Complete Scrotal Agenesis: New Surgical Approach Using Self-inflating Tissue Expander.

Complete agenesis of the scrotum is an extremely rare entity: to date, only 8 cases have been reported. The authors describe 1 case carrying a heterozygous genomic variant in exon 17 of the MAP3K1 gene, whose surgical treatment included osmotic prosthesis implant to achieve reconstruction of a new scrotum. By constant and gradual expansion, self-inflating prothesis avoids patient discomfort and facilitates orchiopexy.

Safety of tissue expander/implant versus autologous abdominal tissue breast reconstruction in postmastectomy breast cancer patients: a systematic review and meta-analysis.

BACKGROUND: Breast reconstruction after mastectomy for breast cancer should be informed by evidence-based knowledge, such as complication rates. The authors compared the safety of tissue expander/implant reconstruction with that of autologous abdominal tissue reconstruction. METHODS: A systematic literature review identified peer-reviewed studies published from January of 2000 to October of 2012 that compared tissue expander/implant against autologous abdominal tissue reconstruction in the MEDLINE, EMBASE, Cochrane Library, PubMed, and ProQuest Dissertations and Theses databases. Two reviewer

Modification in the technique of ear framework fabrication for congenital microtia.

BACKGROUND: Ear framework fabrication with autologous costal cartilage is considered to be one of the important factors for successful auricular reconstruction. Congenital microtia in adults has many particular physiological characteristics different from that in children. The authors report an effective ear framework-fabricating technique in congenital microtia in adults. Technical points regarding the reduction of the amount of costal cartilage harvested and the increase of the flexibility of the cartilage are discussed. METHODS: One hundred three adult patients diagnosed with unilateral co

The use of intraabdominal tissue expanders as a primary strategy for closure of giant omphaloceles.

BACKGROUND: Giant omphaloceles present a unique challenge to pediatric surgeons because of the difficulty in obtaining timely, tension-free closure of tissues over the defect. Reports of the use of tissue expanders in the subcutaneous space, intramuscular space, or intraabdominal cavity have illustrated the usefulness of this technique to provide biologic closure of abdominal wall defects. However, these reports have focused on use of tissue expanders as a second-line treatment after other options, such as silastic silos or attempted primary closure, have failed. METHODS: We report 2 cases in

Secondary revisions due to unfavourable results after microtia reconstruction.

Autologous rib cartilage grafting is a basic modality in auricular reconstruction, especially in cases of congenital microtia. However, due to heavy scarring at operative sites, lack of healthy normal tissues and an unfavourable blood circulation status, revision auricular reconstruction has always presented a surgical challenge to reconstructive surgeons. Rib cartilage grafts were used for revision auricular reconstruction in six cases. In all six, previous attempts at auricular reconstruction had failed due to graft or implant infection. Four cases had previously undergone a rib cartilage gr

Subtotal ear reconstruction for correction of type 3 constricted ears.

Constricted ears are characterized by four features: (a) lop deformity, (b) protrusion, (c) low ear position, and (d) decreased ear size. These deformities, resulting from inadequate length of the helix, have been described by Tanzer (1975) as a purse-string closure of the ear. Constricted ears are classified into types 1, 2, and 3 according to the severity of the deformity. Type 3 constricted ear, the most severe, with decreased size and loss of the upper half of the normal ear contour, is classified currently as a mild form of microtia. Therefore, autogenous rib cartilage was used to reconst

Acute ureteral elongation in two animal models using a balloon expander.

BACKGROUND AND OBJECTIVE: Repair of ureteral injuries and strictures often necessitates a major reconstructive procedure such as a psoas hitch, Boari flap, renal mobilization, ileal interposition, or autotransplantation. Tissue expanders have been used to elongate nerves and arteries. We examined the effects of acute ureteral elongation in two animal models. MATERIALS AND METHODS: In eight female rabbits, we exposed the left ureter through a midline incision and placed a Ruiz-Cohen balloon beneath the undermined portion. The expander was then inflated until the ureter was tightly stretched ac

Amputation of penis after circumcision--penoplasty using expandable prosthesis.

A two-year-old boy was referred for correction of an amputation of penis following ritual circumcision. The reconstruction involved mobilization of the profound part of corpora cavernosa and coverage by a vascularised flap taken from the hypogastric area through the use of a sub-cutaneous expandable prosthesis. After two years, appearance of the neo-penis is satisfactory considering the initial lesions.

The role of tissue expanders in vaginoplasty for congenital malformations of the vagina.

There are many techniques of vaginoplasty in congenital malformations of the vagina, all of which involve the use of labial and perineal skin flaps. The results are not uniformly satisfactory because of delayed stenosis in many patients. To avoid this complication, we have used labial expansion with tissue expanders to obtain an elastic and sufficiently large skin flap. We recommend this technique in patients with the adrenogenital syndrome associated with varying degrees of virilisation and in other intersex anomalies.

Tissue expanders for soft tissue reconstruction in the head and neck area--requirements and limitations.

OBJECTIVES: The repair of skin defects in the head and neck region still poses a significant problem for many clinicians. Tissue expansion is described as a treatment option providing good color, texture, and thickness match of the expanded skin. Unfortunately, the complication rates for tissue expansion range from 0 to 48 %. Therefore, the purpose of this study was to investigate risk factors for the use of tissue expanders in head and neck reconstructions. MATERIALS AND METHODS: Forty-nine patients with skin deficits in the head and neck area underwent tissue expansion. Sixty-two implanted …

Local infiltration of tranexamic acid during tissue expander insertion reduces postoperative drainage: A retrospective matched study.

BACKGROUND: Tranexamic acid (TXA) has gained increasing recognition for its efficacy in minimizing blood loss, ecchymosis, and edema. METHODS: This study investigates whether local infiltration of TXA during tissue expander insertion safely reduces postoperative drainage. A single-surgeon retrospective cohort study was performed to analyze patients undergoing tissue expander insertion between 2021 and 2023. Following expander insertion, patients in the intervention group received 5 mL of TXA (0.25 g in NaCl 0.9 %) infiltrated into the dissection area, whereas the historical controls did not.

Reconstructing Failure: Assessing Surgical and Patient-Reported Outcomes after Loss of Initial Breast Reconstruction.

BACKGROUND: Breast reconstruction failure, defined as the removal of a prosthetic device or flap without immediate replacement, can be traumatic. The authors describe the progression of tissue expander (TE), implant, or autologous breast reconstructive failure, and assess the patient-reported outcomes (PROs) among patients who undergo additional reconstruction. METHOD: Patients undergoing TE, implant, or autologous breast reconstruction between 2017 and 2022 were included, and patients with reconstructive failures were identified. Outcomes of interests included receipt of additional reconstru

Direct-to-Implant vs Tissue Expander Placement in Immediate Breast Reconstruction: A Prospective Cohort Study.

BACKGROUND: Direct-to-implant (DTI) breast reconstruction after mastectomy has gained increasing popularity. While concerns over ischemic complications related to tension on the mastectomy flap persist, newer techniques and technologies have enhanced safety of this technique. OBJECTIVES: To compare clinical and patient-reported outcomes of DTI and 2-stage tissue expander (TE) reconstruction. METHODS: A prospective cohort design was utilized to compare the incidence of reconstructive failure among patients undergoing DTI and TE reconstruction by unadjusted bivariate and adjusted multivariable

Ear Reconstruction with the Combination of Expanded Skin Flap and Medpor Framework: 20 Years of Experience in a Single Center.

BACKGROUND: In ear reconstruction, the difficulty lies in reestablishing the ear's bionic form with adequate skin coverage and an appropriate framework. Skin expansion and a porous polyethylene (i.e., Medpor) framework are often used for ear reconstruction. However, a long-term review of the combined application of the expanded skin and Medpor framework has not been reported. This article reviews ear reconstruction combining these two factors over the past 20 years in the authors' center to summarize the surgical technique and analyze the postoperative results and complications. METHODS: A re

Therapeutic Translation of Epicardial Adipose Tissue Targeting in Heart Failure With Preserved Ejection Fraction: How Far Are We?

Heart failure with preserved ejection fraction (HFpEF) exhibits an inflammatory-metabolic phenotype in association with multiple factors. Among them, epicardial adipose tissue (EAT), a type of visceral adipose tissue surrounding the heart, has been gaining attention. EAT expansion is associated with the development of HFpEF, the increased risk for heart failure-related mortality in patients with HFpEF, independent of age, body mass index, diabetes, and sex. Thus, reducing EAT expansion has been proposed as a promising therapeutic strategy for treating HFpEF. Although specific treatment for EAT

Breast reconstruction with implants, tissue expanders and AlloDerm: predicting volume and maximizing the skin envelope in skin sparing mastectomies.

AlloDerm has been used as a tissue supplement in conjunction with the pectoralis major muscle to provide full coverage over an implant in breast reconstruction. While this method of reconstruction has shown promising results there is little known on the relationship of AlloDerm size and potential immediate expansion volume. A retrospective chart review was completed evaluating all tissue expander or primary implant reconstructions using AlloDerm. Data recorded included: The type/size of implant/expander, dimensions of the AlloDerm used, initial fill volume, number of expansions and time period
