Comparison of Surgical Outcomes Between Vertebral Body Stenting (VBS) and Balloon Kyphoplasty (BKP)-Multicenter Cohort Study.

Background/Objectives: Vertebral body stenting (VBS) and balloon kyphoplasty (BKP) are widely used for the treatment of osteoporotic vertebral fractures (OVFs). However, it remains unclear whether the theoretical biomechanical advantages of VBS translate to superior clinical or radiographic outcomes. This study aimed to compare VBS and BKP with respect to clinical outcomes, radiographic parameters, and complications. Methods: In this multicenter retrospective comparative cohort study, 123 patients with OVF treated with VBS (n = 24) or BKP (n = 99) were analyzed. VBS was indicated for complex f…

Posterior spinal fusion versus vertebral body tethering for paediatric scoliosis: a meta-analysis of comparative studies.

PURPOSE: Posterior spinal fusion (PSF) is currently the gold standard technique for surgical correction of scoliosis however, there is a growing interest in non-fusion techniques like vertebral body tethering (VBT). The aim of this study is to compare surgical outcomes between PSF and VBT. METHODS: This systematic review was performed as per the Preferred Reporting Items for Systematic Reviews and Meta-analyses (PRISMA) Guidelines with a search of the following databases to identify all comparative studies: MEDLINE, EMBASE, and the Cochrane Central Register of Controlled Trials (CENTRAL). The efficacy of anterior vertebral body tethering in lenke type 6 curves for adolescent idiopathic scoliosis.

PURPOSE: Spinal fusion is the standard treatment for severe forms of adolescent idiopathic scoliosis (AIS). However, with the lowest instrumented vertebra that is usually located at L3 or L4, patients are prone to develop adjacent segment degeneration in the long term. Vertebral body tethering (VBT) as motion preserving technique has become an alternative for select patients with AIS. Several studies have presented the outcome after thoracic VBT but no study has analyzed the outcome after VBT for Lenke type 6 curves. METHODS: This is a retrospective single center data analysis of patients who…

Vertebral Body Erosions After Glue Embolization of an Aortic Endograft Type II Endoleak: A Case Report.

CASE: We present a case of a 66-year-old man with lumbar vertebral body erosions after glue embolization of a Type II endoleak secondary to endovascular repair of an infrarenal aortic aneurysm. Multiple biopsies of the affected vertebrae were culture-negative confirming no evidence of infection. He underwent posterior spinal fusion from L2 to L5 with complete resolution of mechanical low back pain and improved functional outcomes. CONCLUSION: Vertebral body osseous erosion is a rare complication of aortic endoleak intervention that can be successfully treated with spinal fusion.

Surgical complications and incomplete canal widening of the vertebral body sliding osteotomy to treat cervical myelopathy.

OBJECTIVE: Vertebral body sliding osteotomy (VBSO) is a surgical technique that anteriorly translates the vertebral body with compressive lesions and achieves cord decompression through canal widening. However, data on the surgical complications of VBSO are lacking. Furthermore, it has not been known whether VBSO could be a viable alternative in the treatment of cervical myelopathy even when the preoperative canal-occupying ratio (COR) is large, which seems to frequently result in incomplete canal widening. This study aimed to describe the incidence of VBSO-associated surgical complications an…

Anterior vertebral body tethering for thoracic idiopathic scoliosis leads to asymmetric growth of the periapical vertebrae.

PURPOSE: To evaluate 3D growth of the periapical vertebrae and discs in the 2 years after anterior vertebral body tether (AVBT) placement in patients with idiopathic scoliosis (IS). METHODS: Patients with IS treated with AVBT, ≥ 2 years of follow-up, and 3D spine reconstructions created from simultaneous, biplanar radiographs were studied. Patients were divided into two groups: progressive scoliosis correction (PC) or no/limited correction (NPC). The average of the 3 apical vertebral and disc heights and angular measures were made. The rate of change for each measure (mm/mo, °/mo) from first …

Post-operative tranexamic acid decreases chest tube drainage following vertebral body tethering surgery for scoliosis correction.

PURPOSE: Anterior vertebral body tethering (VBT) is a non-fusion surgical treatment for Adolescent Idiopathic Scoliosis requiring chest tube(s) (CT). We sought to assess the efficacy of post-op intravenous tranexamic acid (IV TXA) in reducing CT drainage and retention. METHODS: 35 VBT patients received 24 h of post-op IV TXA (2 mg/kg/h) were compared to 49 who did not. Group comparisons were performed using Wilcoxon rank-sum and chi-squared tests. Multivariate linear regression analysis was used to assess the relationships between TXA and both CT drainage and retention time. RESULTS: There w…

Prediction of post-operative adding-on or compensatory lumbar curve correction after anterior vertebral body tethering.

PURPOSE: Anterior Vertebral Body Tethering (AVBT), a fusionless surgical technique based on growth modulation, aims to correct pediatric scoliosis over time. However, medium-term curvature changes of the non-instrumented distal lumbar curve remains difficult to predict. The objective was to biomechanically analyze the level below the LIV to evaluate whether adding-on or compensatory lumbar curve after AVBT can be predicted by intervertebral disc (ID) wedging and force asymmetry. METHODS: 33 retrospective scoliotic cases instrumented with AVBT were used to computationally simulate their surger…

Percentile-based averaging and skeletal muscle gauge improve body composition analysis: validation at multiple vertebral levels.

BACKGROUND: Skeletal muscle metrics on computed tomography (CT) correlate with clinical and patient-reported outcomes. We hypothesize that aggregating skeletal muscle measurements from multiple vertebral levels and skeletal muscle gauge (SMG) better predict outcomes than skeletal muscle radioattenuation (SMRA) or -index (SMI) at a single vertebral level. METHODS: We performed a secondary analysis of prospectively collected clinical (overall survival, hospital readmission, time to unplanned hospital readmission or death, and readmission or death within 90 days) and patient-reported outcomes (p

The effects of vertebral body tethering on sagittal parameters: evaluations from a 2-years follow-up.

INTRODUCTION: While the effects of VBT on coronal parameters have been investigated in various studies, this has not yet been the case for sagittal parameters. This is of particular relevance considering that VBT does not allow direct correction of the sagittal profile. Thus, we investigated the effects of VBT on sagittal parameters in patients with adolescent idiopathic scoliosis. MATERIALS AND METHODS: Retrospective, 2-Center study. Patients who underwent VBT and presented a 2-years follow-up were included. The differences in sagittal parameters were evaluated, along with modifications of s

Vertebral compression fracture during stereotactic body radiotherapy for spinal metastasis: A rare case of tracking failure.

Spinal metastasis are a daily challenge in clinical practice. Stereotactic body radiotherapy (SBRT) allows delivery of definitive treatment with excellent long-term control rates. Its implementation needs dedicated devices and day-to-day image-guided radiotherapy (IGRT). The XSight™ spine tracking system, integrates with the CyberKnife® (Accuray™), provides a fiducial-free tracking system for spinal SBRT. We report a rare case of tracking failure during treatment due to the occurrence of a vertebral compression fracture (VCF).

The Link Between the 3D Spino-pelvic Alignment and Vertebral Body morphology in Adolescent Idiopathic Scoliosis.

BACKGROUND: Vertebral anterior overgrowth has been suggested as part of the etio-pathogenesis of adolescent idiopathic scoliosis (AIS). However, the link between 3D spinopelvic alignment and the vertebral anteroposterior height asymmetry in different scoliotic curves types and whether it deviates from the non-scoliotic controls, has not been studied. PURPOSE: We aimed to retrospectively describe the link between the anteroposterior vertebral height differences (ΔAPVH) measured in the true sagittal plane of each vertebra and the spinopelvic parameters in three anatomical planes. METHODS: 30 A

Titanium versus polyetheretherketone implants for vertebral body replacement in the treatment of 77 thoracolumbar spinal fractures.

BACKGROUND: Titanium and polyetheretherketone (PEEK) implants have been used in spinal surgery with low rejection rates. Compared to titanium, PEEK has many advantages, including a density more similar to that of bone, radiolucency, and a lack of artifacts in computed tomography (CT) and magnetic resonance imaging (MRI). In this study, we evaluated the effectiveness of PEEK cages as an alternative to titanium for bone fusion after fractures of the thoracolumbar spine. We also propose a classification to the impaction index. METHODS: We evaluated 77 patients with fractures of the thoracic or l

Blood flow in internal carotid and vertebral arteries during graded lower body negative pressure in humans.

What is the central question of this study? Recently, the heterogeneity of the cerebral arterial circulation has been argued. Orthostatic tolerance may be associated with an orthostatic stress-induced change in blood flow in vertebral arteries rather than in internal carotid arteries, because vertebral arteries supply blood to the medulla oblongata, which is the location of important cardiac, vasomotor and respiratory control centres. What is the main finding and its importance? The effect of graded orthostatic stress on vertebral artery blood flow is different from that on internal carotid ar
