The decline rate of tuberculosis has decreased recently in Japan. One of the problems is the tendency of increasing doctor's delay in the diagnosis of tuberculosis. One of measures against this problem is to develop a new laboratory diagnostic method. We studied anti-PPD (Purified Protein Derivative of tuberculin) antibody in serum, pleural effusion, and bronchoalveolar lavage fluid (BAL), and found its clinical usefulness in the medical practice of tuberculosis. Firstly the methods of enzyme linked immunosorbent assay (ELISA) for the antibody to PPD were examined. The expression of antibody t

STUDY OBJECTIVES: Tuberculosis (TB), the single most frequent infectious cause of death worldwide, also is a major cause of pleural effusion, which in TB usually has lymphocytic and exudative characteristics. Differential diagnosis between TB and nontuberculous pleural effusion can be sometimes difficult, representing a critically important clinical problem.
METHODS: We studied 46 patients presenting with pleural effusion to the National Sanyo Hospital between April 2000 and January 2001 (34 men and 12 women; mean age, 64 years). Ten patients (22%) had tuberculous pleurisy, 19 patients (41%)

Tuberculosis (TB) remains a major global health concern, and its diagnosis can be particularly challenging when microbiological tests yield negative results. We present two diagnostically complex cases, one pulmonary and one pleural, in which tuberculosis was confirmed only after extended clinical evaluation and diagnostic escalation, resulting in delays of approximately eight weeks and two weeks, respectively. The first patient, a 31-year-old asymptomatic man with recent TB exposure, underwent bronchoalveolar lavage (BAL), with smear microscopy and polymerase chain reaction (PCR) both negativ

Concentration of zinc in blood serum and effusion was determined in 104 patients with the pleural effusion of different etiology. The importance of zinc concentration in serum and effusion was analyzed, as well as their relation regarding the differential diagnosis of pleural effusion. It was established that the isolated zinc concentrations in serum and pleural effusion could not be used separately either in differing transudates from exudates or in the diagnosis of the pleural diseases. The average value of zinc in the pleural effusion in relation to the serum value in patients with tubercul

AIMS: Tuberculous pleurisy is an important cause of pleural effusions in areas with a high incidence of tuberculosis. In this study, we developed an IS1081-based LAMP for the detection of Mycobacterium tuberculosis complex and investigated its usefulness in the diagnosis of tuberculous pleurisy.
METHODS AND RESULTS: Investigation of pleural effusion samples from patients with tuberculous pleurisy, majority of them smear-/culture-negative, and control individuals with non-TB diseases showed that the LAMP assay with incubation time of 60 min has much higher specificity and the LAMP assay with i

The paper deals with the data of the clinical follow up observation and treatment of 186 patients with various clinical forms of tuberculosis and nonspecific pulmonary diseases complicated by spontaneous pneumothorax (SP). Treatment was composed of etiotropic, antibacterial, general invigorating and symptomatic measures and correction of the protein and volemic disorders. Local treatment was started with puncture of the pleural cavity. Drainage was applied after 2-3 punctures failed to bring lung expansion. Other measures included persistent aspiration, regular washing of the pleural cavity an

Tuberculous pleural effusion (TPE) is currently the most common form of extrapulmonary tuberculosis and remains a significant cause of pleural disease worldwide, particularly in endemic regions. It often presents with non-specific clinical symptoms, such as fever, chest pain, cough and weight loss, thereby complicating the diagnostic process. This narrative review provides an expert comprehensive clinical practice overview covering the following issues correlated with diagnosis and management of TPE: 1- epidemiology, clinical presentation and underlying pathophysiology; 2- limitations of conve

Minithoracoscopy, currently defined as thoracoscopy with instruments of a diameter between 2 and 5 mm, is part of the general evolution of endoscopy towards mini-invasiveness. Its most relevant indications in the field of medical thoracoscopy are small effusions, pleural effusions in patients with narrow intercostal spaces and suspected tuberculous pleurisy in areas of low incidence of tuberculosis. In general, it increases the versatility of medical thoracoscopy.

Parietal pleural biopsy, together with a bacteriological and chemical analysis of the pleural fluid, is often the most useful diagnostic technique in pleurisy. This is a simple technique, with moderate aspiration across a transparent filter trap, as used in blood transfusions. The procedure is done under direct vision and has the following advantages : it improves the specificity of the pleural material aspirated. It is without danger, can be repeated a number of times and thus increases the diagnostic value of the test. It avoids pleural superinfection and instrumental pneumothorax. It improv

Identifying the cause of pleural effusion is challenging for pulmonologists. Imaging, biopsy, microbiology and biochemical analyses are routinely used for diagnosing pleural effusion. Among these diagnostic tools, biochemical analyses are promising because they have the advantages of low cost, minimal invasiveness, observer independence and short turn-around time. Here, we reviewed the past, present and future of pleural fluid biochemical analysis. We reviewed the history of Light's criteria and its modifications and the current status of biomarkers for heart failure, malignant pleural effusio

INTRODUCTION: Uniportal video-assisted thoracoscopic surgery (U-VATS) is emerging as a promising approach for the treatment of pulmonary aspergilloma (PA). Nevertheless, it remains technically challenging and is associated with significant morbidity and a considerable risk of conversion to thoracotomy. This study aimed to evaluate the feasibility and safety of U-VATS in patients with PA and to identify prognostic factors that influence postoperative morbidity and conversion rates.
PATIENTS AND METHODS: We retrospectively reviewed the medical records of 51 patients who underwent U-VATS resecti

BACKGROUND: Monocytes are the predominant innate immune cells at the early stage of Mycobacterium tuberculosis (M. tb) infection as the host defense against intracellular pathogens. Understanding the profile of different monocyte subpopulations and the dynamics of monocyte-related biomarkers may be useful for the diagnosis and prognosis of tuberculosis.
METHODS: We enrolled 129 individuals comprising patients with pulmonary tuberculosis (PTB) (n = 39), tuberculous pleurisy (TBP) (n = 28), malignant pleural effusion (MPE) (n = 21), latent tuberculosis infection (LTBI) (n = 20), and healthy con

ADA was examined in pleural fluid in cases of tuberculous and cancerous pleurisy. The level of ADA in pleural fluid in tuberculosis group (59.6 +/- 33.6 U/L) was significantly higher than that of cancerous group (13.3 +/- 4.0 U/L) (P less than 0.01). The positive rates were 90.0% and 3.3% respectively.

BACKGROUND: Differentiating pleural effusion remains a challenge in clinical practice. Serum and effusion chemistries help pulmonologists assess the risk of underlying causes and select further diagnostic procedures. Amino acids (AAs) in body fluids are promising tools for estimating the probability of underlying causes. This proof-of-concept study aimed to investigate the accessibility of serum and pleural fluid AAs in distinguishing the four major etiologies of pleural effusions: malignancy, heart failure, tuberculosis, and pneumonia.
METHODS: This study prospectively enrolled 153 patients

BACKGROUND AND OBJECTIVE: The diagnosis of tuberculous pleurisy by analysis of pleural fluid using standard diagnostic tools is difficult. Recently, T-cell interferon-γ release assays (IGRA) have been introduced for the diagnosis of tuberculous pleurisy. The aim of the present meta-analysis was to establish the overall diagnostic accuracy of IGRA on both pleural fluid and peripheral blood, for diagnosing tuberculous pleurisy.
METHODS: A systematic review was performed of English language publications. Sensitivity, specificity and other measures of the accuracy of IGRA for the diagnosis tuberc
