Ventricular fibrillation induced by short-coupled premature ventricular complex is an uncommon cause of cardiac arrest in young individuals with no structural heart disease. The genetic substrate of this condition is heterogeneous and remains incompletely defined. We describe a case of short-coupled premature ventricular complex-triggered ventricular fibrillation with a likely pathogenic variant in the titin (TTN) gene. (Level of Difficulty: Advanced.).

Treatment of a patient with new onset atrial fibrillation with intravenous adenosine was followed by transient but significant acceleration of the ventricular rate. This unexpected response to adenosine can be explained by a predominating increase in sympathetic discharge relative to the drug's direct atrial ventricular nodal-blocking action.

Theophylline increases sinus rate, but as yet its use has not been investigated in patients with chronic atrioventricular conduction disturbances. We report a rare case of intraoperative ventricular fibrillation (VF) induced by electrocautery conduction to the pericardium during lung cancer surgery. A 59-year-old man underwent thoracoscopic left lower lobectomy. During dissection of the pulmonary ligament using an electric scalpel in coagulation mode( 30 W), the tip contacted the pericardium, resulting in VF.
