BACKGROUND: We describe a case involving technical success with internal trapping using controllable detachable coils yet antegrade recanalization of the occluded vertebral artery, in the vertebral artery dissecting aneurysm. Possible explanations for the antegrade recanalization of the occluded vertebral artery and lessons from the case are also discussed.
CASE DESCRIPTION: A 42-year-old woman with a history of stupor (Hunt and Hess grade III) and right vertebral artery dissecting aneurysm, was admitted to our hospital. Endovascular treatment was performed. The dissecting aneurysm and affect

A case of traumatic extracranial vertebral arterial dissection leading to vertebrobasilar thrombosis and respiratory compromise requiring mechanical ventilation was managed with intraarterial thrombolysis and stenting of the vertebral intimal dissection. In contrast to similar, previously reported cases, this critically ill patient made a full recovery, returning to his job as a secondary school teacher.

Background: In rare cases, isolated hypoglossal palsy may arise from dissection and/or pseudoaneurysm of either the internal carotid artery (ICA) or the vertebral artery (VA). However, the mechanism of this pathology has not been elucidated, and no high-quality randomized data exist to guide its management. Case Description: A 43-year-old man without a significant medical history presented with signs of isolated right hypoglossal palsy following a vigorous coughing episode. Imaging demonstrated dissection and pseudoaneurysm of the left ICA in addition to dissection of the right VA. After 2 wee

OBJECTIVE: To assess clinical outcomes of ruptured vertebral artery dissecting aneurysms, focusing on comparing parent artery occlusion (PAO) with stent-assisted treatments, and to identify risk factors for unfavorable outcomes and postprocedural complications.
METHODS: This retrospective review included 36 ruptured vertebral artery dissecting aneurysms treated between February 2009 and April 2020. Treatment modalities included PAO without stent and stent-assisted treatments. Stent-assisted treatments included PAO with posterior inferior cerebellar artery (PICA) stenting and stent-assisted co

Double origin of the posterior inferior cerebellar artery (PICA) is an infrequent developmental anomaly predisposing an individual to intracranial aneurysm formation. Fenestration of the PICA is extremely rare, and furthermore, fenestration of the double origin of the PICA in association with an intracranial aneurysm is unique. We present the first reported case of fenestration of the double origin of the PICA associated with a dissecting aneurysm of the contralateral vertebral artery (VA) that healed spontaneously after medical observation.

The primary clinical manifestation of vertebral artery dissection may be an isolated neck headache, and therefore, dissection diagnosis may be challenging in patients with chronic headaches. A headache that has acutely developed and changed its character, localization, or trigger, an atypical aura in patients with migraine, requires a specialist's scrutiny.

OBJECTIVE: To evaluate the efficacy of flow diverter implantation for treating CTA-negative giant vertebral artery dissection aneurysm (VADA) and to address the challenges in lesion characterization using MRI.
METHODS: A 66-year-old male patient presented with a 3-month history of left facial numbness and dysarthria. Initial MRI-T1 revealed a mixed signal intensity lesion in the CPA region. However, both CTA and digital subtraction angiography (DSA) failed to identify any significant vascular abnormalities. Subsequently, CT-perfusion and dynamic contrast-enhanced computed tomography (DCE-CT) …

Wallenberg syndrome, or lateral medullary syndrome, is caused by ischemia of the posterolateral medulla, most often attributed to vertebral … diffusion-restricted lesion in the right posterolateral medulla and signs of proximal intracranial vertebral artery dissection. Case 2 involved a 41-year-old man with vertigo, dysphonia, dysphagia, ipsilateral facial sensory loss, and contralateral body hypoalgesia. MRI showed an acute small infarct in the right posterolateral medulla without evidence of dissection. The com …

The video highlights a challenging case of bilateral vertebral artery dissection presenting with subarachnoid hemorrhage. The patient was found to have a critical flow-limiting stenosis in his dominant right vertebral artery and a ruptured pseudoaneurysm in his left vertebral artery. A single-stage endovascular treatment with stent reconstruction of the right vertebral artery and coil embolization sacrifice of the left side was performed. The case highlights the rationale for treatment and potential alternative strategies. The video can be found here: https://youtu.be/e0U_JE2jISw.

BACKGROUND: Vertebral artery dissection (VAD) may cause cerebral ischemia and impair quality of life (QOL) despite of good functional outcome. The aim of this study was the multimodal analysis of patient characteristics after VAD to identify contributing factors.
METHODS: In an exploratory study, 34 consecutive patients with first-ever spontaneous VAD were prospectively examined in comparison to 38 patients with cerebral ischemia without dissection and 25 stroke mimics as control groups. Multimodal assessment was performed for clinical, neurological, cognitive, psychological and radiological …

BACKGROUND: There are limited clinical studies of bilateral vertebral artery dissection (VAD).
OBJECTIVE: To compare the characteristics, imaging findings, and treatments between patients with bilateral and unilateral VAD.
METHODS: Between February 2007 and May 2017, 31 (mean age: 53.0 years; 23 men, 8 women) out of 171 VAD patients were hospitalized because of bilateral VAD. Onset type, dissection site, dominant side of the VA, imaging features, treatments, and outcomes were investigated based on medical records. The dominant side of the VA was determined by basi-parallel anatomical scannin…

Traumatic vertebral artery dissection (VAD) has been reported in various types of sports and recreation. However, there have been no reports of VAD in rugby players, except for those with cervical spine injuries. We experienced two cases of VAD without cervical spine injuries in high school rugby players. Attention should be paid to protect youth rugby players from VAD.

BACKGROUND: Arterial dissection is a rare complication of pregnancy and puerperium. There have been reports of aortic, coronary and cervical artery dissection in association with preeclampsia, however, vertebral artery dissection is rarely reported particularly in the antenatal setting in the presence of a Hypertensive Disorder of Pregnancy (HDP).The general annual incidence of symptomatic spontaneous cervicocephalic arterial dissection is 0.0026 % and a data registry reported that 2.4 % of these occurred in the post-partum period. The actual incidence of vertebral artery dissection in HDP is …

Vertebral artery (VA) dissection caused by swinging a golf club is extremely rare, and the mechanism of the dissection has not been elucidated. A 39-year-old man suddenly felt sharp neck pain and dizziness when he swung a driver while playing golf and visited our clinic. Imaging studies showed acute right cerebellar infarction and complete occlusion of the right VA at the C2 (axis) level. After 1 month of 100 mg aspirin treatment, the occluded right VA was completely recanalized and the patient became totally asymptomatic. Professional golfers look at the position of the ball on the ground or
