Electrodiagnosis of Martin-Gruber connection and its clinical importance in peripheral nerve surgery.

We propose new criteria for the electrodiagnosis of Martin-Gruber connection. Forty-seven patients with cubital tunnel syndrome were studied, and 8 (17%) were found to have a Martin-Gruber connection. Eighty-seven patients without cubital tunnel syndrome were also studied, and 14 (16%) had a Martin-Gruber connection. The rate of occurrence of the anastomosis in these two groups was approximately the same. No sensory communication was demonstrated, and the lateral intrinsic muscles received more innervation from the communicating branch than the hypothenar muscles. The entry point of the crossi

Bilateral reversed palmaris longus muscle: a case report and systematic literature review.

PURPOSE: We present a case of a bilateral reversed palmaris longus muscle and a systematic review of the literature on this anatomical variation. METHODS: Routine dissection of a 90-year-old male cadaver revealed a rare bilateral reversed palmaris longus. This was documented photographically, and length and relation to anatomical landmarks were recorded. This finding stimulated a systematic review of the literature on the reversed palmaris longus variation, from which measurements were collated and statistical analysis performed to determine the prevalence, average length, relationship to sid

The pathogenesis of pneumatic tourniquet paralysis in man.

The relative importance of ischaemic and direct mechanical injury to nerves compressed by a tourniquet, in the pathogenesis of tourniquet paralysis in man has not been established. To investigate this question, conduction in ulnar or median nerve fibres has been measured in healthy subjects both at the level of the pneumatic tourniquet and distal to the tourniquet. Measurement was prior to, for the period of tourniquet inflation, and following release of the tourniquet. The earliest conduction delays and block were observed at the level of the tourniquet, particularly across the proximal tourn

Ultrasonographic and functional changes of the ulnar nerve at Guyon's canal after carpal tunnel release.

OBJECTIVE: To describe morphologic and functional modifications of the ulnar nerve at the wrist in carpal tunnel syndrome (CTS) after carpal tunnel release (CTR). METHODS: Ultrasonography was used to study the cross sectional area (CSA) of the ulnar nerve at Guyon's canal, before and 1 and 6 months after CTR, in 18 CTS patients. A parallel electrophysiological and clinical analysis was also conducted. RESULTS: CSA of the ulnar nerve significantly increased 6 months after CTR. Ten (55%) cases showed abnormal CSA values compared to a control group before surgery and five (28%) at 6 month follo

[Nerve affection in connection with bloodletting II. Four case reports].

Four blood donors with persistent pain and dysfunction of the upper extremity after venepuncture are presented. Physical examinations identified median nerve-affections in all and additionally a varying involvement of other nerves in the elbow region. Such complications appear to have developed following to a vascular lesion. They are rare but potentially disabling and should be known in blood banks and clinical departments to which these patients are referred.

[Muscle anomalies of the upper extremity as an atavistic cause of peripheral nerve disorder].

We report on 2 patients with anomalous muscles of the upper extremity causing symptoms of peripheral nerve entrapment. Our first case clinically showed a lesion of the R. dorsalis manus of nervus ulnaris produced by an accessory muscle belly at the dorsum of the hand to be identified as an extensor indicis brevis muscle. The second case presented a carpal-tunnel-syndrome, tunnel into the palm. Phylogenetically and ontogenetically these muscles are classified as atavistic. We demonstrate the homologies of the upper extremity of fishes, amphibians, reptiles, lower mammals and man, which show an

Nonarthritic inflammatory problems of the hand and wrist.

There is a group of painful inflammatory conditions that involve the soft tissues of the hand, wrist, and forearm. These generally occur without underlying systemic diseases, and the pain frequently results from tendon or nerve compression. Recognition of these difficulties can lead to appropriate treatment, often nonoperative, resulting in symptomatic cure. Occasionally, surgical treatment is required.

Anatomical considerations of US-guided carpal tunnel release in daily clinical practice.

Carpal tunnel syndrome is the most frequent compression neuropathy with an incidence of one to three subjects per thousand. As specific anatomical variations might lead to unintended damage during surgical interventions, we present a review to elucidate the anatomical variability of the carpal tunnel region with important considerations for daily clinical practice: several variants of the median nerve branches in and around the transverse carpal ligament are typical and must - similarly to the variant courses of the median artery, which may be found eccentric ulnar to the median nerve - be tak

Outcome of partial medial epicondylectomy for cubital tunnel syndrome.

UNLABELLED: Partial medial epicondylectomy aims to eliminate potential drawbacks of total epicondylectomy for treatment of cubital tunnel syndrome. In this series, we retrospectively evaluated 80 patients (80 elbows) who had partial medial epicondylectomies for established cubital tunnel syndrome. Our main purpose was to compare clinical outcomes among partial, minimal, and total epicondylectomies. Specific attention was given to the functional outcome in severely impaired patients, and potential postoperative complications of total epicondylectomy, such as elbow instability, and medial elbow

Influential factors of surgical decompression for ulnar nerve neuropathy in Guyon's canal.

BACKGROUND: Guyon's canal syndrome is nerve compressive pathology which can lead to sensory and/or motor function deficits. This problem is usually difficult to distinguish from cubital tunnel syndrome and relatively less common than cubital tunnel syndrome. This study evaluated the functional results and patient-reported outcomes following decompression of the ulnar nerve in Guyon's canal. METHODS: Patients who were diagnosed with Guyon's canal syndrome confirmed by electrodiagnostic studies and underwent nerve decompression surgery were included in this study. The functional improvement by

Intermuscular aponeuroses between the flexor muscles of the forearm and their relationships with the ulnar nerve.

INTRODUCTION: The aim of this study was to clarify the morphological characteristics of the intermuscular aponeurosis between the flexor carpi ulnaris (FCU) and flexor digitorum superficialis (FDS; IMAS), and that between the FCU and flexor digitorum profundus (FDP; IMAP), and their topographic relationships with the ulnar nerve. MATERIALS AND METHODS: Fifty limbs of 38 adult cadavers were studied. RESULTS: The IMAS extended along the deep surface of the FCU adjoining the FDS, having the appearance of a ladder, giving off "steps" that decreased in width from superficial to deep around the mi

Ulna Nerve Decompression at the Elbow in Patients with Normal Nerve Conduction Tests.

BACKGROUND: Ulna nerve compression at the elbow is the second most common neuropathy of the upper limb. It has been suggested that nerve conduction tests are required to correctly make the diagnosis. The aim of this study was to assess whether patients with normal nerve conduction testing benefitted from surgical release of the ulna nerve. METHODS: 56 patients with symptoms of ulna nerve compression at the elbow were evaluated prospectively. All patients underwent electrophysiology testing followed by ulna nerve decompression irrespective of the results of the electrophysiology testing. Funct

Use of ultrasonography in ulnar nerve entrapment surgery--a prospective study.

The purpose of our study is to assess the usefulness of high-resolution ultrasonography in observing the morphology and dynamics of the ulnar nerve in the cubital tunnel and also the efficacy of ultrasonography in a more accurate diagnosis and appropriate surgical treatment decision. Cross-sectional area of the ulnar nerves of 40 healthy volunteers in the control group were measured bilaterally at the level of the epicondyle, 2 cm proximal to and 2 cm distal to the epicondyle. Measurements were obtained for elbows both in extension and flexion. Then, we prospectively obtained the cross-section
