Tracing the journey: Patient engagement in long-term antiretroviral therapy care in Malawi.

BACKGROUND: Consistent engagement in antiretroviral therapy (ART) care is crucial to improve health outcomes and reduce HIV transmission. This study examined ART engagement patterns among ART clients during their first two years at two public ART clinics in Lilongwe, Malawi. Retention support for new initiates is provided by ART "Buddies" or through a two-way texting (2wT) system for those with phones and interest. METHODS: ART engagement patterns were assessed over six-month intervals (>0-6, > 6-12, > 12-18, > 18-24) among clients stratified by retention support group: 1) Buddy without phone

Exploring brushing and questionnaire data from a feasibility randomised control trial of a school-based smart, connected toothbrushing program.

BACKGROUND: Children's oral health continues to be a key issue, with dental caries remaining one of the most prevalent non-communicable diseases. One of the main ways to prevent dental caries is to carry out adequate toothbrushing with fluoridated toothpaste. Connected or 'smart' toothbrushes can provide real-time feedback on brushing behaviour to individuals or be used to provide feedback to groups for example within a school setting. This study aimed to identify the engagement level of school children with a smart, connected toothbrush, explore brushing frequency, timing, coverage, and durat

Nudging towards COVID-19 and influenza vaccination in children with medically at-risk conditions.

BACKGROUND: Non-coercive 'nudge' interventions have shown the potential to promote health behaviours. This study aimed to evaluate the impact of nudge interventions on COVID-19/influenza vaccine uptake among children with medically at-risk conditions. METHODS: Randomized controlled trials (RCTs) were conducted, and participants were randomized (1:1) to standard care or intervention. The multicomponent nudge interventions included three text messages with video links, developed using the MINDSPACE framework. RESULTS: In the COVID-19 RCT, 554 were randomized to intervention and 552 to standard

An experimental examination of appearance-related safety behaviors in a clinical sample of women.

Appearance-related safety behaviors (ARSBs) have been identified as a key mechanistic target in individuals with elevated appearance concerns, social anxiety symptoms, and body dissatisfaction. The aim of the present study was to experimentally test the effect of fading these behaviors in individuals with body dysmorphic disorder (BDD), social anxiety disorder, and/or an eating disorder (ED). Ninety-four female participants were randomized to either a 1-month text message-based ARSB fading condition (n = 47) or a self-monitoring control condition (n = 47). Findings demonstrated that individual

Heart up! RCT protocol to increase physical activity in cardiac patients who report hopelessness: Amended for the COVID-19 pandemic.

Hopelessness is associated with decreased physical activity (PA) and increased adverse events and death in patients with ischemic heart disease (IHD). Rates of PA in patients with IHD continue to be low in both hospital-based cardiac rehabilitation and home settings. While researchers have investigated strategies to increase PA among patients with IHD, interventions to promote PA specifically in IHD patients who report hopelessness are lacking. We describe the protocol for a NIH-funded randomized controlled trial designed to establish the effectiveness of a 6-week intervention (Heart Up!) to p

Exploring Sexting and Online Sexual Victimization during the COVID-19 Pandemic Lockdown.

The COVID-19 pandemic lockdown has impacted daily routines, forcing people to stop socializing in person and changing the way people express their feelings and their romantic or sexual interactions. Social distancing has changed the way people behave online, and we expect that engagement in sexting and online sexual victimization behaviors have increased during lockdown. The aim of this paper is to study the prevalence of sexting and online sexual victimization behaviors during the COVID-19 lockdown in Spanish adults in order to explore how social distancing has affected these behaviors. The s

SMS messaging to improve retention and viral suppression in prevention of mother-to-child HIV transmission (PMTCT) programs in Kenya: A 3-arm randomized clinical trial.

BACKGROUND: Pregnant and postpartum women living with HIV (WLWH) need support for HIV and maternal child health (MCH) care, which could be provided using short message service (SMS). METHODS AND FINDINGS: We compared 2-way (interactive) and 1-way SMS messaging to no SMS in a 3-arm randomized trial in 6 MCH clinics in Kenya. Messages were developed using the Health Belief Model and Social Cognitive Theory; HIV messages were integrated into an existing MCH SMS platform. Intervention participants received visit reminders and prespecified weekly SMS on antiretroviral therapy (ART) adherence and M

Qualitative evidence synthesis informing our understanding of people's perceptions and experiences of targeted digital communication.

Health communication is an area where changing technologies, particularly digital technologies, have a growing role to play in delivering and exchanging health information between individuals, communities, health systems, and governments.[1] Such innovation has the potential to strengthen health systems and services, with substantial investments in digital health already taking place, particularly in low‐ and middle‐income countries. Communication using mobile phones is an important way of contacting individual people and the public more generally to deliver and exchange health information. Su

To protect or not to protect: examining reasons for sun protection among young women at risk for skin cancer.

We aimed to further the understanding of the low rates of sun protection in young women at risk for skin cancer. Six-hundred-sixty-one daily diary entries were received via text message over 14 days from 56 young women at moderate to high risk of developing skin cancer. Women reported whether or not they used sun protection and also listed what their reasons were for using protection or not using sun protection each day. Multi-level modeling was used to examine the influence of study variables when predicting daily sun protection or lack of protection. The number of days in which sun protectio

Aphasic Dystextia as Presenting Feature of Ischemic Stroke in a Pediatric Patient.

Aphasia is an important presenting symptom of acute stroke. With increasing reliance on electronic communication, incoherent texting or "dystextia," which is a subset of aphasia that is reflected in text messages, can be a useful tool for symptom recognition and analysis. It can be a red flag for the family and therefore can help in early identification of an acute neurological deficit. It is also useful for providers to reliably analyze the deficit as well as establish a timeline of evolution of symptoms. There have been case reports where dystextia has been the presenting feature of stroke o
