Acute urinary retention due to pseudoephedrine hydrochloride in a 3-year-old child.

Pseudoephedrine hydrochloride (PEH) is a sympathomimetic agent that is widely used in common cold disease in children. Though side effects of PEH are well known, it is preferred by many pediatricians in order to benefit from its symptomatic relief in common cold disease. A case of acute urinary retention due to PEH in a three-year-old boy is reported. The aim of this case report is to emphasize the clinical importance and differential diagnosis of PEH overdose in children and to discuss the appropriate treatment approach to PEH overdose in the emergency department.

Urinary retention during sacral nerve stimulation for faecal incontinence: report of a case.

Sacral nerve stimulation (SNS) was proposed for the treatment of patients with urologic symptoms in 1967 but was not used until 1981. SNS has also proven to be a promising treatment in idiopathic faecal incontinence when conventional treatments have failed. The modality has been used for faecal incontinence since the mid-1990s. Eighty percent of the patients who were selected for percutaneous nerve evaluation (PNE) because of faecal incontinence report an improvement in the symptoms and qualify for a permanent implantation. Accordingly, SNS is now used for faecal incontinence and urologic symp

Efficacy of sacral nerve stimulation for urinary retention: results 18 months after implantation.

PURPOSE: We investigate the efficacy of sacral neurostimulation in patients with idiopathic urinary retention in a prospective, randomized multicenter trial. MATERIALS AND METHODS: A total of 177 patients with urinary retention refractory to standard therapy were enrolled in the study. Greater than 50% improvement in baseline voiding symptoms during a 3 to 7-day percutaneous test stimulation qualified a patient for surgical implantation of an InterStim parallel system. Of the patients who qualified for implantation 37 were randomly assigned to a treatment and 31 to a control group. Patients i…

Persistent postpartum urinary retention in contemporary obstetric practice. Definition, prevalence and clinical implications.

OBJECTIVE: To prospectively evaluate the prevalence, presumed etiologies and clinical implications of persistent postpartum urinary retention in modern obstetric practice. STUDY DESIGN: The study population comprised 8,402 consecutive, unselected parturients delivered in a university-affiliated maternity hospital over a one-year period. If a woman was unable to void spontaneously until the third postpartum day despite intermittent use of a Foley catheter, a diagnosis of persistent postpartum urinary retention was established. Patients were treated by insertion of a Foley catheter for up to tw…

Prospective Observational Study to Evaluate Post-Operative Urinary Retention With Pediatric Epidural Analgesia.

BACKGROUND AND OBJECTIVES: Epidural analgesia is an effective analgesic method for children throughout the postoperative phase. The post-operative urinary retention affects the post-operative recovery immensely. Many studies look at post-operative urinary retention in adults who receive epidural analgesia, while there is a paucity of data in children. This is a study that addresses the question of post-operative urinary retention in children in whom epidural analgesia is employed. METHODOLOGY: This is a prospective study undertaken in the pediatric surgery department of Christian Medical Coll…

Early Prediction of Postoperative Urinary Retention After General Anesthesia Using Explainable Machine Learning.

PURPOSE: Postoperative urinary retention (POUR) is a common and clinically important perioperative complication associated with pharmacological effects and autonomic dysfunction, which may lead to increased patient discomfort and delayed recovery. Its multifactorial etiology makes early clinical identification challenging. This study investigated the feasibility of using machine learning to predict early POUR after general anesthesia using readily available, non-invasive perioperative data. Model interpretation techniques were further applied to improve transparency and identify key clinical f…

Electrical Pudendal Nerve Stimulation as an Initial Neuromodulation Strategy for Neurogenic Urinary Retention.

BACKGROUND AND OBJECTIVE: Chronic neurogenic urinary retention often requires assisted emptying and remains challenging to treat. We evaluated whether electrical pudendal nerve stimulation (EPNS) improves bladder emptying and compared short-term outcomes with a standard sacral neuromodulation (SNM) test stimulation strategy. METHODS: In this prospective, multicenter, assessor-masked study, 70 adults with chronic refractory neurogenic nonobstructive urinary retention received either EPNS (12 sessions over 4 wk) or a standard 4-wk continuous SNM test phase using a percutaneously placed S3 lead.…

Development of post-operative urinary retention (POUR) in pediatric patients undergoing orthopedic surgery.

BACKGROUND: Pediatric patients who are unable to void after orthopedic surgery can develop post-operative urinary retention (POUR). The aims of this study were to identify variables that contributed to POUR by comparing characteristics of children who did and did not require urinary intervention and to identify which variables predict patients who are at risk for POUR. METHODS: This retrospective chart review evaluated 73 pediatric patients who developed POUR after an orthopedic procedure and matched their characteristics to 73 pediatric patients who did not develop POUR. Multiple levels of a…

The Hidden Threat: Chronic Urinary Retention and Thromboembolism in Underlying May-Thurner Syndrome.

May-Thurner syndrome (MTS) is characterized by the formation of an intravenous scar or venous "spur" resulting from chronic pulsatile compression of the left common iliac vein (LCIV) by the right common iliac artery (RCIA) against the fourth or fifth lumbar vertebral body. This pulsatile compression creates a flow-limiting stenosis of the LCIV, leading to increased intravenous pressure in the veins draining the left lower extremity (LLE). Consequently, this elevated venous pressure manifests as a spectrum of chronic symptoms including unilateral LLE edema, pain, tenderness, warmth, skin inflam

Urinary Retention Following Methamphetamine and Cannabis Abuse in a 33-Year-Old Male.

Urinary retention is described as an inability to voluntarily empty the bladder, with potential etiologies including mechanical obstruction and neurologic dysfunction. Abused substances like methamphetamine and cannabis can induce this dysfunction. We report a case about a patient with no prior psychiatric history with concomitant methamphetamine and cannabis use, presenting with an acute delirious state and urinary retention. Due to the multifactorial nature and acuity of a patient's presentation, clinicians should be aware of the potential for substance abuse to impact bladder function and c…

Fluoxetine Related Urinary Retention in a 15-Year-Old Girl: a Case Report.

Fluoxetine is an effective and safe agent frequently used in the treatment of childhood depression and obsessive-compulsive disorder. Urinary retention is defined as the inability to empty the bladder completely. Drug use is one of the most important factors in the etiology of urinary retention. Urinary retention is an uncommon side-effect of fluoxetine use. We report a case of chronic urinary retention during fluoxetine monotherapy in a 15-year-old girl, resolving following the discontinuation of treatment.
