The co effect of prophylaxis and radiosynovectomy on bleeding episodes in haemophilic synovitis.

Prophylactic substitution treatment and radiosynoviorthosis have a leading role in preventing irreversible haemophilic arthropathy. The aim of the study was to evaluate the effects of prophylaxis treatment and radiosynovectomy on the length of intervals between subsequent haemorrhages in haemophilic patients. Thirty-three joints were treated with radiosynovectomy in 28 patients with bleeding disorders. (90)Y colloid was used in knees and (186)Re colloid for elbows, shoulders and ankles. Twenty patients were on prophylaxis. Joint X-rays were evaluated on the Pettersson scale between 0 (normal)

Power Doppler sonography in the diagnosis of hemophilic synovitis--a promising tool.

BACKGROUND: Recurrent hemarthroses in hemophilia results in synovitis and joint arthropathy. Primary prophylaxis when universally instituted at current doses can prevent joint deterioration but is expensive. Alternatively, the selective implementation of prophylaxis would require a more sensitive tool for detecting synovitis than possible with clinical surveillance or plain radiographs. Magnetic resonance imaging (MRI) is such a tool and is utilized for the evaluation of hemophilic joint disease (HJD). However, it is expensive, and requires sedation in younger children precluding its utility f…

Localized pigmented villonodular synovitis in the knee associated with locking symptoms.

Pigmented villonodular synovitis (PVNS) occurs in the knee more frequently than other joints. Most cases involve diffuse PVNS while the localized type is relatively rare. This report describes a patient who had PVNS and a lateral meniscus injury which induced locking symptoms in the knee. A closed reduction of the meniscus tear was performed under local anesthesia. However, complete extension of the knee was not achieved and the locking symptoms persisted. MRI examination showed a neoplastic lesion measuring approximately 2 x 2 cm in the intercondylar space together with a lateral meniscus tea…

Posterior interosseous nerve palsy related to rheumatoid synovitis of the elbow.

Posterior interosseous nerve palsy (PINP) is a rare complication of rheumatoid arthritis of the elbow. A 58-year-old woman with rheumatoid arthritis, who complained of an inability to extend her left fingers, was referred to our hospital. After a series of studies, extensor tendon ruptures were excluded, and PINP was diagnosed. By means of the Henry anterolateral approach, the radial nerve was exposed, which was compressed by swollen synovial membrane at the Frohse arcade. Elbow synovectomy was performed, and the arcade was opened to release the nerve. The PINP has been recovered completely wi

[A case of pigmented villonodular synovitis of the elbow joint with bone invasion].

Pigmented villonodular synovitis is a benign proliferative disorder of the joint and of the tendon sheath synovium. It has a predilection for the lower extremities, particularly the knee and the hip. The elbow joint is rarely affected. A 56-year-old woman had complaints of pain and swelling in the left elbow for three years. She had no history of trauma. On physical examination, she had swelling of the left elbow, varus deformity, and flexion contracture of 20 degrees. Active and passive joint movements were painful. Magnetic resonance imaging showed synovial thickening and signal changes. An …

Urine glycosaminoglycans in children with transient hip synovitis and Perthes disease.

Urine glycosaminoglycan concentrations were measured in 24-h urine samples collected from ninety children (30 children with Perthes disease, 30 with transient synovitis and 30 control participants). Significantly decreased levels of urine glycosaminoglycans were found in Perthes disease patients compared with those with transient synovitis and the control group.

Bone age delay in Perthes disease and transient synovitis of the hip.

Patients with transient synovitis of the hip were found to have a bone age delay similar to the one found in patients in the active stages of Perthes disease. This finding led to a study on the relationship between the bone and chronologic ages in patients affected by either disease, in activity, and in the residual stage through to the end of growth. Bone age of all patients was established by means of a radiograph of both wrists and hands and the quantified data were compared with the Greulich and Pyle Atlas. All the patients in the active stage of both diseases revealed a bone age delay wit…

The outcome of knee synovitis in early arthritis provides guidelines for management.

The aim of the study was to examine the clinical outcome of patients presenting to an early arthritis clinic with synovitis of the knee. The patients were assessed at presentation for evidence and pattern of joint inflammation. These patients were then reassessed at 3, 6 and 12 months and thereafter annually to determine clinical outcome. One thousand six hundred and thirty-three consecutive referrals were examined, 903 of whom had early synovitis. One hundred and thirty had knee synovitis at presentation, of whom 73 fulfilled ACR criteria for rheumatoid arthritis (RA) during the study. All 73…

Pigmented villonodular synovitis of the foot and ankle: a review of six cases.

Pigmented villonodular synovitis is a rare but well recognized proliferative lesion of synovial tissue. It has been most commonly described in the knee and hip, with most series reporting <5% occurrence in the foot and ankle. Six patients with pigmented villonodular synovitis of the foot and ankle that was treated between 1978 and 1997 were reviewed. Four of the patients had not been previously diagnosed, and two patients presented with recurrent disease. All six were women. Two patients' disease presented isolated to the ankle joint. The other four involved more than one joint: subtalar joint…

[Villonodular synovitis--a rare condition causing intermittent locking of the knee].

Two patients with intermittent locking, mimicking a meniscal lesion or a loose body and with a palpable tumour in the knee joint are described. X-ray examinations were normal. Athroscopy revealed pedunculated tumours in both knees. The tumours were removed arthroscopically in one patient and by an arthrotomy in the other patient. The histological diagnosis was localized villonodular synovitis (giant-cell tumour) in both patients. This is a benign but very rare condition.

Comparable histological appearance of synovitis in seropositive and seronegative rheumatoid arthritis.

OBJECTIVE AND METHODS: To compare the histological characteristics of the synovium between seropositive and seronegative rheumatoid arthritis (RA), synovial tissue was obtained from 19 patients with rheumatoid factor-positive (RF+) RA, 11 with rheumatoid factor-negative (RF-)RA and 11 non-RA controls. RESULTS: There were no differences in the frequency of each histological feature or histological scores between RF+ and RF-RA. However, significant differences in the frequency of histological findings such as lining cell proliferation and inflammatory cell infiltration were found both between R

Pigmented villonodular synovitis. Monoclonality and metastasis--a case for neoplastic origin?

We report a 48-year follow-up of a case of recurrent pigmented villonodular synovitis of the knee. Subcutaneous metastasis to the contralateral thigh was an unusual finding. Histology demonstrated fibroblastic and histiocytic proliferation, as well as increased mitotic activity in recurrent lesions. Cytogenetic analyses demonstrated monoclonality and chromosomal abnormalities. Our findings support a neoplastic origin of this lesion.

Destructive synovitis in contralateral Perthes' disease. A report of 2 cases.

2 boys had unilateral Perthes' disease at the age of 5 years. After 6 and 7 years, respectively, both patients developed contralateral femoral head necrosis with rapid destruction leading to ankylosis of the hip. Histology of the synovium showed nonspecific synovitis. Both patients fulfilled criteria for oligo-articular juvenile chronic arthritis (JCA). The association with Perthes' disease suggests a common etiology.

Ultrasonographic evidence of synovitis in axial joints in patients with polymyalgia rheumatica.

The central joints of 19 patients identified as suffering from polymyalgia rheumatica were examined using ultrasonography (US). At US 13 patients (68%) were found to have an effusion in the hip and/or glenohumeral joints. Synovial fluid analyses were conducted on the central joints of four patients. These findings suggest that at least one patient group with the syndrome of PMR could better be designated as having axial arthritis and this synovitis might play an important role in the understanding of their symptoms.

A case of adult T cell leukemia complicated by proliferative synovitis.

A 60-year-old woman was admitted to our hospital with symmetrical arthritis of the knees. During the 2 years preceding admission, she had experienced recurrent arthritis. A histological examination of her synovial tissue showed prominent villous proliferation of the synovial cells, prominent vascularity throughout and an inflammatory infiltrate composed of abnormal mononuclear cells. Three months later, she developed fever, skin eruptions, lymphadenopathy and hepatosplenomegaly. She also had hypercalcemia and there was abnormal lymphocytosis in her blood smears. She was diagnosed as having adu
