Advances in first-line treatment for patients with HER-2+ metastatic breast cancer.

BACKGROUND: The prognosis for breast cancer patients overexpressing human epidermal growth factor receptor (HER)-2 has changed with anti-HER-2-targeted therapy. Although anti-HER-2 therapy with trastuzumab and chemotherapy is the standard first-line treatment, the best therapeutic regimen has yet to be defined, and new strategies are evolving. METHODS: A literature review of well-established and recently published trials, reviews, and ongoing clinical trials addressing first-line treatment for HER-2(+) metastatic breast cancer patients was performed. RESULTS: Taxanes are the agents most comm

Pathological complete response induced by first-line chemotherapy with single agent docetaxel in a patient with advanced non small cell lung cancer.

BACKGROUND: Defining the optimal treatment for patients with inoperable non small cell lung cancer (NSCLC), presenting with metastatic mediastinal lymph nodes, is challenging. Nevertheless, preoperative chemotherapy or radiotherapy might offer a chance for these patients for radical surgical resection and, possibly, complete recovery. CASE PRESENTATION: A 62-year old man with IIIA-N2 inoperable NSCLC was treated with first-line single agent docetaxel. A platinum-based treatment, though considered more active, was ruled out because of renal impairment. The patient tolerated the treatment very

[Successfully resected stage IIIA non-small cell lung cancer with mediastinal lymphnode metastasis after chemotherapy of cisplatin and docetaxel combined with concurrent radiation].

A 47-year-old man with no symptoms was admitted to our hospital for the treatment of NSCLC, which was incidentally detected by an X-ray examination at the mass screening. Computed tomography (CT) of the chest and FDG-PET revealed a 3.6 cm tumor in the right upper lobe with multiple lymphadenopathy in the right mediastinum. Based on these clinical findings, we classified this case as a T2N2M0, stage IIIA NSCLC. The patient consented to and received 2 courses of systemic chemotherapy consisting of cisplatin (CDDP 40 mg/m(2); day 1, 8) and docetaxel (DOC 40 mg/m(2); day 1, 8) combined with concur

Final results of a phase II trial of preoperative TAC (docetaxel/doxorubicin/cyclophosphamide) in stage III breast cancer.

BACKGROUND: The use of preoperative chemotherapy for breast cancer has been demonstrated to result in similar disease-free survival (DFS) and overall survival (OS) as postoperative adjuvant chemotherapy. Additionally, the rate of pathologic complete response (pCR) in the breast after preoperative chemotherapy has been shown to correlate with survival. The objective of this study was to determine the pCR rate in patients with stage III breast cancer treated with 4 cycles of TAC (docetaxel 75 mg/m2, doxorubicin 50 mg/m2, cyclophosphamide 500 mg/m2) on day 1 before surgery. Neutropenic enterocolitis (typhilitis) associated with docetaxel therapy in a patient with non-small-cell lung cancer: case report and review of literature.

Neutropenic enterocolitis (NE) is an unusual acute complication of neutropenia, most often associated with leukemia and lymphoma which is characterized by segmental cecal and ascending colon ulceration that may progress to necrosis, perforation, and septicemia. We present a case of neutropenic enterocolitis in a patient with non-small-cell lung cancer who received docetaxel and flavopiridol as part of a phase I clinical trial and review cases in the literature where docetaxel was involved. Given the increased use of docetaxel and other taxanes in the treatment of advanced lung cancer, physicia

[A case of advanced gastric cancer that responded to docetaxel with low-dose 5-FU and cisplatin combination chemotherapy after becoming chemoresistant to M-FLP].

We report a case of advanced gastric cancer that responded to docetaxel with low-dose 5-FU and cisplatin combination chemotherapy after becoming chemoresistant to M-FLP. A 52-year-old male was diagnosed with type 3 gastric cancer of angulus (poorly differentiated adenocarcinoma) with left neck, Virchow, mediastinal and abdominal lymph nodes metastases. The patient was treated with 5 courses of M-FLP (MTX + 5-FU + LV + CDDP), and the effect of this therapy was PR, but the tumor was chemoresistant to the sixth course of this therapy. After 7 courses of M-FLP, docetaxel (TXT) with low-dose FP (5-

The treatment of multiple myeloma with docetaxel (an ECOG study).

BACKGROUND: Docetaxel has activity against multiple malignancies. In a previous ECOG study of untreated patients with multiple myeloma, paclitaxel was found to have mild activity, but had excessive toxicity. Docetaxel was evaluated in patients with relapsing or refractory multiple myeloma. METHOD: Well-documented patients with relapsing or refractory multiple myeloma, who had received no more than two prior combination chemotherapy regimens, were treated with docetaxel 75 mgm(-2) intravenously over 1h every 3 weeks. Patients were evaluated after two and four cycles of treatment. Standard ECOG

Colitis associated with docetaxel-based chemotherapy in patients with metastatic breast cancer.

BACKGROUND: Docetaxel and vinorelbine as combined treatment for metastatic breast cancer can have the dose-limiting toxic effects of mucositis and neutropenic fever. We report unexpected ischaemic colitis in six patients associated with docetaxel-based therapy, three of whom were treated in a phase I study designed to establish the maximum tolerated dose of this combination with the prophylactic use of granulocyte-colony-stimulating factor. METHODS: Between August, 1997, and December, 1998, 14 patients with metastatic breast cancer were treated with vinorelbine, docetaxel, and granulocyte-col

Phase II study of docetaxel in patients with epithelial ovarian carcinoma refractory to platinum.

We analyzed the efficacy and toxicity of docetaxel in patients with ovarian cancer who failed previous chemotherapy with platinum. Fifty-five patients with measurable ovarian cancer were entered in this Phase II study at The University of Texas M. D. Anderson Cancer Center. Treatment consisted of 100 mg/m2 docetaxel given i.v. every 3 weeks. Because of hypersensitivity reactions, premedication with steroids and antihistamine was initiated during the study. Twenty-two (40%) patients responded (there were 3 complete responders and 19 partial responders). Twenty-one (38%) patients had stable dise

Standard Clinical Characteristics Fail to Predict Mild Chemotherapy-induced Peripheral Neuropathy in Breast Cancer Patients.

BACKGROUND/AIM: Patients with breast cancer and risk factors are often treated with taxane-based chemotherapy potentially leading to peripheral neuropathy (CIPN). Considering the limited treatment options for CIPN, early diagnosis is desirable. This study aimed to contribute to the knowledge of risk factors that may help achieving this goal. PATIENTS AND METHODS: Ninety-six patients, 48 patients without and 48 patients with mild CIPN following paclitaxel- or docetaxel-based chemotherapy, were included in this retrospective study. Fourteen chemotherapy- or patient-related characteristics inclu

Associations of surgery type, age, and body mass index with cervicothoracic muscle viscoelastic properties after neoadjuvant taxane-based chemotherapy in patients with breast cancer: An observational study.

Breast cancer treatments, including surgery and chemotherapy, may affect the musculoskeletal system even before the patient has received postoperative radiotherapy. This study aimed to investigate the viscoelastic properties of the pectoralis major (PM), upper trapezius (UT), and sternocleidomastoid (SCM) muscles in patients with breast cancer treated with neoadjuvant taxane-based chemotherapy and to examine their associations with surgery type, age, and body mass index (BMI). In this observational study, viscoelastic muscle properties of the PM, UT, and SCM muscles were assessed using the Myo

Current Evidence on Cabazitaxel for Prostate Cancer Therapy: A Narrative Review.

The incidence of prostate cancer (PC) has recently increased in Japan. Androgen deprivation therapy (ADT) has been a key treatment in patients with castration-sensitive PC (CSPC); however, resistance typically emerges through multiple mechanisms, leading to metastatic castration-resistant PC (mCRPC). Taxane-based therapy (i.e., docetaxel, cabazitaxel) has been standard care in patients with mCRPC. New evidence supporting the addition of androgen receptor signaling inhibitors (ARSIs, e.g., enzalutamide, abiraterone) to docetaxel and ADT for patients with metastatic CSPC (mCSPC) raises questions

Pegylated liposomal doxorubicin + cyclophosphamide followed by taxane as adjuvant therapy for early-stage breast cancer: a randomized controlled trial.

BACKGROUND: Pegylated liposomal doxorubicin (PLD) was shown to have comparable efficacy to doxorubicin, with significantly reduced cardiotoxicity. This study evaluated the cardiotoxicity and efficacy of the PLD-based regimen compared with those of the doxorubicin-based regimen as adjuvant therapy for early-stage breast cancer (BC). METHODS: In this open-label, randomized controlled trial, patients with early-stage BC were assigned to receive either 4 cycles of PLD (study group) or doxorubicin (control group) plus cyclophosphamide followed by 4 cycles of docetaxel/paclitaxel.
