Phobic postural vertigo: a new proposed entity.

BACKGROUND: Dizziness and vertigo can be a complaint in various psychiatric conditions where it usually constitutes only one of the features of the syndrome. Lately, a somatoform disorder characterized by almost mono-symptomatic dizziness and unsteadiness has been described. Since phobic postural vertigo usually presents without anxiety or other psychological symptomatology, patients with this condition seek help at neurologic and otolaryngologic clinics where they are often misdiagnosed as suffering from organic vertigo. OBJECTIVES: To present the clinical features of 55 consecutive patients…

Acoustic neuroma presenting as exercise-induced vertigo.

We present two subjects with previously undiagnosed acoustic neuromas who complained of vertigo whenever they ran. One had normal hearing while the other already had a unilateral sensorineural deafness. Hyperventilation for 30 seconds provoked an ipsilateral beating nystagmus and reproduced the vertiginous sensation in both subjects. Hyperventilation is a simple bedside test that should be performed when assessing a subject with vertigo or when there is a clinical suspicion of an acoustic neuroma.

[Clinical features of benign paroxysmal positional vertigo].

Our understanding of the pathomechanism of benign paroxysmal positional vertigo (BPPV) has improved dramatically. A type of BPPV featuring mixed torsional and vertical nystagmus induced by the Dix-Hallpike maneuver involves the posterior semicircular canal (P-BPPV). The other type of BPPV featuring horizontal nystagmus induced by spine-to-lateral head positioning involves the horizontal canal BPPV (H-BPPV). In complaints of vertigo or dizziness, 619 patients visited our department last year. Of these, 142 (23%) was had positional nystagmus consistent with a diagnosis of BPPV, 118 (19%) had no …

Management options for acute versus chronic vertigo.

Optimal management of the dizzy patient depends on an accurate diagnosis and knowledge of medical, surgical, and rehabilitative options. Acute vertigo may represent labyrinthine dysfunction treated with medication or a more serious central nervous system process requiring prompt intervention to avoid long-term disability. Chronic vertigo, however, may require surgical or rehabilitative measures for lasting relief. This article outlines the rationale for treating both groups with a variety of therapeutic options.

Intracranial tumors mimicking benign paroxysmal positional vertigo.

Benign paroxysmal positional vertigo is a common type of vertigo seen by the otolaryngologist; however, intracranial tumors can mimic benign paroxysmal positional vertigo in their presentation. A review of patients seen in the Department of Otolaryngology at The Ohio State University between July 1992 and August 1996 identified five patients with intracranial pathologic conditions mimicking benign paroxysmal positional vertigo. These patients were first seen with episodic vertigo associated with positional change. Failure of the symptoms to respond to the particle repositioning maneuver or the…

Characteristics of six otologic diseases involving vertigo.

To characterize otologic causes for vertigo, data on 564 patients with the six most common diseases involving vertigo were retrieved from the database of a computer-aided diagnostic system for neurotologic diseases. The diseases were Meniere's disease, vestibular schwannoma, benign paroxysmal positional vertigo, vestibular neuritis, sudden deafness, and traumatic vertigo. The prevalence of tinnitus in the study population was 76%. The most severe forms of vertigo and nausea were found in vestibular neuritis, whereas the most severe case of tinnitus appeared in Meniere's disease. Of the patient…

[Acute vertigo caused by cerebellar vascular accident].

At the beginning, small cerebellar strokes may present only with acute onset of vertigo, unsteadiness and unidirectional nystagmus, like a vestibular neuritis. In some cases, it is associated with tinnitus and hearing disturbance, like an endolymphatic hydrops. Other cases may mimic a benign cupulolithiasis, with only a paroxysmal positioning vertigo. Attention should be focused on transient associated symptoms: headache and blurred vision. One should not wait for classical cerebellar clinical signs: they are subtle and they appear late. Within a few days, the clinical picture will change: ver…

[Critical remarks on so-called cervicogenic vertigo].

A critical review of neurophysiological literature is given about the part played by the neck proprioceptors in the regulation of vestibular functions. In comparison with vestibular and optokinetic influences the neck proprioceptors are of secondary importance. A critical review of the publications by the supporters of a cervical genesis of vertigo leads to the conclusion that "cervical nystagmus" must be proven before the concept of neck-triggered vertigo can be accepted. In relation to all forms of vertigo, vertigo induced by cervical disorder represents a very low percentage only.

[Pathophysiology of peripheral, paroxysmal benign position vertigo].

Peripheral paroxysmal, benign positional vertigo is one of the most common peripheral vestibular disorders. According to the "cupulolithiasis hypothesis", it is assumed that inorganic material is attached to the cupula of the posterior vertical canal which renders this organ sensitive to gravitational forces rather than angular accelerations. Quantitative and well-controlled positioning manoeuvres, however, show that the characteristics of this kind of vertigo and the corresponding nystagmus cannot be explained by gravitation-dependent cupular reactions. It seems more probable that the typical…

[Nystagmus and benign paroxysmal vertigo (author's transl)].

Audio-impedence, vestibular, caloric, and electronystagmograph tests were conducted in 22 patients with benign paroxysmal vertigo. The disorder is provoked by certain sudden head movements, a latency period of several seconds occurring between the end of the movement and the onset of vertigo. This markedly differentiates it from other types of vertigo. It is of brief duration and associated with autonomic nervous system signs. Nystagmus, usually rotatory, is provoked by the particular head movement involved or by Hallpike's manoeuver. Results of the majority of tests were normal, occasional sl…

New dimensions of benign paroxysmal positional vertigo.

Twenty-nine patients who demonstrated the classic nystagmus of benign paroxysmal positional vertigo in the provocative, ear-down position had a high incidence of concurrent symptoms. These included vertigo provoked by arising, bending over, head rotation, linear acceleration, and vertical oscillation. Some have not been reported previously in relation to this syndrome. Elimination of both concurrent and classic symptoms via singular neurectomy in nine patients indicates a common pathophysiologic mechanism, probably involving cuplolithiasis in the posterior semicircular canal. These concurrent …

[Vertigo and vertebral artery stenosis (author's transl)].

Forty nine elderly patients with osteochondrosis of the cervical spine and on whom carotid angiography had been performed were divided into two groups. In one 36 patients had vestibular symptoms whereas the other group of remaining patients was free of such symptoms. Comparison of both groups showed that vertigo (20 patients), other vestibular symptoms or nystagmus were not solely related to vertebral artery stenosis. In fact stenosis of one or both vertebral arteries was found in only nine patients.

Vestibular dysfunction associated with benign paroxysmal vertigo.

Benign paroxysmal vertigo (BPV) is a clinical syndrome of vestibular origin although generally no evidence of vestibular dysfunction can be demonstrated with conventional tests. In a review of 1350 consecutive dizzy patients, there were 125 with BPV and of these, 33 underwent a quantitative rotational test of vestibular function. The rotational results showed reduces vestibular system gain for these BPV patients. In addition, they could be subdivided on the basis of a normal or shorter cupular time constant (Tc). Separation of patients into diagnostic categories revealed that those categorized…

[Non-ablative surgical therapies for peripheral vertigo].

Peripheral vestibular vertigo requires surgical management in 10 to 20 % of patients, who present persistent symptoms despite conservative treatment. The main indications include Ménière's disease, superior semicircular canal dehiscence, benign paroxysmal positional vertigo, and perilymphatic fistulas. Non-ablative interventions aim to control vertigo while preserving inner ear function. They include endolymphatic sac surgery, semicircular canal occlusion, and perilymphatic fistula repair.
