Predictors of Outcomes in 1,441 Vascular Injuries: 10-Year Experience in a Large Urban Trauma Center.

BACKGROUND: Few civilian epidemiological studies have been reported for vascular injuries. The aim is to describe epidemiology, survival stratified by mechanism of injury, specific vessel injuries, outcomes of emergency department resuscitative thoracotomy (EDT), and independent predictors of mortality for vascular injuries. STUDY DESIGN: Retrospective 10-year review of patients with vascular injuries. Interventions included EDT and operative repair. Stepwise logistic regression to identify independent risk factors as predictors of mortality. Adjusted odds ratios (AORs) and 95% CIs were calcu

Closing the Gaps: Behavioral Health Screening Intervention and Follow-Up in a Level I Trauma Center.

INTRODUCTION: Substance misuse (SM) and mental health (MH) disorders are highly prevalent among trauma patients and are associated with increased mortality and recurrent injury. Despite national guidelines mandating behavioral health screening of at least 80% of admitted patients at Level I trauma centers, implementation remains challenging at urban safety net centers, which often lack dedicated behavioral health specialists and standardized referral pathways. METHODS: We conducted a single-center, pre-post quality improvement study evaluating an expanded behavioral health screening and inter…

Potentially Avoidable Spine Transfers to a Level I Trauma Center: A Five-Year Retrospective Analysis.

BACKGROUND: Emergency physicians frequently transfer patients with spine pathology to tertiary-care centers, although research suggests many are discharged without requiring a procedure or specialized imaging. Such transfers consume resources and remove patients from their families and communities. Better understanding these transfers might help identify which patients can receive care without transfer. METHODS: This was a retrospective cohort study of transfers for spine evaluation to an urban, tertiary-care, Level I trauma center from October 1, 2017-October 1, 2022. Transfers were defined

Don't blame it on the alcohol! Alcohol and Trauma Outcomes: A 10 year retrospective single-center study.

BACKGROUND: Alcohol use is associated with injury. Although ACS guidelines require screening injured patients for alcohol misuse, outcome data remains inconclusive. METHODS: This single-center, 10-year (2013-2023) retrospective study examined patients with ISS >15 at a Level I trauma center. Patients from an institutional registry were grouped by blood alcohol content (BAC): positive (>80 mg/dL, BAC-P) or negative (BAC-N). RESULTS: Of 517 patients, 401 were BAC-N and 116 BAC-P. BAC-P patients were younger, mostly male, with lower GCS, higher ISS, and higher Shock Indexes (p < 0.001). The

Emergency Operable Traumatic Pulmonary Injury at a Level 1 Trauma Center: A Retrospective Descriptive Study.

INTRODUCTION: Despite the use of body armor, emergency operable pulmonary trauma (EOPT) remains a major cause of battlefield morbidity and mortality. While EOPT during military conflicts has some features that distinguish it from EOPT in civilian settings, the 2 occurrences demonstrate overall parallel findings related to presentation, management, and outcome. The goals of the present study were to provide a descriptive analysis of the nature of EOPT and its management at a level 1 trauma center and to determine the associations between EOPT patient demographics and/or patient management and o

Description and prognostic factors of a cohort of polytraumatized patients with spinal injury in a level I trauma center.

INTRODUCTION: Traumatic spinal injuries are a significant public health issue due to their high frequency and severity, impacting the entire healthcare system, especially when neurological sequelae are involved. These injuries require comprehensive resuscitative management, prioritizing spinal injuries within the context of associated injuries. Understanding the epidemiology of spinal fractures in polytraumatized patients is essential for improving care planning, primary prevention methods, and hospital management. METHODS: This retrospective, single-center, observational study used the TRAUM

Bilateral Femur Fractures in Children: 14 Years of Experience at a Level I Pediatric Trauma Center.

BACKGROUND: Simultaneous bilateral femur fractures (SBFFs) are an extremely rare type of injury, particularly among pediatric patients. Because of their infrequency, pediatric SBFFs are poorly characterized in the literature. Here, we review our center's experience with managing them. We hypothesized that such injuries were likely to be accompanied by additional traumatic injuries to other organ systems/parts of the body. METHODS: This retrospective, observational cohort study was conducted on all patients who underwent treatment for SBFFs at a freestanding, level I pediatric hospital between

Characterization of Major Complications of Bridge Plating of Distal Radius Fractures at a Level I Trauma Center.

BACKGROUND: Comminuted, markedly displaced distal radius fractures can cause instability requiring advanced stabilization with dorsal bridge plating. However, published complication rates of bridge plating widely vary. We hypothesize that complications of bridge plating of distal radius fractures are more prevalent than published rates. METHODS: A retrospective review was performed on all patients at an academic level I trauma center treated with a bridge plate for a distal radius fracture from 2014 to 2022. RESULTS: Sixty-five wrists were included in the final analysis: average age 53 years

Trauma center risk conditions for blood alcohol-positive and alcohol misuse patients: a retrospective study.

Because few studies have assessed blood alcohol concentration (BAC)-positive risk conditions in trauma activation patients, this retrospective investigation pursued such an analysis. The parent database included consecutive trauma center admissions from January 21 to July 21 for 2018-2020. The supplementary electronic medical record audit of trauma activation patients aged 18-60 years (TA18-60) assessed alcohol misuse, smoking history, and serum bicarbonate levels. An alcohol misuse risk score was created by assigning a value of 0 (no) or 1 (yes) for each risk condition: 1) smoking history, 2)
