Acute cardiac arrhythmias are among the most common and prognostically relevant emergency situations in clinical cardiac electrophysiology. They range from bradycardia and supraventricular tachycardia to atrial fibrillation and atrial flutter to potentially life-threatening ventricular tachyarrhythmias and electrical storm. In these situations, rapid, structured and guideline-based pharmacotherapy is essential to prevent hemodynamic instability and mortality. The selection of appropriate emergency medication depends primarily on the underlying arrhythmia, the hemodynamic situation and individua

Recurring questions when dealing with arrhythmias in athletes are about the cause of the arrhythmia and, more importantly, about the eligibility of the athlete to continue sports activities. In essence, the relation between sports and arrhythmias can be understood along three lines: sports as arrhythmia trigger on top of an underlying problem, sports as arrhythmic substrate promotor, or sports as substrate inducer. Often, there is no sharp divider line between these entities. The athlete's heart, a heart that adapts so magically to cope with the demands of exercise, harbours many structural an

In 1889, in an age preceding the invention of the electrocardiogram, a physician working in the hospitals of Lyon published a remarkable paper on essential paroxysmal tachycardia, of which Bristowe in England and Huppert in Germany had already spoken. The study described 12 cases (including 3 personal cases) of essential paroxysmal tachycardia and 6 secondary tachycardias. They were not all benign, there being 4 deaths out of the 12 cases. From this period on, the term "Bouveret's tachycardia" has been used in France and, with the advances in rhythmology, some have assimilated it to paroxysmal

The efficacy and safety of flecainide were studied in the maintenance of sinus rhythm after electrical cardioversion for chronic atrial fibrillation or atrial flutter. Eighty-one patients were randomized to flecainide treatment or no treatment. Baseline characteristics of both groups were comparable. Compared to previous studies, patients could be classified as difficult-to-treat patients. Multiple regression analysis showed New York Heart Association class I for exercise tolerance (p = 0.0004) and flecainide treatment (p = 0.01) to be the main factors increasing the arrhythmia-free episode. The patterns of valve motion were analyzed in 14 patients with combined aortic and mitral valve prolapse by two-dimensional echocardiography. Four patients have also tricuspid valve prolapse. In one we could find evidence of a myxomatous pulmonic valve with prolapse of the posterior cusp. By two-dimensional echocardiography all patients had redundant aortic leaflets bulging into left ventricular outflow tract during diastole. M-mode examination of the aorta was normal in five patients; eccentricity of the diastolic aortic valve echo was found in two patients; seven showed multiple echoes from

The results of treating 95 patients suffering from various disorders of cardiac rhythm with ethmosin are discussed. Sixty-eight patients received oral ethmosin in a daily dose of 600 mg, 12 of them were treated for a long time (85 +/- 39 days on the average), the others for no more than 3--7 days. Fifty patients received a daily dose of 150 mg ethmosin in a single intravenous injection. The therapeutic effect was considered positive if the number of extrasystoles reduced by no less than 50% with complete removal of group extrasystoles and paroxysms of tachycardia according to the data of daily

BACKGROUND: Automatic mode switching is defined as the ability of a pacemaker to reprogram itself from tracking to non-tracking mode in response to atrial tachyarrhythmias, and to regain tracking mode as soon as the tachyarrhythmia terminates. In contrast to upper rate behavior, mode switching does not only limit atrial tracking at a certain rate but actively drives the ventricular pacing rate back to lower rate or sensor rate as long as the atrial tachyarrhythmia persists. In contrast to DDD with mode switch, AV synchrony may be lost in DDIR mode if the sinus rate exceeds the sensor rate. DDD

Compressive sensing (CS) is studied for the acquisition of different QRS complexes in ECG signals. By applying sparse binary measurement matrix and by applying regularization for promoting temporal correlation, performance of CS techniques for the reconstruction of ECG segments containing different QRS complexes, such as, normal QRS, paced QRS, right bundle branch block beat, left bundle branch block beat, and ventricular flutter beat, has been studied. Simulation results demonstrate that performance of CS differs with the types of beats, and ECG segments with well-known beats can be reconstru…

A biatrial pacemaker was inserted in a 68-year-old female with paroxysmal atrial fibrillation and atrial flutter refractory to antiarrhythmic drugs based on a sick-sinus-syndrome and concomitant interatrial conduction delay. One electrode positioned in the right atrium and another electrode located in the coronary sinus were connected to a dual-chamber pacemaker. The electrode in the right atrium was connected to the atrial channel, the electrode in the coronary sinus to the ventricular channel. The pacemaker was programmed in DDD-mode with an AV-delay of 30 ms. Under a chronic antiarrhythmic

BACKGROUND: The incidence and outcomes of early cardiac complications in patients with intracerebral hemorrhage (ICH) are poorly understood. These cardiac complications may be part of the so-called stroke-heart syndrome in patients with ICH. We investigated this issue in an individual patient data pooled analysis from an international repository of clinical trial data.
METHODS: We used the Virtual International Stroke Trials Archive to investigate the incidence of cardiac complications within 30 days post-ICH or acute ischemic stroke (AIS). These complications included acute coronary syndrome

During the acute phase of myocardial infarction, two groups of patients are observed. Patients in the first group have no significant complications, and approximately 95 per cent of these patients recover fully without any specific therapy. Patients in the second group may have various complications, some of which are benign, whereas others may lead to a fatal outcome. The complications may be divided into four major types: 1. Cardiac arrhythmias and conduction defects. The tachyarrhythmias and bradyarrhythmias are the most frequently encountered complications in patients with acute myocardial

Patients with diabetes mellitus (DM) often present other chronic comorbidities including arterial hypertension (AH), chronic kidney disease (CKD), ischemic heart disease (IHD) and heart failure with preserved ejection fraction (HFpEF). The frequent association of the latter conditions is considered part of the spectrum of cardio-renal syndromes (CRS), a group of disorders of the heart and kidneys whereby acute or chronic dysfunction in one organ may induce acute or chronic dysfunction of the other. Heart failure patients are predisposed to develop arrhythmias. Supraventricular arrhythmias can exacerbate the heart failure symptoms by decreasing the effective cardiac output and their control require pharmacological, electrical, or catheter-based intervention. In the setting of atrial flutter or atrial fibrillation, anticoagulation becomes paramount to prevent systemic or cerebral embolism. Patients with heart failure are also prone to develop ventricular arrhythmias that can present a challenge to the managing clinician. The management strategy depends on the type of arrhythmia, the underl

The use of radiofrequency energy for the treatment of supraventricular tachycardia in pediatric patients has gained widespread acceptance, especially for tachyarrhythmias associated with palpitations, dizziness, presyncope or syncope, cardiomyopathy, and cardiac arrest. Ablation of the substrate supporting atrioventricular reentry, atrioventricular node reentry, and automatic atrial tachycardia yields a 90%-98% success rate with low incidence (< 1%) of complications and adverse side-effects. Ablation of intra-atrial reentry, including atrial flutter and fibrillation, appears to be promising an

INTRODUCTION: As point-of-care ultrasound (POCUS) applications expand in the emergency department (ED), its utilization within the management of patients presenting with rapid atrial fibrillation (AF) or flutter (AFL) is unclear. The co-morbidity of heart failure and AF/AFL complicates the rate/rhythm control and disposition of this population. ED physicians diagnose heart failure by integrating POCUS left ventricular function assessment, but studies have not focused on the rapid AF/AFL population.
METHODS: Our survey aimed to explore the use, integration, and perceived barriers of POCUS left

Experience with oral sotalol, a beta-blocker with class III-antiarrhythmic properties, is limited in the pediatric population. Sotalol was administered to 32 patients with a mean age of 8.7 years (range 1 day-19.9 years). Mean dosage was 4.6 (1.5-9.4) mg/kg or 122.1 (52-306) mg/m2, respectively. In 27/32 patients, at least 1 antiarrhythmic agent had failed to control the dysrhythmia before sotalol was started. Cardiac diagnoses included normal heart (n = 16), status after correction of congenital heart disease (n = 13), and cardiomyopathy (n = 3). Success (based on symptoms and 24-h electrocar

160 survivors of acute myocardial infarction (AMI) were evaluated to assess the clinical significance of supraventricular tachyarrhythmias (SVTA) occurring at discharge from the hospital after the acute event. All the variables considered for the study were estimated before hospital discharge; arrhythmias were quantified with a 24 h Holter ECG monitoring system. SVTA occurred in 88 patients (55%). Single or repetitive supraventricular premature beats were found in 65 (41%), paroxysmal atrial or junctional tachycardias in 20 (12%), bouts of atrial flutter or fibrillation in 3 (2%). Bivariate st
