Crawl, Walk Fast, Run Hard: Military-Civilian Partnership Training at the Army Trauma Training Course Improves Perceived Deployment Readiness.

INTRODUCTION: The Army Trauma Training Course (ATTC) has held the responsibility of training deploying U.S. Army Forward Resuscitative and Surgical Detachments (FRSDs) since September 2001. The program of instruction (POI) consists of didactics and skill stations (Crawl), a mass casualty (MASCAL) situational training exercise (STX) (Walk), and clinical integration into an urban Level I trauma center (Run). Over the program's 25-year existence, there has been no report on the subjective assessment of the rotator's perceived effectiveness of the program as a whole. We hypothesized that a post-co

The double 90 rule: A new strategy for resuscitation in non-academic level II trauma centers.

BACKGROUND: Efficient resuscitation after trauma and shorter time to definitive hemorrhage control help improve trauma outcomes. We aimed to improve the speed and efficiency of resuscitation for critically ill trauma patients in the emergency department by involving interventional radiology and a second surgeon. STUDY DESIGN: In 2017 our community, non-academic level II trauma center implemented the Double 90 rule-for trauma patients with 2 confirmed systolic blood pressures <90 mm Hg-which involves a second activation including the interventional radiology team, backup trauma surgeon, and op

Breaking the Retention Crisis: Cognitive Rumination and Psychological Fortitude as Predictors of Departure Inclination Among Psychiatric Nurses in Egypt.

BACKGROUND: Psychiatric nurses are essential to the provision of mental health care, but they are especially prone to workplace stressors, causing an escalating retention crisis worldwide and negatively affecting their mental health. METHODS: A descriptive cross-sectional correlational research design was conducted among psychiatric nurses working in Port Said and New Damietta Mental Health and Addiction Treatment Hospitals, Egypt. A convenience sample of 265 nurses was recruited. Data were collected using three standardised tools: the Fortitude Questionnaire (20 items, four-point Likert scal

Exploring the experience of birth trauma from the midwife's perspective.

BACKGROUND: Childbirth is a complex process, and midwives involved in assisting or witnessing it often experience psychological trauma, which can impact their well-being and professional performance. Psychological trauma in midwives is under-researched, despite its implications for their mental health and the quality of childbirth services. Addressing this issue is critical for ensuring safe and compassionate care. This study aimed to explore the psychological trauma experienced by midwives during childbirth and identify key influencing factors to enhance the safety and humanization of childbi

Inpatients in an accident unit.

A one-year prospective study was undertaken in the 79-bed accident unit of the Bristol Royal Infirmary to define reasons for continuing inpatient care. This showed that, of a consecutive series of 466 patients staying in hospital for over two weeks, 35% lengthened their stay beyond that needed for acute nursing or continued medical care. A combination of factors usually led to total social dependence, the major problem being the absence of a caring relative. Other factors included pre-existing locomotor disorder or mental infirmity, unmanageable incontinence of urine after catheterisation, and

A comparison of chronic pain prevalence in Japan, Thailand, and myanmar.

BACKGROUND: Pain has been regarded as important in the improvement of quality of life (QOL). In the advanced countries of Europe and the North America, a number of large-scale epidemiological surveys on pain, particularly chronic pain, have thus been performed in general populations. However, few epidemiological surveys of chronic pain have been reported from developing countries, and no surveys appear to have examined chronic pain in the least developed countries. OBJECTIVES: To compare the incidence of chronic pain in Asian countries, using Japan as an advanced country, Thailand as a develo

Impact of Ultrasound-Guided Peripheral Intravenous Training on Central Line Placement in the Emergency Department.

INTRODUCTION: Central venous catheters are placed in the emergency department (ED) when difficult peripheral access limits care or when critical medications require central access because of their sclerotic nature or incompatibility with other drugs. Ultrasound-guided peripheral intravenous (IV) access offers a less invasive alternative. We evaluated whether implementation of a structured ultrasound-guided peripheral intravenous training program for ED nurses and technicians was associated with changes in ED central venous catheter use. METHODS: We performed a retrospective chart review at ou

Impact of the October 7 Gaza war on post-traumatic stress symptoms and quality of life in Palestinian nursing.

The October 7, 2023, Gaza War imposed profound psychological and occupational burdens on Palestinian nurses, including those in the West Bank who were indirectly affected by the conflict. This study assessed the prevalence of post-traumatic stress symptoms (PTSS) and quality of life (QoL) outcomes among nurses working in the West Bank. A descriptive cross-sectional study was conducted from January to March 2025 with 350 registered nurses recruited through stratified random sampling. Data were collected using the Impact of Event Scale-Revised (IES-R) to measure PTSS and the World Health Organiz

Treatment of Pregnant Patients With Orthopaedic Trauma.

Fracture management in pregnant patients is challenging. Anatomic and physiologic changes in pregnancy increase the complexity of treatment. Maternal trauma increases the risk of fetal loss, preterm birth, placental abruption, cesarean delivery, and maternal death. Initial resuscitation and treatment in a facility equipped to handle the orthopaedic injury and preterm births are paramount. Pelvic and acetabular injuries are potentially life threatening. The benefits and risks of surgical treatment must be carefully considered. The risks posed by anesthetic agents, antibiotic agents, anticoagula

Recombinant factor VIIa: review of current "off license" indications and implications for practice.

Uncontrolled bleeding and coagulopathy are associated with trauma, liver failure, obstetric conditions, and a variety of surgical circumstances, resulting in increased morbidity and mortality in the critically ill. Recently, the role of recombinant factor VIIa (rFVIIa) in the management of uncontrolled bleeding has attracted interest. rFVIIa was initially developed (and licensed) for the treatment of hemophilia. Increasingly, evidence suggests rFVIIa causes cessation of bleeding, improves coagulation markers, and reduces blood product use for treatment of severe bleeding due to other causes. T

Developing a high-fidelity trauma surgery simulation to enhance team performance: Lessons learned from a complex implementation.

BACKGROUND: Effective trauma care depends on technical proficiency and teamwork under pressure. While simulation has become integral to trauma education, few reports detail the methodological processes and lessons learned from developing highly complex operative simulations designed to both assess and improve team performance. METHODS: We developed and implemented a high-fidelity trauma surgery simulation to evaluate multidisciplinary intraoperative team performance. Twenty-two surgical teams-each composed of an attending surgeon, resident, anesthesiologist, circulating nurse, and scrub techn

Onychomycosis in close quarter living review of the literature.

Onychomycosis is defined as a fungal infection of the nail bed and/or nail plate. The prevalence of onychomycosis has increased dramatically as a worldwide condition in the twentieth century due to occlusive footwear, global wars and natural migration. Risk factors generally leading to onychomycosis development include bodily spread of dermatophyte and non-dermatophyte tinea pedis, peripheral vascular disease, damaged nails via sports and trauma, older age, genetics, immunodeficiency and diabetes. Many publications discuss prevalence, symptoms and treatment of the disease in individual cases,

The Effect of Hospital Trauma Level on Odds of Non-home Discharge from Hospital Following Dens Fracture.

INTRODUCTION: Dens fractures are cervical spine injuries with high morbidity and mortality in the elderly. Many patients require discharge to rehabilitation facilities or skilled nursing care, which impacts patient recovery, healthcare system resource allocation, and prognosis. This study evaluated the effect of trauma center level designation on discharge disposition in patients with dens fractures. MATERIALS AND METHODS: We conducted a retrospective study using data from a Texas State Trauma Registry from 2020 to 2022. Patient demographics, injury characteristics, and outcomes were queried

The penetrating injury risk of non-home discharge (PIRD) score: Development and validation of a predictive tool using the National Trauma Data Bank.

BACKGROUND: Penetrating trauma disproportionately affects young patients, yet many survivors cannot be discharged directly home and instead require post-acute care. Identifying these patients early could expedite rehabilitation referral and discharge planning, but no validated tool predicts non-home discharge in penetrating trauma. We developed and internally validated the Penetrating Injury Risk of Non-home Discharge (PIRD) score. METHODS: Using the National Trauma Data Bank (NTDB) Trauma Quality Improvement Program Participant Use Files (2019-2024), we selected adult patients (≥18 years) wi

The use of Open Dialogue in Trauma Informed Care services for mental health consumers and their family networks: A scoping review.

UNLABELLED: WHAT IS KNOWN ON THE SUBJECT?: Open Dialogue was developed in Finland in the 1980s by clinical psychologist, Jaakko Seikkula. It is a development of family therapy, recognises previous trauma and has proven to be very effective in situations of acute mental illness, and in particular psychosis. Trauma Informed Care is a practice based on the understanding of and responsiveness to the impact of trauma. When people have experienced trauma, they may have difficulties in their everyday life and experience negative physical health outcomes as well as the risk of developing mental ill he

The Efficacy of a Health Promotion Intervention for Indigenous Women: Reclaiming Our Spirits.

Indigenous women globally are subjected to high rates of multiple forms of violence, including intimate partner violence (IPV), yet there is often a mismatch between available services and Indigenous women's needs and there are few evidence-based interventions specifically designed for this group. Building on an IPV-specific intervention (Intervention for Health Enhancement After Leaving [iHEAL]), "Reclaiming Our Spirits" (ROS) is a health promotion intervention developed to address this gap. Offered over 6 to 8 months in a partnership between nurses and Indigenous Elders, nurses worked indivi

"A Sick Child is Always the Mother's Property": The Jane Austen Pediatric Trauma Management Protocol.

Two pediatric accidents in Jane Austen's Persuasion (1818) and one in Margaret Oliphant's The Doctor's Family (1863) are examined from the point of view of trauma management with analysis of contributing risk factors, medical management, concerns of parents and bystanders, and course of recovery. Risk factors for injury are impulsivity, poor supervision, and parents who are unable to set limits. Medical attention is swift and competent, but no heroic measures are used; the management of the injuries, concussion with loss of consciousness and dislocation of the collar bone, is consistent with t

Knowledge of Flemish paediatricians about children's oral health--results of a survey.

OBJECTIVE: To examine the knowledge paediatricians have about oral health in children. METHODS: A questionnaire on oral health knowledge was mailed to all members of the Flemish Paediatric Association representing 350 out of 676 Flemish paediatricians. Questions concerned timing of the first dental visit, effects of thumb sucking, fluoride supplements, trauma, fluorosis, mutans streptococci and caries, prevention of dental caries, nursing bottle caries and use of sealants. RESULTS: Two hundred seventy-six questionnaires (79%) were returned. Of the respondents, 71% said they have had some tra
