Physical Activity Discussions as the First Step Towards Movement in Non-Curative Cancer Care: A Qualitative Study.

BACKGROUND: Due to advances in technology, non-curative cancer patients are living longer with the burden of the disease. Physical activity (PA) has value as a supportive service for individuals at any point on the cancer care continuum, including advanced cancer. OBJECTIVES: The objective of this study was to understand how oncology care providers (OCPs), family/friend caregivers, and health system decision makers can include conversations of PA into care for those living with non-curative cancer. METHODS: We conducted 24 semi-structured interviews guided by the Capability, Opportunity, Mot

Cardiologists' perceptions on multidisciplinary collaboration in heart failure care - a qualitative study.

BACKGROUND: Cardiologists play a key role in multidisciplinary care by guiding heart failure (HF) management in the hospital and in the community. Regional implementation of multidisciplinary health care interventions depends on how they perceive collaboration with other health care disciplines, yet research on this topic is limited. This study aimed to explore the views and opinions of cardiologists on multidisciplinary collaboration in HF care. METHODS: We conducted a qualitative study based on face-to-face semi-structured interviews with 11 Belgian cardiologists between September 2019 and

Regulatory and clinical trial exclusion of pregnant women: economic implications for preventive interventions in low-income countries.

INTRODUCTION: Preventive interventions during pregnancy represent a high-return investment, and some of the most cost-effective strategies because they help prevent diseases, reducing the cost for hospital treatment, and neonatal intensive care treatments, complications, and long-term health problems for both mother and child. We searched PubMed, Google, ClinicalTrials.gov, the International Clinical Trials Registry Platform, and Ministry of Health websites from low-income countries (LICs) to identify clinical trials and regulatory documents governing pregnancy-related research. AREAS COVERED

Biophilic design, neuroarchitecture and therapeutic home environments: harnessing medicinal properties of intentionally-designed spaces to enhance digital health outcomes.

Digital health technologies (DHT) support patient-centered care by delivering behavioral, educational, self-efficacy and self-management interventions. Yet, multifactorial chronic diseases are shaped by complex interactions between genetics, environment and behavior, embodied in social and commercial determinants of health. Given that people in the United States spend on average 18 h per day at home, the impact of home environment on a person's health is underutilized in medicine. Herein, we discuss opportunities to improve therapy outcomes through bridging digital interventions with intention

Changes in hand function and health state utility after cubital tunnel release using the United Kingdom Hand Registry.

This study aimed to analyse and contrast changes in health-related quality of life (HR-QoL) and hand symptoms in the first 6 months after surgical treatment for primary cubital tunnel syndrome. Data originated from the United Kingdom Hand Registry. HR-QoL was assessed using the generic EuroQol five-dimensional assessment tool (EQ-5D-5L) and hand symptoms using the Patient Evaluation Measure (PEM). In total, 281 patients were included in the statistical analysis. Cubital tunnel release resulted in clinically relevant relief of hand symptoms. However, no improvement in HR-QoL was detected by the

Normative values for grip strength, gait speed, timed up and go, single leg balance, and chair rise derived from the Canadian longitudinal study on ageing.

BACKGROUND: decreased muscle strength and physical function often precede disability, nursing home admission, home care use and mortality in older adults. Normative values for commonly used physical performance-based tests are not widely available for older adults but are required for clinicians and researchers to easily identify individuals with low performance. OBJECTIVE: to develop normative values for grip strength, gait speed, timed up and go, single-leg balance and five-repetition chair rise tests in a large population-based sample of Canadians aged 45-85 years. METHODS: baseline data

Pragmatic trial of a virtual dementia collaborative care management program: protocol for the Aging Brain Care Virtual (ABCV) program.

INTRODUCTION: Providing care management, treatment and support to patients with Alzheimer's Disease and Related Dementias (ADRD) is a difficult task for health systems. Over the past 20 years, interventions designed to improve outcomes for patients living in the community with dementia and their care partners have moved progressively, but separately, from large scale trials and pragmatic models of collaborative care. Given the projected increase in the number of people living with dementia coupled with the realignment of payment for services to be value-based and provided in the community, sys

An Initial Typology to Compare Continuum of Care Policies, Structures, and Processes in Two Canadian Jurisdictions.

Continuums of care (e.g., home-based services, assisted living, nursing homes) deliver essential health and social services for older people and are organized differently across healthcare settings. Informed by academic research and consensus-building sessions with policymakers and older people from two Canadian provinces, this article presents a typology of 12 dimensions that define how care continuums are organized and operationalized. Dimensions define the care provided by different services, including how they are accessed and their recipients; paid staffing, illustrating provider type, vo

Enhancing Patient Care and Efficiency in Gastroenterology Nursing Through Standardized Discharge Instructions.

This quality improvement initiative at Hartford HealthCare Endoscopy Unit was designed to enhance both patient outcomes and operational efficiency by implementing standardized, procedure-specific discharge instructions. Standardized discharge instructions reduce complications, increase patient satisfaction, and decrease readmissions. The goal was to improve patient satisfaction metrics (Press Ganey overall satisfaction and instruction regarding recovery). To address inconsistencies in postprocedure care, the team employed the Plan-Do-Study-Act model to develop and implement structured discharg

Care coordination and telemedicine improves glycaemic control in ethnically diverse veterans with diabetes.

We conducted a pilot study of a care-coordination programme involving daily monitoring and education of elderly diabetic veterans from different racial/ethnic groups. A telephone-based, in-home messaging device was used for patient monitoring and education. Sixty-nine patients were enrolled in the study and HbA(1c) values were obtained both before and after the telemedicine intervention in 41 of them. The mean HbA(1c) before enrolment was 7.6% and the mean value 9 months later was 7.3% (P = 0.09). The greatest fall in HbA(1c) occurred in African-Americans (0.65%, P = 0.05). The total number of

"Gleaning a lot from the history and physical exam," and "reasonably confident without imaging": a qualitative study of primary care clinicians' management of patients with low back pain.

BACKGROUND: Routine seeking of imaging for patients with low back pain is not concordant with the evidence-based recommendation that imaging is rarely of diagnostic value. Inappropriate imaging is a waste of resources and can lead to undesirable downstream effects for individuals and health systems. To develop effective strategies to reduce unwarranted referrals for imaging in primary care, we must understand the drivers for, and barriers to, guideline-adherent practice. We explored clinicians' views to identify the dominant influences on clinicians as they choose to pursue, or avoid, imaging

Team-Based Hypertension Management in Primary Care: A Synthesis of Systematic Reviews.

BACKGROUND: People with hypertension often require personalised education and support from health professionals to optimise outcomes. Multidisciplinary team-based care in primary care has been proposed as a strategy to empower patients to manage their blood pressure. Although substantial literature reports team-based interventions for hypertension management in primary care, the optimal models and their components remain unclear. AIM: This scoping review critically synthesised existing systematic reviews that have explored elements of team-based care and collaboration among primary care clini

Accelerating Health Data Sharing: A Solution Based on the Internet of Things and Distributed Ledger Technologies.

BACKGROUND: Huge amounts of health-related data are generated every moment with the rapid development of Internet of Things (IoT) and wearable technologies. These big health data contain great value and can bring benefit to all stakeholders in the health care ecosystem. Currently, most of these data are siloed and fragmented in different health care systems or public and private databases. It prevents the fulfillment of intelligent health care inspired by these big data. Security and privacy concerns and the lack of ensured authenticity trails of data bring even more obstacles to health data s
