Competence and paternalism.

Some bioethicists have argued in favor of a sliding scale notion of competence, paternalistically requiring greater competence in relation to more significant risk. I argue against a sliding scale notion, taking issue with the positions of Allen E. Buchanan and Dan W. Brock, Ian Wilkes, and Joel Feinberg. Rejecting arguments that a sliding scale is supported by legal cases, by ordinary usage, and by fallible judgments about competence, I argue in favor of greater evidence of competence when risk is greater. Two clinical cases are examined, both involving amputation, to show that my fixed conce

The management of severely malformed newborn infants: the case of conjoined twins.

The birth of 'Siamese' twins in August 2000 whose parents refused to consent to surgery for separation required English courts to decide whether the twins could lawfully be separated despite that refusal when one twin would certainly die as a direct surgical result. The Court of Appeal unanimously upheld the trial judge's decision to authorize surgery, taking account of principles of family law, criminal law and human rights law. Parental duties to the viable twin were found consistent with the justification of allowing, without intending, natural death of the non-viable twin. The right to hum

[The scientific revolution in medicine of last quarter of XIX - first half of XX centuries: New approaches to diagnostic and treatment of diseases].

The article considers changes in mass physician consciousness approaches to diagnostic and treatment of diseases occurred in the end of XIX-first half of XX centuries. It was the result of gaining a foothold in medicine of new view on organism as a procedural system and concepts of diseases as a complex feedback reaction of this system to unfavorable effect of environmental factors. It is demonstrated that the major change in diagnostic became refusal of traditional for XIX century orientation of diagnostic search exclusively on “clinical prediction of pathological picture" in favor of complex

Knowledge, attitudes and beliefs regarding HIV among medical students in Zagreb, Croatia.

BACKGROUND: Medical students, as future health-care providers (HCPs) play a significant role in shaping attitudes towards people living with … tus (aOR = 1.62; CI: 1.17-2.27) than females. Overestimating needle injury transmission risk raised treatment refusal likelihood (aOR = 2.22; CI = 1.29-3.92) compared to those accurately informed of this risk. CONCLUSION: Results indicate lack of knowledge of HIV transmission risks after specific exposures and about PrEP, PEP and treatment effectiveness. Gender an …

Embedded ring injuries.

Two patients, each with a history of mental illness, were assessed for embedded rings. Removal of the ring was accomplished without loss of digit or further loss of function in one patient. The other patient refused treatment. Altered mental status is a common finding in patients with embedded ring injuries.

Natural history of untreated cytomegalovirus retinitis.

Cytomegalovirus infection is common in patients with AIDS, and often causes retinitis. Treatment is rarely curative, but the progression of retinitis is delayed. The untreated course of cytomegalovirus retinitis in AIDS is unknown. We report a 35-year-old man with retinitis who refused treatment. Retinitis resulted in blindness within 6 months. Measurement of cytomegalovirus genomes showed an increasing viral load in blood and urine.

The "torpillage" neurologists of World War I: electric therapy to send hysterics back to the front.

The French neurologists and psychiatrists who were mobilized during the Great War were confronted with numerous soldiers with war neuroses, often with novel clinical manifestations such as camptocormia. They addressed hysteria and pithiatism according to concepts that had been formed before the war, and many doctors considered these soldiers to be malingerers. As a result, the use of aggressive therapies to enable their prompt return to the battlefront was advocated. In 1915-1916, Clovis Vincent (1879-1947) developed a method called torpillage, a "persuasive" form of psychotherapy using faradi

Acceptability of azithromycin for the control of trachoma in Northern Tanzania.

Trachoma causes blindness; the prevention strategy includes mass antibiotic treatment. In a community in Northern Tanzania offered mass treatment with azithromycin for the control of trachoma, we used focus group discussions, individual interviews, questionnaires and direct observation to quantify, explore and contextualize reasons for acceptance or refusal of the drug. In the village studied, 76% of the population eligible to receive azithromycin were treated. Uptake was significantly higher among women (79% treated) than men (72%). Factors affecting acceptability included: local prevention n

Developments in conservative treatment for BCG-unresponsive non-muscle invasive bladder cancer.

INTRODUCTION: To reduce the risk of disease recurrence and progression of intermediate and high-risk Non-Muscle Invasive Bladder Cancers (NMIBCs), intravesical adjuvant treatment with Bacillus Calmette-Guerin (BCG) represents the standard of care, although up to 50% of patients will eventually recur and up to 20% of them will progress to Muscle Invasive Bladder Cancer (MIBC). Radical Cystectomy (RC) is the treatment of choice in this setting; however, this represents a major and morbid surgery, thus meaning that not all NMIBCs patient could undergo or may refuse this procedure or may refuse. Toward the Goal of Leaving No One Behind: Orthostatic Dysregulation.

Orthostatic dysregulation (OD), a common psychosomatic disorder in children, is caused by circulation disturbance resulting from autonomic imbalance. OD is a significant public health threat due to its association with school refusal and depression in children. During the COVID-19 pandemic, many children suffered from school closures, lack of exercise, smartphone addiction, and school refusal. The COVID-19 pandemic made it much more difficult to deliver existing approaches to patients and families with OD and increased the risk of prolonged and severe OD. In response, web-based digital health

Successful Treatment of Primary Mediastinal Seminoma With Radiotherapy Following Chemotherapy Under a Bloodless Treatment Policy Based on Patient's Religious Sentiments.

INTRODUCTION: Primary mediastinal seminoma is rare with a favorable prognosis when appropriately treated, and surgical resection after chemotherapy is the standard treatment. However, refusal of blood transfusion for religious reasons can limit curative surgical management. CASE PRESENTATION: A 47-year-old Jehovah's Witness man was found to have a 10.8-cm anterior mediastinal tumor. Biopsy confirmed seminoma. As the patient refused transfusion, definitive chemotherapy followed by radiotherapy was selected. Three cycles of chemotherapy (etoposide, ifosfamide, and cisplatin) resulted in 44% tum

Respect for illiterate or unconscious patient's autonomy as a requirement for the legality of medical procedures in the polish healthcare system: a case report and review of the literature.

According to the Polish law, each patient has the right to refuse to consent to a medical procedure, even if the refusal concerns a life-saving procedure. It may be difficult for a physician to accept this kind of decision. In each case, however, medical intervention requires patient's consent. Its lack makes physician's actions illegal. Such a situation becomes more complicated when the patient who is intellectually incompetent, unconscious or illiterate is unable to express a consent to a medical procedure. Then, the possibility and the need to document and prove the patient's consent become

Case report: Electroacupuncture for acute pain flare-up of knee osteoarthritis.

Acute pain flare-up of knee osteoarthritis (KOA) is a common disease in orthopedics and is mainly treated with analgesic drugs. Patients usually refuse to take western medicines orally owing to gastrointestinal side effects or unsatisfactory treatment results. We report the case of a 69-year-old woman who had an acute pain flare-up of right KOA induced by long-distance walking. As the patient refused medication, we used electroacupuncture (EA) to relieve her symptoms. EA with a 2-Hz frequency and a 1-2-mA intensity had an analgesic effect on the acute pain flare-up of KOA. After 12 weeks of EA
