Association between the 20-minute whole blood clotting test and fibrinogen concentrations in green pit viper envenomations in Bangkok.

BACKGROUND: The 20-minute whole blood clotting test (20WBCT) is widely used in patients with viper envenomation in resource-limited settings. The unclotted result guides the need for antivenom administration. Confusion has arisen on how to interpret "partially clotted" for the test due to a paucity of data. This study's primary aim was to evaluate the association between fibrinogen concentrations (FC) and states of clotting from the 20WBCT in green pit viper (GPV) envenomation. METHODS: Patients aged ≥18 years who presented to our hospital with GPV bites were enrolled between September 2022 a

Post-Mortem Effects as a Variation Factor in Lachesis muta Venom.

Snakebite incidents could occur even with deceased animals due to improper animal manipulation, since the venom may retain biochemical activity. However, it is unclear what alterations death may cause in snake venom, because even in well-preserved genera, such as Lachesis, venom is susceptible to variation due to several factors. Therefore, the purpose of this research is to compare Lachesis muta venom characteristics before and after its death. The investigation encompasses protein profile analyses, immune recognition by antivenom, and enzymatic and biological activities. Venom samples were c

Clinical features of puff adder envenoming: case series of Bitis arietans snakebites in Kenya and a scoping review of the literature.

INTRODUCTION: The puff adder (Bitis arietans) is a medically important snake species found across much of Africa, yet there is limited literature on the clinical features and pathophysiology of envenoming after a puff adder bite. METHODS: We conducted a case-series study to describe the clinical features of patients with puff adder bites who were treated in two primary healthcare facilities in Kenya and complemented our case-series with a scoping review of all published cases of puff adder envenoming that contained sufficient clinical details to highlight the major features. RESULTS: Between

PLA2 Inhibitor Varespladib as an Alternative to the Antivenom Treatment for Bites from Nikolsky's Viper Vipera berus nikolskii.

Although envenoming by a small East European species of viper is rarely severe, and only exceptionally fatal, lack of specific antivenom stocks in a few areas within this region and possible severe side effects of antivenom application leave most bites to be treated only with antihistamines and supportive therapy. Varespladib is an effective inhibitor of snake phospholipase, and, as such, it could be considered as first-line therapy. The Nikolsky's viper venom contains an extremely high concentration of phospholipase A2 (PLA2), responsible for the toxic effects of the venom, as well as minor a

Inhibitory effects of ascorbic acid toward snake venom metalloproteinase (SVMP) from Indian Echis carinatus venom: Insights from molecular modeling and binding studies.

Classical antivenom therapy is unable to shield complications of viper bite and has limitations such as anaphylaxis and serum sickness. Snake venom metalloproteinases are responsible for local tissue damage and hemorrhage at the bitten site in viper envenomation, and this has led to a persistent search for metalloproteinase inhibitors. Here, we report the inhibitory effects of ascorbic acid against metalloproteinase from Echis carinatus venom both in-silico and in-vitro. Ascorbic acid effectively inhibited the proteolytic activity of E. carinatus venom in a dose-dependent manner. Interaction s

Proteome and Peptidome of Vipera berus berus Venom.

Snake venom is a rich source of peptides and proteins with a wide range of actions. Many of the venom components are currently being tested for their usefulness in the treatment of many diseases ranging from neurological and cardiovascular to cancer. It is also important to constantly search for new proteins and peptides with properties not yet described. The venom of Vipera berus berus has hemolytic, proteolytic and cytotoxic properties, but its exact composition and the factors responsible for these properties are not known. Therefore, an attempt was made to identify proteins and peptides de

Pit viper envenomation in a military working dog.

Military working dogs are often trained and/or work in locations where the potential for snake bites is increased. Knowledge of the local venomous snakes, the effects of their venom, and appropriate initial stabilization is essential for the US Army Veterinary Corps officer (VCO). As military practitioners, VCOs are uniquely situated to benefit from collaboration with other military assets for air evacuation and treatment of their patients. A recent clinical case of envenomation is presented, along with a review of the most current literature regarding treatment of envenomation in veterinary p

Identification of proteins interacting with ammodytoxins in Vipera ammodytes ammodytes venom by immuno-affinity chromatography.

In order to perform their function, proteins frequently interact with other proteins. Various methods are used to reveal protein interacting partners, and affinity chromatography is one of them. Snake venom is composed mostly of proteins, and various protein complexes in the venom have been found to exhibit higher toxicity levels than respective components separately. Complexes can modulate envenomation activity of a venom and/or potentiate its effect. Our previous data indicate that the most toxic components of the Vipera ammodytes ammodytes (Vaa) venom isolated so far-ammodytoxins (Atxs)-are

First draft of the genomic organization of a PIII-SVMP gene.

The evolutionary pathway of highly toxic proteins expressed in snake venom glands from proteins without toxic function and expressed in non-parotid tissues remains poorly understood. Here we examine gene structure of a representative of a venom protein with an ADAMs metalloproteinase evolutionary origin. The structure of the 15,652 bp Echis ocellatus pre-pro EOC00089-like PIII-SVMP gene was assembled from PCR-amplified sequences of overlapping genomic fragments. The gene comprises 12 exons interrupted by 11 introns. In a homology model of the EOC00089-like protein, the insertion of introns int

Life-threatening systemic toxicity and airway compromise from a common European adder bite to the tongue.

A 24-year-old man was bit on the tongue by a European common adder. Within 15 min following envenomation, he experienced tongue swelling, hypotension and impaired consciousness. Antihistamine, corticosteroid and crystalloids were administered. Within 105 min of envenomation, increasing oral, pharyngeal and facial oedema compromised the airway, leading to respiratory failure, concomitant with circulatory failure related to hypoxaemia and systemic toxic effects. Acute tracheotomy secured the airway, and two doses of antivenom successfully treated the systemic, toxic effects. The reaction was sev

Mechanisms of analgesic action of Gln49-PLA(2) from Gloydius ussurensis snake venom.

Gln49-PLA(2) is a phospholipase A(2) isolated from the Gloydius ussurensis snake venom. In this paper, we studied its effect on the function of neural conduction. Electrophysiological studies demonstrated that Gln49-PLA(2) reduced the amplitude of the action potential and the velocity of nerve conduction on isolated mouse sciatic nerve. Patch clamp recordings confirmed that Gln49-PLA(2) significantly decreased neural excitability by the potentiation of sodium channels and the blockade of potassium channels in nerve terminal. In freshly isolated hippocampal pyramidal neurons, 54.25% of potassiu

Transcriptome analysis of Deinagkistrodon acutus venomous gland focusing on cellular structure and functional aspects using expressed sequence tags.

BACKGROUND: The snake venom gland is a specialized organ, which synthesizes and secretes the complex and abundant toxin proteins. Though gene expression in the snake venom gland has been extensively studied, the focus has been on the components of the venom. As far as the molecular mechanism of toxin secretion and metabolism is concerned, we still knew a little. Therefore, a fundamental question being arisen is what genes are expressed in the snake venom glands besides many toxin components? RESULTS: To examine extensively the transcripts expressed in the venom gland of Deinagkistrodon acutus

Viper bites: treat or ignore? Review of a series of 99 patients bitten by Vipera aspis in an alpine Swiss area.

In a well defined alpine area of Switzerland (Valais, about 300000 inhabitants, tourists included) we studied retrospectively over 32 years, 99 patients bitten by vipers (Vipera aspis, the likely unique species in this area). The annual incidence was estimated at 3/100,000, as in other European countries. The mortality was 0% for the adults. The patients, 72 adults and 17 children (13 years and less), were classified in four groups: grade 0 no envenomation (8%), grade 1 minimal (42%), grade 2 moderate (40%), and grade 3 severe envenomation (10%). The 10 patients of grade 3 showed impressive cl

Comparative proteomics and subtyping of venom phospholipases A2 and disintegrins of Protobothrops pit vipers.

To explore the venom diversity and systematics of pit vipers under the genus Protobothrops, the venom phospholipases A2 (PLA2s) of P. mangshanensis, P. elegans and P. tokarensis were purified and characterized for the first time. The results were compared with the corresponding venom data of other co-generic species including P. mucrosquamatus, P. flavoviridis and P. jerdonii. Based on sequence features at the N-terminal regions, we identified five PLA2 subtypes, i.e., the Asp49-PLA2s with N6, E6 or R6 substitution and the Lys49-PLA2. However, not all subtypes were expressed in each of the spe

Enzymatic activity and inhibition of the neurotoxic complex vipoxin from the venom of Vipera ammodytes meridionalis.

Vipoxin from the venom of Vipera ammodytes meridionalis is an unique neurotoxic complex between a toxic phospholipase A2 and a highly homologous non-toxic protein inhibitor. It is an example of evolution of a catalytic and toxic function into inhibitory and non-toxic one. The activity of the V. ammodytes meridionalis toxin is 1.7 times higher than that of the closely related (92% sequence identity) neurotoxic complex RV4/RV7 from the venom of Vipera russelli formosensis The enhanced enzymatic activity of vipoxin is attributed to limited structural changes, in particular to the substitutions G5

Identification of bothrojaracin-like proteins in snake venoms from Bothrops species and Lachesis muta.

Bothrojaracin, a 27 kDa protein isolated from Bothrops jararaca venom, forms a non-covalent complex with thrombin, thus blocking its activity. We have previously identified a bothrojaracin-like protein in B. alternatus venom [Castro, H.C., Dutra, D.L.S., Oliveira-Carvalho, A.L., Zingali, R.B., 1998. Bothroalternin, an inhibitor of thrombin from the venom of Bothrops alternatus. Toxicon 36, 1903-1912]. In this report, we have examined snake venoms from six different Bothrops species (B. atrox, B. cotiara, B. jararacussu, B. moojeni and B. neuwiedi), from Lachesis muta and from Crotalus durissus

Lack of myocardial damage following Sri Lankan Russell's viper and hump-nosed viper bites.

INTRODUCTION: Cardiac effects following the bite of Burmese Russell's vipers and European vipers are well known. The question whether envenomation caused by Sri Lankan viper bites results in myocardial damage remains largely unanswered. The aim of this prospective study was to investigate whether myocardial damage occurs after Sri Lankan viper bites, using a highly specific and sensitive marker, troponin T. METHODS: 45 patients admitted after a definite viper bite [Russell's viper (RV), n = 13, hump-nosed viper (HNV), n = 32] were studied with regard to cardiac symptoms, ECG changes, and trop

Envenomation by the horned viper (Vipera ammodytes L.).

Snake venom poisoning is a medical emergency that requires urgent therapeutic procedures. The treatment of venomous snakebites is still controversial because of unclear therapeutic modalities. Choice of treatment is dictated in part by regional characteristics with regard to patient population and types of venomous snakes. The purpose of the study presented here was to report regional experience with venomous snakebites and to describe first-aid, pre-hospital, and hospital therapeutic procedures for horned viper bite. During a 16-year period, from 1980 to 1996, at the Clinical Hospital Split (

Correlation of renal status with hematologic profile in viperine bite.

Viper venom is mainly hemotoxic and causes coagulation disorders. One of the serious complications of viperine bite is renal failure. Forty adult cases of viper bite with systemic envenomation were studied for renal abnormality. Renal involvement was noted in 13 (32.5%) cases, of which eight (61.5%) had primary fibrinogenolysis and five (38.5%) had disseminated intravascular coagulopathy (DIC). Eight cases (61.5%) with renal dysfunction had incoagulable blood, whereas five (38.5%) had mild coagulation abnormality; however, none of the cases with incoagulable blood who received polyvalent anti-
