Importance of biomarkers in streptococcal acute tonsillitis & peritonsillar abscess.

Background & objectives Acute tonsillitis is a disease that can often be cured with medical treatment. However, complications may occur during this disease process. One of these complications is peritonsillar abscess. In recent years, biomarkers have been frequently used in the diagnosis of diseases. The aim of the study was to reveal whether peritonsillar abscess develops after acute tonsillitis, and acute tonsillitis can be differentiated using biomarkers and which biomarker has higher predictive value for this differentiation. Methods The control group consisted of individuals who were oper…

Quality of Life Postchronic Tonsillitis Treatments: An Original Research.

INTRODUCTION: The tonsillectomy is a common surgical procedure performed for the treatment of chronic tonsillitis (CT). In the present study, we aim to evaluate the quality of life (QOL) after CT. MATERIALS AND METHODS: We conducted a retrospective study among 100 tonsillectomy patients whose data were collected from the department records. The QOL was evaluated using the "Glasgow Benefit Inventory." We assessed various parameters pre-and post-surgery. We deliberated P < 0.05 as significant when comparing the pre-and post-surgical QOL. RESULTS: We observed that of the 100 subjected selected …

Analysis of Biochemical Parameters in Children with Chronic Tonsillitis.

Chronic tonsillitis are frequent in pediatric pathology with numerous involved risk factors and pathogenic mechanisms. In this study, epidemiological data and biochemical values addressed to inflammation and mineral, lipid and hepatic metabolism were analyzed for 37 children of school age with chronic tonsillitis. We found that in the majority of cases, chronic tonsillitis was associated with increasing number of blood circulating inflammatory cells, high values of transaminases, cholesterol, triglycerides and low values of procalcitonin, C-reactive protein, calcium, vitamin D and serum iron. …

Predisposing factors and management of complications in acute tonsillitis.

CONCLUSION: RPA and NF was diagnosed with a sensitivity/specificity of 100%/94% in patients with acute tonsillitis and without suspicion for disease complication after ENT examination, but an age >35 years and serum CRP >15.5mg/dl. BACKGROUND: Acute tonsillitis represents a frequent disease in the otorhinolaryngology. Some patients exhibit disease aggravations resulting in (descending) peritonsillar abscess (PTA, dPTA), para-/retropharyngeal abscess (PPA, RPA), or necrotising fasciitis (NF). The study analyses the underlying predisposing factors. METHODS: The retrospective cohort study inclu…

Tonsillitis in children: unnecessary laboratory studies and antibiotic use.

BACKGROUND: The Finnish Current Care Guidelines on diagnostics and treatment of sore throat recommend the treatment of only group A streptococcus (GAS) positive cases with penicillin. The aim of the study was to evaluate how these guidelines are followed in the pediatric emergency unit. METHODS: We analyzed retrospectively the data on microbiological studies and blood tests done, and data on prescribing of antibiotics, of 200 children admitted for febrile exudative tonsillitis. RESULTS: After the clinical diagnosis of exudative tonsillitis, antigen test and/or culture for GAS identification …

Necrotizing tonsillitis caused by group C beta-hemolytic streptococci.

Tonsillitis and pharyngitis are among the most common infections in the head and neck. Viral tonsillitis is usually caused by enterovirus, influenza, parainfluenza, adenovirus, rhinovirus and Epstein-Barr virus (causing infectious mononucleosis). Acute bacterial tonsillitis is most commonly caused by group A beta-hemolytic streptococci. On the other hand, pseudomembranous and necrotizing tonsillitis are usually caused by fusiform bacilli and spirochetes. Here we report what is, to our knowledge, the first case of necrotizing tonsillitis caused by group C beta-hemolytic streptococci.

Efficacy of a homeopathic complex on acute viral tonsillitis.

BACKGROUND: Acute viral tonsillitis is an upper respiratory tract infection prevalent in school-aged children. Because this condition is self-limiting, conventional treatment options are usually palliative. Homeopathic remedies are a useful alternative to conventional medications in acute uncomplicated upper respiratory tract infections in children, offering earlier symptom resolution, cost-effectiveness, and fewer adverse effects. This study aimed to determine the efficacy of a homeopathic complex on the symptoms of acute viral tonsillitis in African children in South Africa. METHODS: This w…

Wegener's granulomatosis with an atypical presentation as acute tonsillitis.

Wegener's granulomatosis (WG) is a rare necrotizing granulomatous vasculitis of unknown etiology commonly involving vessels, the upper and lower respiratory system and the kidneys. There are various symptoms and signs of WG due to a wide spectrum of involvement sites. The initial presentation of the disease as intraoral lesions is rare. In this paper, a case of WG that presented with a necrotic lesion confined to the right tonsil before other symptoms and signs occurred is described. The diagnosis was established based on the clinical presentation and the histopathological findings of the char

Penicillin failure in the treatment of streptococcal pharyngo-tonsillitis.

The inadequate penetration of penicillins into the tonsillar tissues and tonsillar surface fluid and microbiologic interactions between Group A beta-hemolytic streptococci (GABHS) and other pharyngo-tonsillar bacterial flora can account for their failure in eradicating GABHS pharyngo-tonsillitis (PT). These interactions include the presence of beta-lactamase producing bacteria (BLPB) that "shield" GABHS from penicillins, the absence of bacteria that interfere with the growth of GABHS, and the coaggregation between GABHS and Moraxella catarrhalis. In the treatment of acute tonsillitis, the use …

Acute necrotizing herpetic tonsillitis: a report of two cases.

The finding of herpetic tonsillitis is rare. Tonsillectomies are usually done for children with recurrent chronic tonsillitis, while viral throat infections are generally self-limiting. We present two cases: A 5 year-old girl, with atypical hemolytic anemia managed with Eculizumab, who presented with a pharyngeal infection and tonsillar enlargement that did not respond to intravenous antibiotics or antifungal therapies; and a 30 year-old man who presented with upper airway obstruction and fever; bilateral tonsillectomies were performed. Histopathological examination showed a necrotizing tonsil…

[Certain peculiarities of the clinical course of chronic tonsillitis].

The present review of the literature data demonstrates that the frequency of tonsillectomies has reduced during the last 10 years. The authors consider the possible causes accounting for this decrease. In addition, results of an epidemiological study are presented based on the analysis of medical records of the patients with chronic tonsillitis complicated by paratonsillar abscess who were admitted to the Department of Otorhinolaryngology of No 1 City Clinical Hospital between 2002 and 2010. The severity of the inflammatory process was evaluated from the results of radioautographic investigati…

[A case of thyroid crisis in acute tonsillitis treatment].

We report a case of fatal thyroid crisis induced by acute tonsillitis. A 33-year-old woman with untreated hyperthyroidism developed thyroid crisis during acute tonsillitis treatment. The four days passing from crisis onset to treatment initiation unduly compromised her condition, resulting in death. Such cases point up the need for prompt thyroid crisis diagnosis and treatment, the difference between a proactive life-sustaining response and a negative mortal result.
