Azathioprine for glucocorticoid resistant noninfectious uveitis.

BACKGROUND: the systemic steroids represent the first line treatment in the majority of the noninfectious uveitis, except some specific indications as the Behcet disease. Nevertheless, this treatment may be ineffective and immunosuppressive therapy is mandatory. PURPOSE: to evaluate effectiveness and side effects of azathioprine (AZA) in corticosteroid resistant noninfectious uveitis (CRNIU). METHODS: This prospective study (2002- 2009), concerned 21 patients (mean age 37 years), 37 eyes, with CRNIU. Patients received oral AZA 2,5mg/kg/day, initiated in association with high dose steroids, w…

New infectious etiologies for posterior uveitis.

Emergent and resurgent arthropod vector-borne diseases are major causes of systemic morbidity and death and expanding worldwide. Among them, viral and bacterial agents including West Nile virus, Dengue fever, Chikungunya, Rift Valley fever, and rickettsioses have been recently associated with an array of ocular manifestations. These include anterior uveitis, retinitis, chorioretinitis, retinal vasculitis and optic nerve involvement. Proper clinical diagnosis of any of these infectious diseases is based on epidemiological data, history, systemic symptoms and signs, and the pattern of ocular inv…

Ocular signs predictive of tubercular uveitis.

PURPOSE: To determine ocular signs predictive of tubercular uveitis. DESIGN: Retrospective, nonrandomized, comparative interventional case study. METHODS: Three hundred eighty-six patients with active uveitis were treated at a tertiary care single-center uveitis practice. Uveitis was presumed to be tubercular in patients who showed evidence of latent or manifest tuberculosis without any other known cause and who did not show recurrence of uveitis after 12 months of antitubercular therapy. One hundred eighty-two patients who thus obtained clinical diagnoses of presumed tubercular uveitis were…

[Non-infectious systemic diseases and uveitis].

Uveitis can be defined as any inflammation affecting the uveal tract, although in clinical practice this term includes any intraocular inflammatory event. The etiology of this inflammation can be related to an endogenous mechanism in the clinical course of a systemic disease (sarcoidosis, Behçet's disease, multiple sclerosis, Vogt-Koyanagi-Harada disease, etc.), or an isolated ocular entity. Sometimes, ocular inflammation is the initial manifestation of an undiagnosed systemic disease. On the other hand, ocular involvement could be the main cause of morbidity of the disease, and early diagnosi…

[Systemic management of autoimmune anterior uveitis].

Autoimmune uveitis is an acute, recurrent, sight-threatening disease that can lead to severe visual loss and blindness. It requires systemic immunosuppressive therapy and continuous monitoring by ophthalmologist and rheumatologist. There are no universally accepted referral patterns for the treatment of endogenous uveitis. Most specialists indicate corticosteroids, methotrexate and cyclosporine A as the first use drugs. Anti-cytokine drugs are a new opportunity for the patients unresponsive to the conventional anti-inflammatory and immunosuppressive therapy.

Recombination in uveitis-causing enterovirus strains.

The complete nucleotide sequences of three human echovirus (EV) 11 strains and one EV19 strain, all of which caused outbreaks of enterovirus uveitis (EU), a new infant disease first identified in 1980 in Siberia, were determined. One EV11 strain which caused an outbreak of sepsis-like disease in Hungary was also sequenced. All four EV11 strains were mosaic recombinants of the prototype EV11 strain Gregory, with their non-structural coding regions and 5' NTRs being more similar to other prototype enteroviruses (EV1, EV9). However, this finding is probably a feature of all circulating enteroviru…

Macrophage migration inhibitory factor in uveitis.

To summarize, we found that high levels of MIF were detected in the serum of uveitis patients with Behçet's disease, VKH disease, and sarcoidosis, as well as idiopathic iridocyclitis. In Behçet's disease and sarcoidosis, the MIF levels were higher in the active uveitis stage than in the inactive stage. Serum MIF increased after corticosteroid therapy in VKH disease. A high aqueous humor MIF concentration was detected at the onset of EAU. Finally, anti-MIF antibody suppressed EAU when administered in the early phase. MIF is thought to be an important cytokine in the pathogenesis of uveitis.

Uveitis and central nervous system vasculitis.

Vasculitis confined to the central nervous system (CNS) is a rare disease usually characterized by headache and focal neurologic symptoms. Patients with primary vasculitis of the CNS may have symptoms and laboratory findings of systemic disease such as fatigue and elevated erythrocyte sedimentation rate, but by definition, focal inflammation should not be present outside the CNS. We describe 3 patients with uveitis in association with this diagnosis. The recognition of this association adds to the complex differential diagnosis of uveitis in association with CNS disease, and indicates that "is…

[Immunological aspects of tuberculosis uveitis pathogenesis].

Based on clinical and immunological examinations of 251 patients with tuberculous uveitis (TU), 3 types of its course (productive, exudative, and mixed) were identified. The immunopathologic pattern of each type was found to be different: the productive type was characterized by delayed hypersensitivity, the exudative one had immediate hypersensitivity, and the mixed one had both. The immunogenetic markers of mixed TU were defined. These included HLA antigens, such as A2, B7, B21, and B27.

Systemic diseases associated with intermediate uveitis.

BACKGROUND: Intermediate uveitis is characterized by vitreal inflammation with associated inflammation of the vitreous base and peripheral anterior retina and choroid. It may be found as an isolated and idiopathic condition or in association with systemic disorders such as multiple sclerosis and sarcoidosis. OBJECTIVE: To identify the clinical features of intermediate uveitis and assess its association with systemic diseases. METHODS: Retrospective study of 83 patients presenting with intermediate uveitis between 1970 and 1991. RESULTS: Evidence of systemic disorders was found in 26 of 83 p…

[Uveitis, caused by Echo-11 virus].

Analysis of clinical manifestations of a total enteroviral infection with ECHO-11 virus-induced uveitis in 58 infants aged 2 weeks to 1.5 years has shown that uveitis develops on days 1-10 of the disease and is associated with a weak injection, endothelial edema, hyperemia and edema of the iris rapidly eventuating in destruction of the pigment lamina and fenestration. Posterior synechiae, pupil deformations, grave uveitis with hypotonia of 4-10 mm Hg are rapidly developing. A two-year follow-up has shown marked hemo- and hydrodynamic disorders, retarded growth of the involved eye, development …

Ophthalmological significance of HLA associated uveitis.

The association between HLA-A29 and birdshot chorioretinopathy is the strongest association between HLA and disease ever described. The determination of HLA-A29 is therefore of diagnostic significance. The association between HLA-B27 and acute anterior uveitis (AAU) is much weaker. However, it is evident that B27+ AAU is clinically different from B27-AAU. Half of the B27+AAU patients have or will have ankylosing spondylitis or Reiter's syndrome. The determination of HLA-B27 is therefore of clinical significance and should be determined in each case of AAU. The B27+ patients should be referred …

Anti-neutrophil cytoplasmic antibodies in uveitis.

The antineutrophil cytoplasmic antibody test has become available over the past 4 or 5 years. It is positive in a limited number of conditions whose characteristic feature is the presence of a small vessel vasculitis. These include Wegener's granulomatosis and microscopic polyarteritis. The test was applied to 63 patients with uveitis of various types. 14 positive tests were found amongst which anterior uveitis was the most common disease present.
