Dynamic balance of the aortomitral junction.

OBJECTIVE: The aortic and mitral valves have been studied in isolation, as if their functions were independent. We hypothesized that both valves work in synchrony on the basis of the shared myocardial pump and orifice. METHODS: Six sonometric crystals (7 sheep) were placed in both trigones, the midpoint of the anterior and posterior anulus, and the lateral extremities of the posterior anulus. In a separate series of animals, 3 crystals (8 sheep) were implanted in the aortic annular base of the right, left, and noncoronary sinuses of Valsalva. In an acute, open-chest model, under stable hemody

Expanding the clinical spectrum of Lemierre syndrome and Lemierre-like septic internal jugular vein thrombophlebitis: a four-case series.

INTRODUCTION: Lemierre syndrome is a rare but potentially life-threatening condition, characterized by septic thrombophlebitis of the internal jugular vein and classically following an oropharyngeal infection. Although traditionally associated with young, otherwise healthy adults and Fusobacterium necrophorum infection, contemporary reports increasingly recognize a broader clinical spectrum, including culture-negative and Lemierre-like presentations. CASE PRESENTATION: We describe four patients with classic Lemierre syndrome or Lemierre-like septic internal jugular vein thrombophlebitis manag

Selective Bridging Vein Embolization of a Transverse-Sigmoid Sinus Dural Arteriovenous Fistula Draining via the Jugular Foramen: A Case Report.

OBJECTIVE: Bridging veins adjacent to the jugular foramen (JF-BVs) are rare drainage pathways in dural arteriovenous fistulas (DAVFs). We report a unique case of a transverse-sigmoid sinus DAVF (TS-DAVF) with retrograde venous drainage via a JF-BV, successfully treated with selective transvenous coil embolization. CASE PRESENTATION: A man in his 60s presented with a 2-month history of right-sided pulsatile tinnitus. Cerebral angiography revealed a high-grade TS-DAVF supplied by branches of the external carotid and vertebral arteries. Venous drainage involved a compartmentalized portion of the

Postpartum Cerebral Venous Sinus Thrombosis: A Case Report.

INTRODUCTION: Cerebral venous thrombosis (CVT) remains one of the most frequent cause of stroke in young adults although it is a rare disorder. It has a serious emergency manifestation during pregnancy and postpartum. It is frequently under diagnosed and limited publications in developing countries especially sub-Saharan Africa. CASE PRESENTATION: Here, we report the case of a 35-year-old female, para 8 and gravida 8, brought to emergency department on her ninth day postpartum, having severe headaches and confusion; during her admission to emergency, she suffered two episodes of generalized s

Case Report of a Novel Technique for Repair of the Vertebral Artery During Cranial Surgery.

Surgery of complex cranial base lesions carries a high risk of damage to main vessels, often resulting in life-changing or even life-threatening injuries. We describe a rapid, effective, and noninvasive application of a collagen-based hemostatic patch to repair the vertebral artery during cranial surgery. A 61-year-old male patient underwent retrosigmoid craniotomy to remove a foramen magnum meningioma that encased the vertebral artery. A linear incision was made behind the ear and standard retrosigmoid craniotomy was performed with preservation of the transverse and sigmoid sinuses. The dura

Gradenigo syndrome: unusual consequence of otitis media.

INTRODUCTION: In 1904, Giuseppe Gradenigo published his case series on the triad of ipsilateral abducens nerve palsy, facial pain in the trigeminal nerve distribution, and suppurative otitis media, which would subsequently be referred to as Gradenigo syndrome. CASE REPORT: Our patient was a 36-year-old female, 23 weeks pregnant, with a 6-day history of right-sided otalgia and hearing loss and a 4-day history of purulent otorrhea, who presented with severe, holocephalic headache, meningeal signs, fever, photophobia, and mental status decline. Lumbar puncture yielded a white blood cell count of

Neuropeptide Y- and substance P-like immunoreactive nerve fibers in the rat dura mater encephali.

Density and pattern of nerve fibers with neuropeptide Y-like immunoreactivity (NPY-LI) and substance P-like immunoreactivity (SP-LI) in the rat dura mater encephali were investigated by light and electron microscopy using whole-mount preparations. NPY-LI fibers are observed throughout the encephalic dura mater. A remarkable net of NPY-LI nerve fibers is located in the walls of the sagittal and transverse sinuses. Beyond that NPY-LI network, distinct NPY-LI nerve fibers or plexus occur in the rostral falx, parietal dura mater of the olfactory bulb, supratentorial dura mater, parietal dura mater…

The Gibraltar Sign: An Anatomic Landmark for Predicting Transverse Sinus Dominance Laterality on Conventional MRI.

BACKGROUND AND PURPOSE: Physiologic asymmetry of transverse sinus (TS) caliber is common and can lead to misinterpretation of magnetic resonance venography (MRV) studies of the brain. This retrospective study assesses the ability of multiple anatomic features on axial noncontrast T1-weighted images (T1WIs) of the brain to predict congenital TS dominance. METHODS: One hundred consecutively acquired combined MRI-MRV studies of the brain were reviewed. On noncontrast axial T1WI, each reviewer measured (1) inclination of the groove for the superior sagittal sinus (SSS), (2) angulation of the post

Post-traumatic cerebral venous sinus thrombosis complicated with syndrome of inappropriate antidiuretic hormone secretion: a case report.

BACKGROUND: Cerebral venous sinus thrombosis is a rare but potentially life-threatening condition resulting from the formation of a thrombus in the venous sinuses in the brain, resulting in impaired venous drainage, increased intracranial pressure, and severe neurological deficits. Traumatic cerebral venous sinus thrombosis is a rare but potentially devastating complication of head trauma. Cerebral venous sinus thrombosis can present with a variety of symptoms including headache, vomiting, blurred vision, loss of consciousness, seizures, and coma. Syndrome of inappropriate antidiuretic hormone

Mesh suture repair of rectus diastasis with and without a concurrent ventral hernia.

PURPOSE: The optimal technique for rectus diastasis (RD) repair, particularly when a ventral hernia is present, remains undefined. Mesh suture is a novel device designed to resist suture pull-through in high-tension closures. This study evaluates the technical feasibility and early outcomes of mesh suture use for linea alba plication during open abdominoplasty, with and without concurrent ventral hernia repair. METHODS: A retrospective review of consecutive cases of abdominoplasty with mesh suture linea alba plication was performed between January 2023–2025. Patients who underwent concurrent p

A large growing occipital meningocele with Dandy-Walker syndrome: A case report and review of the literature.

BACKGROUND: Dandy-Walker syndrome (DWS) is a well-known developmental anomaly. An occipital meningocele (OMC) is recognized as a malformation that is relatively often associated with DWS, but the association of DWS with OMC has been reported in approximately 40 cases. We present herein a rare clinical course of DWS with OMC, in which the sac was small at birth and became progressively larger. CASE DESCRIPTION: A 5-day-old baby boy was referred to our hospital due to OMC. He was born at 33 gestational weeks due to premature rupture of the membranes. He was diagnosed as having DWS associated wi

Gunshot Wound to the Posterior Fossa With a Transcerebellar Retromesencepahlic Bullet Path, Transient Mutism, and Unexpected Functional Recovery: The Pivotal, Energy-Absorbing Function of the Petrous Bone and Tentorial Leaflet.

Given the abundance of vital neurovascular structures, gunshot wounds (GSWs) to the posterior fossa are generally fatal. We present a unique such case where a bullet entered the petrous bone, traversed the cerebellar hemisphere and overlying tentorial leaflet, and reached the dorsal aspect of the midbrain, resulting in transient cerebellar mutism with an unexpectedly favorable functional recovery. A 17-year-old boy sustained a GSW to the left mastoid region with no exit wound and presented with agitation and confusion, ultimately leading to a coma. Head CT revealed a bullet trajectory through

Comparison of ascending aortic cohesion between patients with bicuspid aortic valve stenosis and regurgitation.

OBJECTIVES: A bicuspid aortic valve (BAV) is commonly associated with aortic wall abnormalities, including dilatation of the ascending aorta and increased potential for aortic dissection. We compared the mechanical properties of the aortic wall of BAV patients with aortic valve stenosis (AS) and regurgitation (AR) using a dissectometer, a device mimicking transverse aortic wall shear stress. METHODS: Between March 2010 and February 2013, 85 consecutive patients with bicuspid aortic valve undergoing open aortic valve replacement at our institution were prospectively enrolled, presenting either
