Novel therapeutic approaches to xeroderma pigmentosum.

BACKGROUND: The study of xeroderma pigmentosum has yielded unforeseen advances regarding how defects in the nucleotide excision repair pathway result in this devastating disease, but development of therapeutic strategies has trailed behind the mechanistic discoveries. OBJECTIVES: This review aims to cover clinical presentation, molecular mechanisms and current management, and highlights more recent insights into targeting the deficiencies secondary to the DNA repair defects to prevent skin cancer and/or neurological degeneration. METHODS: This review article discusses novel therapeutic appro

Juvenile Pemphigus Foliaceus in a Patient With Psoriasis Receiving Narrow-Band Ultraviolet-B: Successful Treatment With Rituximab.

Pemphigus foliaceus is an autoimmune blistering disease of the skin that is not frequently associated with mucous membrane involvement. It is characterized by immunoglobulin G (IgG) antibodies against desmoglein-1, a component of epidermal intercellular adhesion, in the granular layer of the epidermis. Pemphigus foliaceus consists of scattered, arcuate, crusted erythematous lesions often in a seborrheic distribution that may progress to diffuse skin involvement and exfoliative erythroderma. Several cases in the literature discuss pemphigus foliaceus arising in patients with pre-existing psoria

Modified technique of autologous noncultured epidermal cell transplantation for repigmenting vitiligo: a pilot study.

BACKGROUND: Several reports have demonstrated that grafting of autologous melanocytes from normally pigmented donor skin can be used for repigmentation of achromic macules in vitiligo. OBJECTIVE: To investigate a modified approach in which noncultured autologous melanocytes and keratinocytes are grafted on superficially laser dermabraded vitiligo lesions in a suspension enriched with hyaluronic acid. METHODS: Four patients with stable vitiligo were treated using a noncultured melanocyte-keratinocyte suspension. The cellular suspension was grafted on vitiliginous lesions previously dermabrade

The Comparison Between Conventional Photodynamic Therapy and "Painless"TM Photodynamic Therapy in the Treatment of Actinic Keratosis: A Mini Review.

Actinic keratosis is a prevalent skin condition associated with prolonged exposure to ultraviolet radiation and serves as a precursor to squamous cell carcinoma. This condition arises from DNA mutations and abnormal cell growth triggered by ultraviolet rays. While uncertainties persist regarding the progression rate of actinic keratosis (AK) to squamous cell carcinoma, the significance of nonmelanoma skin cancers underscores the need for safe, efficient, cost-effective, and accessible treatment options. Factors such as number of lesions, skin type, and sun exposure increase the risk of progres

Efficacy and Safety of Laser Therapy and Phototherapy in Cicatricial and NonCicatricial Alopecia: A Systematic Review Study.

BACKGROUND AND AIMS: In recent years, the application of various light and laser devices in the treatment of different types of alopecia has been established. This systematic review aims to assess the efficacy and safety of laser therapy and phototherapy in cicatricial and non-cicatricial alopecia. METHODS: A comprehensive search was conducted on PubMed, Scopus, Science Direct, and Google Scholar. Articles were evaluated across four subgroups: alopecia areata, androgenic alopecia, telogen effluvium, and cicatricial alopecia. Included studies were published in English or Persian between Januar

Generalized lichen nitidus.

We report a 38-year-old man who presented with a generalized papular eruption that was clinically and histologically consistent with lichen nitidus. This patient's condition had been persistent for approximately 1 year; however, soon after assuming employment that entailed significant, regular sun exposure, the patient noted marked clearing of his lesions in sun-exposed areas. This case corroborates previous reports that suggest that generalized lichen nitidus can be successfully managed with ultraviolet light therapy.

Treatment of lichen planus.

Lichen planus is an inflammatory pruritic dermatosis in which T lymphocytes attack the basal epidermis, producing characteristic clinical and histological lesions. Mild cases can be treated successfully with rest, topical corticosteroids with or without wet dressings, or occlusion. Severe generalised cases may require systemic corticosteroids. Chronic, aggressive lesions may need additional therapy ranging from psoralen plus ultraviolet A (PUVA) to retinoids or cyclosporin. Complications ranging from infectious, bullous disease or ulceration require special treatment considerations.

Photochemotherapy for mycosis fungoides: current status.

Phototherapy for mycosis fungoides is reviewed with particular emphasis on PUVA (psoralen ultraviolet A-range) therapy and its combination with other topical and systemic treatments. PUVA therapy has the advantages of being available in all major centres, is simple to administer, relatively inexpensive and has relatively low toxicity. Medium-term results suggest that PUVA is as effective as any of the other topical therapies for stage I disease. Its combination with alpha-interferon produces significant benefit in stage II or more advanced disease. Long-term non-melanoma skin cancer is increas

Malignant melanoma in situ in two patients treated with psoralens and ultraviolet A.

Two patients are reported who were treated with 8-methoxypsoralen and ultraviolet A (PUVA) for psoriasis and developed cutaneous lesions of malignant melanoma in situ (atypical melanocytic hyperplasia). One patient received 324.5 joules/cm2 of UVA. Seven months after discontinuing therapy, he developed a superficial spreading melanoma in situ in association with an intradermal nevus on the left posterior thoracic area. The second patient received 2,802 joules/cm of UVA. While on PUVA therapy she developed an in situ lentigo melanoma on her lower lip. To our knowledge only one other psoriatic p

Lymphomatoid papulosis associated with recurrent cutaneous T-cell lymphoma.

INTRODUCTION: Lymphomatoid papulosis is a chronic benign disease which may be associated with malignant lymphomas. This case illustrates the relapsing and remitting nature of both lymphomatoid papulosis and its potential of developing cutaneous T-cell lymphoma and narrow-band ultraviolet B (NB-UVB) phototherapy as a new modality of treatment of early-stage mycosis fungoides in these patients. CLINICAL PICTURE: A 44-year-old woman has had recurrent crops of papules and nodules of lymphomatoid papulosis on the limbs for 15 years. Histological features are consistent with the type B lesions of l

Combination of psoralens and ultraviolet A and ultraviolet B in the treatment of psoriasis vulgaris: a bilateral comparison study.

Forty-two adults with psoriasis vulgaris were treated total body thrice weekly with both ultraviolet B (UVB) and psoralens and ultraviolet A (PUVA) with the use of protocols shown to be effective with either treatment alone. Patients previously difficult to manage with either UVB or PUVA and at least 25% psoriasis were selected. No other treatment was used for 2 months prior to initiation or during the study. All patients tolerated treatments well and cleared in an average of only 11.3 treatments, less than half the number of treatments usually required with either PUVA or UVB. The mean UVB do

Pilot phase results of a prospective, randomized controlled trial of narrowband ultraviolet B phototherapy in hospitalized COVID-19 patients.

COVID-19 morbidity and mortality are driven by poor immune regulation. Narrowband ultraviolet B (NB-UVB) phototherapy is standard of care in a number of immune-dysregulated diseases. To assess the efficacy of NB-UVB phototherapy for improving COVID-19 outcomes in high-risk, hospitalized, we developed the Adaptive Photo-Protection Trial. This is a multi-center, prospective, double-blinded, randomized, placebo-controlled trial. The pilot phase results are reported here. Consecutive patients admitted with a positive COVID-19 PCR were screened for eligibility. Enrolled subjects were computer rando

Efficacy and safety of tretinoin 0.05% cream to prevent hyperpigmentation during narrowband UV-B phototherapy in patients with facial vitiligo: a randomized clinical trial.

BACKGROUND: Narrowband UV-B (NBUVB) phototherapy is the mainstay of vitiligo treatment, but hyperpigmentation is one of the limitations. Meanwhile, topical tretinoin is effective against pigmentary disorders. OBJECTIVE: To determine whether tretinoin 0.05% cream would prevent hyperpigmentation when patients with facial vitiligo underwent phototherapy. METHODS: A randomized, controlled, split-face trial was conducted. Adult patients with stable, non-segmental facial vitiligo were enrolled. The left/right sides of the face were randomly allocated to receive either topical tretinoin 0.05% cream

[Phototherapy for psoriasis; clearing lesions and maintaining lesion-free status].

Since time immemorial, sunlight has been used to treat a wide variety of skin afflictions. Consequently, probably on the basis of patients' experience and consequent experimentation with lamps, phototherapy has become an important dermatological treatment, particularly for psoriasis. The active component in sunlight proved to be ultraviolet (UV) radiation. Optimized UV lamps (type TL01) are now routinely used for clearing psoriatic lesions, after which therapy is stopped due to potential carcinogenic effects. The lesions reappear after a few months. Daily home treatment with low-dose UV radiat

Erythrodermic pityriasis rubra pilaris treatment: Two case reports and literature review.

Pityriasis rubra pilaris (PRP) is a rare inflammatory dermatosis characterized by hyperkeratotic follicular papules and erythematous-desquamative plaques that tend to progressively evolve into erythroderma. Treatment is challenging given that international guidelines are not available and large-scale trials do not exist. Traditionally, many topical and systemic drugs had been used as consolidated agents; recently, biologicals are gaining increasing importance, promisingly dominating the therapeutic scenario ahead. Herein, we present a case series showing the "past" and the "future" therapeutic
