[Perioperative management of vagotomy for treatment of frequent syncope].

A 66-year-old man was admitted to our hospital because of frequent chest pain and loss of consciousness. He had a 6-year history of angina a … ay without atropine or any other treatment. His frequent attack of bradycardia and hypotension with syncope was due to vagal reflex by the recurrent tumor.

Sudden Wenckebach periods and their relationship to neurocardiogenic syncope.

Throughout a 9-month period during which 1,125 Holter tapes were reviewed prospectively we identified 13 nonmedicated patients with an arrhy … The episodes were separated into two groups. Group I included 11 patients with symptoms other than syncope and Group II included 2 patients with syncope. There were 26 episodes of sudden WP in Group I. Twenty-five terminated in a single (and one in double) blocked P waves. Most episodes occurred between 10 PM and 7 AM. Symptoms did not correlate with the …

Urodynamic and cardiovascular measurements in patients with micturition syncope.

We describe the findings of urodynamic studies, together with blood pressure and heart rate monitoring, in five patients with micturition syncope. All patients had almost normal storage and evacuation function and no evidence of prostate hypertrophy. Conventional head-up tilt testing with an empty urinary bladder caused no change in arterial blood pressure, but a moderate increase in heart rate. Urinary bladder filling caused minimal increases of the arterial pressure and heart rate. The sitting posture with a distended bladder caused mild orthostatic hypotension. Urinary bladder evacuation ca…

[Syncopes associated with mitral valve prolapse. Mechanisms].

Prolapsed mitral valve prolapse (PMV) is classically associated with disorders of ventricular excitability whose significance is unclear. However, syncope can suggest the possibility of a serious ventricular arrhythmia. The objective of this study was to try to identify the mechanisms of dizziness and syncope associated with PMV. We report the results of programmed atrial and ventricular stimulation performed under baseline conditions and after administration of Isuprel in 56 patients with PMV: 27 patients had a history of presyncope or syncope (group I), 14 had spontaneous atrial or supravent…

Advances in the diagnosis and therapy of syncope and palpitations in children.

Although advances in diagnosis and therapy of congenital heart disease have received most of the attention, in the past few years advances in the more common troublesome problems of adolescents with syncope and palpitations have been as dramatic. Syncope may be due to a cardiac arrhythmia, but is more commonly due to an autonomic dysfunction of blood pressure control. In cases where the diagnosis is equivocal by history alone, autonomic testing, including head-up tilt table testing, may be useful. Treatment with a salt-enriched diet and a mineralocorticoid is usually effective in reducing or e…

Syncope in elderly patients. Why their risk is higher.

The elderly are at increased risk for syncope not only because of physiologic changes due to aging but also because they have a higher incidence of chronic illness and use a greater number of medications that may cause orthostatic hypotension. Although evaluation should emphasize cardiovascular and neurologic components, the cause may remain elusive in as many as 50% of patients.

[Electrophysiologic study in patients with syncope of unknown cause].

Electrophysiologic studies were performed in 51 patients with syncopes of unexplained origin. 25 patients (49%) had organic heart disease. Electrophysiologic testing included determination of corrected sinus node recovery time, AV-nodal effective refractory period, AH- and HV-intervals, and AV-nodal Wenckebach rate. During programmed right ventricular stimulation, 1-3 premature stimuli were used. 26 patients (53%) had an abnormal outcome that strongly suggested an arrhythmogenic cause of the reported syncopes. In ten patients (20%), corrected sinus node recovery time was prolonged; AV-nodal co…

Epidemic faintness and syncope in a school marching band.

On Sept 21, 1973, during and following a football game at which they had participated, 57 members of an Alabama high school marching band (and one accompanying adult) experienced an illness characterized by headache, nausea, weakness, or dizziness. Six girls fainted. Thirty-six students were treated at a hospital emergency room. Those who had played wind instruments and had worn heavier uniforms including an impermeable plastic jacket overlay were affected earlier and more frequently than the others. Several organic causes were examined in an epidemiologic investigation and considered unlikely

Comparison of 30-Day Serious Adverse Clinical Events for Elderly Patients Presenting to the Emergency Department With Near-Syncope Versus Syncope.

STUDY OBJECTIVE: Controversy remains in regard to the risk of adverse events for patients presenting with syncope compared with near-syncope. The purpose of our study is to describe the difference in outcomes between these groups in a large multicenter cohort of older emergency department (ED) patients. METHODS: From April 28, 2013, to September 21, 2016, we conducted a prospective, observational study across 11 EDs in adults (≥60 years) with syncope or near-syncope. A standardized data extraction tool was used to collect information during their index visit and at 30-day follow-up. Our prima…

Deceleration capacity as a predictor of vasovagal syncope subtypes.

BACKGROUND: Autonomic function may play a crucial role in the pathogenesis of vasovagal syncope (VVS). However, the characteristics of autonomic function across different VVS subtypes remain unclear. Deceleration capacity (DC), a novel vagal indicator, may offer improved discriminatory value. OBJECTIVE: To evaluate the ability of DC to distinguish between different subtypes of VVS. METHODS: Patients with VVS confirmed by a positive head-up tilt test were included. All heart rate variability (HRV) and DC data were obtained from 24-h Holter monitoring and analyzed among different subtypes usin…

Unveiling the Reason for Syncope Associated With Cardiac Nodules.

Behcet's disease is a systemic immune-mediated vasculitis predominantly affecting males. Cardiac involvement, termed cardiac Behcet's disease, is rare but potentially fatal, often mimicking infective endocarditis. This case presented with syncope as the initial symptom, leading to the identification of a non-coronary aortic sinus nodule, suspected as the cause of intermittent complete atrioventricular block. After pacemaker implantation, no recurrent syncope occurred, and immunosuppressive therapy reduced nodule size. This case underscores the importance of maintaining a high suspicion for car…

Swallow Syncope as a Late Complication of Sleeve Gastrectomy.

Swallow syncope, also known as swallow-induced syncope, or deglutition syncope, is a type of situational reflex syncope associated with swallowing. It is believed to be due to exaggerated vagal parasympathetic stimulation leading to inhibition of heart rate during swallowing. Swallow syncope has been documented in cases of gastroesophageal structural pathologies, such as achalasia and esophageal stricture, and has even been shown to resolve following surgical correction. To date, there are 118 reported cases of swallow syncope published in medical literature, which includes 14 reports of swall…

Syncope and "Normal" ECG: Is Brugada Syndrome the Culprit?

Brugada syndrome (BrS) is an inherited disorder associated with a risk of ventricular fibrillation (VF) and sudden cardiac death (SCD) in a … ocopies, and it is imperative to rule out these conditions. We present the case of a young man with syncope with an initial suspected "normal" electrocardiogram with posterior development of type 1 Brugada pattern and an episode of non-sustained ventricular tachycardia.

Targets for deprescribing in patients with hypertension and reflex syncope.

BACKGROUND: We aimed to identify the target of deprescribing, i.e. the 24-hour SBP increase needed to achieve the greatest reduction of SBP drops. METHOD: Forty hypertensive patients (mean age 73.6 ± 9.3 years, 26 females) with reflex syncope and SBP drops on a screening ABPM were advised to withdraw or to reduce their therapy. The study objective was the reduction of SBP drops <90 mmHg and <100 mmHg on a second ABPM performed within 3 months. RESULTS: Out of a total of 98 drugs taken during ABPM 1, 44 were withdrawn, 16 had a dose reduction and 38 remained unchanged at the time of ABPM 2. 2…

The Backup That Led to a Blackout: Syncope and Severe Constipation.

Syncope is a common presentation in the emergency department (ED); however, it is rarely due to stercoral proctocolitis causing inferior vena cava (IVC) compression. We describe the case of a young male patient who presented to the ED after having a syncopal episode. Through obtaining a detailed history and physical exam, he was found to have abdominal distention and endorsed constipation. He was diagnosed with severe fecal impaction causing stercoral proctocolitis and IVC compression causing intermittent syncope. He underwent an emergent exam under anesthesia and fecal disimpaction in the ope…

The Rate of Asystolic Reflex Syncope Is Not Influenced by Age.

BACKGROUND: The head-up tilt test (HUT) and other evidence suggest that the vagal effect on the heart decreases with age. OBJECTIVES: The main aim of the study was to assess whether this age effect also affects the rate of asystole in spontaneous reflex syncope (RS). METHOD: We performed an analysis of pooled individual data from 4 studies that recruited patients ≥40 years of age affected by certain or suspected RS who received an implantable loop recorder (ILR) and reported follow-up data on syncope recurrence. We assessed the presence of asystolic syncope of >3 seconds or nonsyncopal asyst…
