius fractures treated with volar locking plate fixation. MATERIALS AND METHODS: In this prospective analytical investigation, 60 patien

Suture-Button Device Stabilization Following Ring Finger Ray Amputation: A Comparative Cadaver Study.

BACKGROUND: The purpose of this study was to determine whether placing the suture-button device between the long and small finger metacarpals following ring finger ray amputation may better close the intermetacarpal gap and allow early range of motion without increasing the risk of malrotation than soft tissue repair alone. METHODS: We performed ray amputation of the ring finger of 14 cadaver specimens by performing an osteotomy of the base of the ring finger metacarpal and then excising the remainder of the digit. We first performed a soft tissue repair of the transverse metacarpal ligaments

Development and calibration of a load sensing cervical distractor capable of withstanding autoclave sterilization.

In surgery of the cervical spine, a Caspar pin distractor is often used to apply a tensile load to the spine in order to open up the disc space. This is often done in order to place a graft or other interbody fusion device in the spine. Ideally a tight interference fit is achieved. If the spine is over distracted, allowing for a large graft, there is an increased risk of subsidence into the endplate. If there is too little distraction, there is an increased risk of graft dislodgement or pseudoarthrosis. Generally, graft height is selected from preoperative measurements and observed distraction

[Treatment of pseudoarthrosis of long bones by surgical stabilization and a transfer of in-vitro cultivated bone marrow interstitial cells].

The paper presents the results of surgical stabilization of pseudoarthrosis of long bone augmented by a transfer of in vitro cultivated bone marrow interstitium cells in 4 patients. In three cases a Zespol external fixation device was applied without opening the pseudoarthrosis. One patient was treated by open reduction of the bone fragments with the use of frozen spongiose bone grafts. The mean age of the patients was 30.2 years. In two cases the pseudoarthrosis had developed in the tibia, in the remaining two cases it developed in the ulna. The pseudoarthrosis was present for a mean time of

Short-term outcomes of modified Lapidus procedure using the InCore® Lapidus System for hallux valgus: Case series of four cases.

INTRODUCTION: Hallux valgus is a common foot deformity often associated with first tarsometatarsal (TMT) joint instability. The Lapidus procedure, involving first TMT joint arthrodesis, is a well-established surgical procedure for severe cases, but the issue lies in the method of fixation. The InCore® Lapidus System is a novel intramedullary fixation system for modified Lapidus procedure. We here report the clinical experience of this system. CASE PRESENTATION: We retrospectively reviewed four cases who underwent a modified Lapidus procedure using the InCore® Lapidus System and followed up at

Open Repair of Posterior Cruciate Ligament Tibial Bony Avulsion With Metal Anchor: A Case Report.

Introduction: The posterior cruciate ligament (PCL) is the largest and strongest intra-articular ligament of the knee joint and the primary posterior stabilizer. PCL injuries are less frequent than other knee ligament injuries and are typically combined with meniscal and chondral injuries or in the context of multiligamentous injuries. It is critical to properly diagnose and treat these lesions in order to avoid the risk of PCL insufficiency, subsequent knee instability, and early osteoarthritis. Surgical management can vary, and the ideal fixation device is still debated. Suture anchors are a

Entrapment of the Median Nerve after Elastic Intramedullary Nailing of a Pediatric Forearm Fracture.

Closed reduction and fixation with elastic intramedullary nails is a popular technique to treat displaced pediatric forearm fractures and has yielded good functional results. Postoperative neurological symptoms can be due to neuropraxia and expectative treatment has been advised for 3 months until further investigation is necessary. We present a case of an 11-year-old boy that presented with median nerve palsy 2 months after a displaced forearm fracture treated with elastic intramedullary nails. Ultrasound and nerve conduction study confirmed the presence of a median nerve entrapped in the cal

Distraction osteogenesis of a unilateral hypoplastic mandible.

Distraction osteogenesis (DO) is a surgical process in which two bony segments are gradually separated so new soft tissue and bone will form between them by applying tension through a fixation device. There are three phases to this process: latency phase, distraction phase, and consolidation phase. The technique was originally applied to long bones but in recent years the method has been adapted for use in maxillofacial surgery. Distraction osteogenesis is a new variation of more traditional; orthognathic surgical procedures which can be applied for the correction of dento-facial deformities a

The Roger Anderson device in the treatment of fractures of the distal end of the radius.

One of the more common as well as difficult fractures to manage is the extension compression fracture of the distal end of the radius. In the physiologically younger patient we believe that every effort should be made to preserve the integrity of the radiocarpal joint and maintain motion of the hand. We have used the technique described by Anderson and O'Neil to treat comminuted, displaced fractures of the distal end of the radius in twenty-five patients from April 1972 through April 1978. Twenty-one of these patients were available for follow-up examination at an average of nineteen months (r

Mass casualty response of a modern deployed head and neck surgical team.

BACKGROUND: The battlefields of the Global War on Terror have created unique demands on deployed surgical teams. Modern high-energy fragmentation devices often inflict complex head and neck injuries. This series analyzes the role of the head and neck surgical team during 3 separate single explosive events that led to civilian multiple casualty incidents (MCIs) treated at a military theater hospital in Iraq from February to April 2007. METHODS: All MCIs occurring between February and April 2008 with triage and treatment at the 332nd Air Force Theater Hospital in Balad, Iraq, were identified an

Primary Fixation of Single Stitch All-Inside and Inside-Out Meniscal Devices for Repairing Vertical Longitudinal Meniscal Tears: A Human Microscopic Imaging and Biomechanical Study.

BACKGROUND: Optimized surgical fixation and meniscal stabilization during rehabilitation increase healing success. However, the latest generation of all-inside devices has not yet been biomechanically compared with inside-out suture tape (IO-ST) repair. HYPOTHESIS: (1) The contact area of a suture anchor (SA) would compensate for a meniscal defect better than polyether ether ketone anchors (PA); (2) adjustable tensioning for all-inside meniscal repair fixation would result in higher initial load than IO-ST repair; and (3) stiffer constructs would decrease secondary displacement. Percutaneous pins versus volar plates for unstable distal radius fractures: a biomechanic study using a cadaver model.

PURPOSE: A biomechanic study using a cadaver model of a dorsally unstable distal radius fracture was used to compare the stability of percutaneous pinning and volar fixed-angle plating. Among the many surgical options for treating distal radius fractures are percutaneous pinning and internal plate fixation. Although percutaneous pin fixation requires less soft-tissue trauma and has low complication rates, plate fixation allows for early active movement with good clinical results. The biomechanic stability of these 2 methods was studied by using a cadaver model of a dorsally unstable intra-arti

Surgical management and imaging evaluation of post-traumatic periscaphoid osteoarthritis (SLAC and SNAC wrists).

An untreated scapholunate interosseous ligament injury or scaphoid nonunion leads to progressive post-traumatic periscaphoid osteoarthritis and carpal collapse. Symptomatic forms, known as scapholunate advanced collapse (SLAC) or scaphoid nonunion advanced collapse (SNAC), are common and disabling, requiring surgical management. Numerous operative options exist depending on the stage of collapse and the extent of joint involvement. We apply a modified Watson radiological classification consisting of four stages of pancarpal collapse, integrating the etiological origin (SLAC or SNAC) and the ar
