Pediatric Gartland Type-IV Supracondylar Humeral Fractures Have Substantial Overlap with Flexion-Type Fractures.

BACKGROUND: Knowledge is limited about the diagnosis and treatment of modified Gartland type-IV supracondylar humeral fractures. We determined the prevalence of type-IV fractures, identified preoperative characteristics associated with these injuries, and assessed operative treatment characteristics. METHODS: We retrospectively identified patients <16 years of age who underwent operative treatment of a supracondylar humeral fracture at 2 centers between 2008 and 2016. We compared patient, injury, and treatment characteristics between type-IV and type-III fracture groups (1:4, cases:controls).

Osteochondral Autograft Transplantation for Capitellar Osteochondritis Dissecans.

BACKGROUND: The goal of the osteochondral autograft transplantation (OAT) procedure is to replace both the bone and cartilage that have been compromised by osteonecrosis of the capitellum, a condition known as osteochondritis dissecans (OCD). In children, the vascularity of the capitellum is limited compared with that in adults because the physis acts as a physical barrier to vascular ingrowth from the metaphysis to the epiphysis. The necrotic subchondral bone cannot keep up with the weight-bearing demands of certain high-level athletes such as gymnasts, accumulating microfractures and eventua

Thirty-six months of experience and 30° of hip internal rotation as critical thresholds for capitellar osteochondritis dissecans in youth baseball players.

BACKGROUND: Capitellar osteochondritis dissecans (OCD) is a serious throwing injury that can lead to long-term elbow dysfunction. While a longer duration of competitive play is a suspected risk factor, no specific cutoff value has been established to identify players at high risk. PURPOSE: To identify independent risk factors for capitellar OCD and determine the optimal cutoff value for the duration of competitive play in youth baseball players. METHODS: This cross-sectional study analyzed 853 youth baseball players (aged 9-12 years) who participated in medical check-ups. OCD was diagnosed u

Sensitivity and Specificity of the Brachial Plexus Outcome Measure Activity Scale: A Retrospective Cohort Study.

PURPOSE: Upper limb impairments occur after incomplete recovery from brachial plexus birth injury (BPBI). Reconstructive surgeries are offered to address specific joint-level impairments; however, complete restoration is not possible. Thus, surgical decisions must carefully consider the goal of improving activity function within the child's life context. The Brachial Plexus Outcome Measure (BPOM) Activity scale is an outcome measure of upper limb activity limitation in children with BPBI. It measures the upper limb function on a 5-point ordinal scale, where a score of ≤3 is theoretically an in

Modified Technique of Single-Bone Forearm in the Treatment of Deformities.

The single-bone forearm is a salvage technique for massive loss of bone due to serious trauma, malignant tumors, infections or congenital deformity. It is also described to treat the sequelae of hereditary multiple exostoses disease that affects the distal end of the ulna. We present the case of a 29-year-old patient, operated for sequelae of hereditary multiple exostoses disease of the left forearm by a modified single-bone forearm technique. The patient, right-handed, operated on twice in childhood for a hereditary multiple exostoses disease of the left forearm: incomplete excision of the ex

The treatment of isolated Mason type II radial head fractures: a systematic review.

BACKGROUND: Fractures of the radial head represent the most common bony injury of the elbow in adults. Radial head fractures are classified according to Mason or one of its classification modifications. Current literature does not indicate consensus on whether to treat isolated stable type II radial head fracture patterns with open reduction and internal fixation (ORIF) or nonoperatively, especially, when there is no mechanical block to motion. METHODS: We systematically reviewed the available literature searching electronic databases, that is, MEDLINE using the PubMed interface and Embase, f

Wrist and hand postures when falling and description of the upper limb falling reflex.

INTRODUCTION: Falling studies, i.e. assessing what happens when an individual falls, have been conducted in controlled environments but not in field studies for ethical reasons; this potentially limits the validity and applicability of previous studies. We performed field studies on existing YouTube © videos of skateboarders falling. The aims of these studies were to measure the wrist angle at impact on videos of real unprotected falls and to study the dynamics of the upper limbs when falling. METHODS: Youtube © videos of skateboarders falling were studied assessing the direction of the fall,

Pediatric Contractures in Burn Injury: A Burn Model System National Database Study.

Joint contractures are a major cause of morbidity and functional deficit. The incidence of postburn contractures and their associated risk factors in the pediatric population has not yet been reported. This study examines the incidence and severity of contractures in a large, multicenter, pediatric burn population. Associated risk factors for the development of contractures are determined. Data from the National Institute on Disability and Rehabilitation Research Burn Model System database, for pediatric (younger than 18 years) burn survivors from 1994 to 2003, were analyzed. Demographic and m

Arthroscopically-assisted treatment of pediatric lateral humeral condyle fractures.

BACKGROUND: Pediatric lateral humeral condyle fractures are common injuries. Current treatment recommendations include nonoperative treatment for nondisplaced Milch type 1 fractures; however, truly nondisplaced fractures may be rare. Although closed reduction and percutaneous pinning under arthrographic visualization are most commonly used, anatomical reconstitution of the articular surface may require a lateral Kocher approach. This approach may compromise the vascularity of the distal fragment. To avoid this catastrophic complication while still obtaining anatomical articular surface reducti

Factors related to injury in youth and adolescent baseball pitching, with an eye toward prevention.

The baseball and medical communities have recognized injuries of the upper extremity associated with the overhead throwing motion and have examined manners in which to reduce risk. The authors examine special features of the adolescent pitcher as compared with skeletally mature pitchers and characteristics of pitch mechanics, type, velocity, fatigue, and overuse. A summary is presented on previously identified risk factors as they relate to pitching injuries. Development, use, and compliance with the most current pitching guidelines in youth baseball, based on the available evidence, will be p

Ultrasound Evaluation and Treatment of Posterior Medial Antebrachial Cutaneous Nerve Injury Following Cubital Tunnel Release.

This case highlights the diagnostic value of high-resolution ultrasonography in identifying postoperative injury of the posterior branch of the medial antebrachial cutaneous nerve (MABCN) following cubital tunnel surgery. A 45-year-old man developed localized pain, warmth, and paresthesia extending from the medial epicondyle to the proximal anterior forearm one month after ulnar nerve decompression and anterior transposition. Physical examination revealed focal allodynia and a positive Tinel sign. Because previous surgery may substantially alter the anatomical location of the surrounding nerve

Corticosteroid infiltration in partial distal biceps ruptures.

BACKGROUND: Little is known about the potential negative or positive effects of peritendinous infiltration with corticosteroids in the non-surgical treatment of partial distal biceps tendon tears. Peritendinous fluid, synovitis, and bursitis often accompany partial tears and can be a source of persistent pain. We hypothesize that peritendinous corticosteroid infiltration is a safe non-surgical treatment option for complaints related to a partial distal biceps tendon rupture. METHODS: A single-center retrospective analysis was performed on a cohort of 52 patients with partial distal biceps tea

Stress Fracture of Proximal Femur after Hip Resurfacing Treated with Cannulated Screw.

Background  Myositis ossificans traumatica (MOT) involves the heterotopic development of lamellar bone after a traumatic injury. Despite being termed "myositis," MOT is not limited to muscle but rather can involve tendons, fat, and fascia. "Traumatica" reflects that lesions are usually associated with a history of significant trauma, that is, fractures or surgery; however, many reports suggest they can also be linked to repetitive low-energy insults. In both cases, the inflammatory response secondary to tissue injury generates a proliferative osteoblastic cascade. Case Description  We present
