INTRODUCTION: Idiopathic gastroparesis (GP) is a syndrome characterized by delayed gastric emptying in the absence of a mechanical obstruction and the presence of cardinal symptoms, spontaneous idiopathic pneumoperitoneum is the presence of free air in the abdomen with the exclusion of a perforated viscera on endoscopy; both diseases have a low prevalence in which no detectable primary etiology can be identified. We observed the morphological characteristics and identified the species of gnathostome larvae obtained from the imported Chinese loaches. Gastroschisis is a congenital anomaly characterized by a defect in the anterior abdominal wall with protrusion of abdominal viscera. Perioperative mortality is very high in these patients. Traditionally gastroschisis repair has been performed under general anesthesia with endotracheal intubation, requiring postoperative intensive care admission and mechanical ventilation. Caudal block is an attractive alternative to general anesthesia. We present a series of three neonates with gastroschisis, repaired solely under caudal anesthesia.

In a French retrospective survey including 103,852 laparoscopic procedure, and 390,000 port-insertion, trocar injuries rate have been evaluated. Mortality is 0.07 per thousand, quite exclusively in relation with vascular injuries (especially from aorta and iliac vessels) with a rate of 0.4 per thousand. All intra-abdominal viscera and vessels must be injured by the first blind trocar insertion. Open laparoscopy is systematically proposed to prevent these accidents.

Necrotising arteritis was found in the spinal arterial branches of the lumbar spinal cord and in skeletal muscles, bones, peripheral nerves, and most viscera, except lungs and brain, of a 41-year-old female who died after a protracted illness finally characterized by severe paralysis due to progressive peripheral neuropathy. Serious intra-abdominal injuries in neonates are very rare. In addition, the signs and symptoms of hemoperitoneum caused by bleeding from solid viscera are vague and nonspecific and often are not recognized before the onset of hypovolemic shock or death. In this report, we describe a 2-day-old infant who presented with shock and pallor who had a ruptured spleen, presumably from birth. We also review the literature and the importance of recognizing this injury in the emergency department setting.

A 34-week stillborn infant had omphalocele, agenesis of the sternum and anterior rib cage, membranous diaphragms with eventration of the viscera, ectopia cordis with absence of the pericardium, and congenital heart defect. These findings are consistent with a diagnosis of Cantrell pentalogy. The presence of bilateral clubfeet, spina bifida, hydrocephalus, abnormal ears, and horseshoe kidneys suggested a chromosome abnormality. Chromosome analysis showed trisomy 18. Individuals with manifestations of Cantrell pentalogy deserve cytogenetic evaluation.

Twenty-seven patients were studied after colposuspension with lateral bead-chain urethrocystography. Successful operation, compared with unsuccessful operation, repositioned the bladder neck significantly closer to the posterosuperior surface of the symphysis pubis, though not significantly higher. After successful colposuspension, the proximal urethra is exposed to compression against the symphysis pubis by the momentary descent of the pelvic viscera during physical effort. Failure of the colposuspension was usually due to an inadequate repositioning of the bladder neck.

The authors describe the diagnostic ultrasound findings of pelvic lipomatosis in a patient presenting with lower extremity thrombophlebitis. In this condition, ultrasound shows the full urinary bladder to assume a tubular or "cigar" shape and to "float" in the pelvic fat with failure to distend normally to the pelvic side walls. Confirmatory computed tomography discloses normal muscular and osseous anatomy but an increase in radiolucent fat surrounding the pelvic viscera. Clinical and radiographic findings are reviewed, and a differential diagnosis is presented.

Situs inversus totalis is the complete transpositioning of thoracoabdominal viscera into a mirror image of the normal configuration. Choledochal cyst is the congenital cystic dilation of the biliary tract. Both these conditions coexisting in a patient is extremely rare. We hereby present a case of type IC choledochal cyst in a patient with situs inversus totalis presenting with biliary sepsis secondary to choledocholithiasis. Also detailed are the management and operative strategies employed to deal with this rare entity.
