RATIONALE: Slipped capital femoral epiphysis (SCFE) is a common hip problem in adolescents, usually individuals between 8 and 15 years old. Because of the frequent finding of growth abnormalities in affected children, various endocrine disturbances have been reported as the cause of the disease. However, there are few case reports of older patients in previous literature. To the best of our knowledge, congenital hypopituitarism with normal growth hormone (GH) level has not been reported.
PATIENT CONCERNS: We describe a 29-year-old man who had a 3-month history of pain in the left hip with tal

BACKGROUND: Slipped capital femoral epiphysis (SCFE) is a common disorder in adolescent for which no consensus exists regarding management. The aim of the present study was to analyze gait modifications following SCFE treated with in situ fixation (ISF) and to relate it to radiologic stage.
RESEARCH QUESTION: To verify if gait biomechanics are impaired in patients with SCFE and to try to determine a degree of slippage from which gait modifications would appear.
METHODS: We evaluated 16 patients treated by ISF for SCFE with slippage ranging from 11° to 61°. Gait variables were compared to nor

BACKGROUND: Isolated coronoid, isolated radial head, and combined coronoid and radial head fractures are common elbow fractures, and specific ligamentous injury of each fracture configuration has been reported. However, the osseous injury mechanism related to ligament status remains unclear.
QUESTIONS/PURPOSES: The objectives of this study were: (1) to determine what ligamentous injury patterns (medial or lateral collateral) and bone contusion patterns (medial or lateral) are associated with isolated coronoid, isolated radial head, and combined coronoid and radial head fractures; (2) to corre

BACKGROUND: Fixation of lateral condyle distal humeral fractures has traditionally been achieved with K-wires. Screw fixation provides the advantage of compression across the fracture site, but the results of screw fixation and risk of iatrogenic physeal damage are not well defined. This study was designed to evaluate the efficacy of screw fixation for lateral condyle fractures.
METHODS: A retrospective study of patients with lateral condyle elbow fractures treated using screw fixation at a single institution was undertaken. Patients 12 years and younger with isolated fractures were included.

UNLABELLED: Slipped capital femoral epiphysis (SCFE) is a common hip condition in adolescents, most commonly treated with in-situ cannulated screw fixation. We report two cases of cannulated screw failure within the femoral neck following SCFE fixation. To our knowledge, this is the first reported case in the literature of cannulated screw failure within the femoral neck following in-situ screw fixation for unstable SCFE.
LEVEL OF EVIDENCE: Level IV.

BACKGROUND: Anterior cruciate ligament (ACL) reconstruction in skeletally immature patients can result in growth disturbance due to iatrogenic physeal injury. Multiple physeal-sparing ACL reconstruction techniques have been described; however, few combine the benefits of anatomic reconstruction using sockets without violation of the femoral or tibial physis.
PURPOSE: To utilize physeal-specific magnetic resonance imaging (MRI) to quantify the zone of physeal injury after all-inside ACL reconstruction in skeletally immature athletes.
STUDY DESIGN: Case series; Level of evidence, 4.

Intrasubstance tears of the anterior cruciate ligament (ACL) were once considered a rare injury in skeletally immature athletes but are now observed with increasing frequency. Treatment strategies have evolved as recent studies have identified unique considerations specific to the skeletally immature patient. The current literature now supports the trend toward early operative treatment to restore knee stability and prevent progressive meniscal and/or articular cartilage damage, but the optimal approach to ACL reconstruction in this age group remains controversial. Despite the reported clinica

Inflammatory pseudotumours occasionally occur after metal-on-metal hip resurfacing and often lead to revision. Our aim was to determine the severity of this complication by assessing the outcome of revision in these circumstances and by comparing this with the outcome of other metal-on-metal hip resurfacing revisions as well as that of matched primary total hip replacements. We identified 53 hips which had undergone metal-on-metal hip resurfacing and required revision at a mean of 1.59 years (0.01 to 6.69) after operation. Of these, 16 were revised for pseudotumours, 21 for fracture and 16 for

BACKGROUND: Intravenous bisphosphonate therapy is associated with preservation of femoral head sphericity and congruence in 77% of children with traumatic avascular necrosis. The aim was to describe the systemic effects of intravenous zoledronic acid (ZA) on bone and mineral metabolism in otherwise normal children and adolescents with femoral head AVN.
MATERIAL AND METHODS: 37 children (age 10.8+/-2.76 years) diagnosed with avascular necrosis AVN (Slipped Capital Femoral Epiphysis (SCFE), N=20 or Legg-Calve-Perthes disease (LCPD), N=17) were treated with at least 12 months of ZA. Bone mineral

BACKGROUND: The aims of this study were to evaluate outcomes of percutaneous epiphysiodesis, as described by Bowen, in 97 patients and to assess complications and determine whether the effect of the epiphysiodesis will accurately follow the Moseley chart predictions.
METHODS: A total of 97 patients were reviewed retrospectively. Fifty-six girls and 41 boys, with a mean skeletal age of 12.6 years (range, 10-16 years) at the time of operation, were followed up until skeletal maturity, a mean of 3.8 years (range, 1-10 years).
RESULTS: The mean residual leg length discrepancy (LLD) in 88 patient

A narrow window of bone age (BA) in slipped capital femoral epiphysis (SCFE) was described a decade ago. Children now mature younger. Does the BA narrow window still exist? It is the purpose of this study to investigate this question. A retrospective review of children with idiopathic SCFE (1998-2003) was performed. The initial study data ("past") were used as a comparison. Anteroposterior pelvis radiographs were scored for BA (Oxford method) in 108 children. Only the first radiograph was used for children with sequential bilateral SCFEs. There were 67 boys and 41 girls. There were 73 children

Patients with stable slipped capital femoral epiphysis (SCFE) usually can ambulate at the time of diagnosis. Satisfactory results have been reported after percutaneous in situ pinning using a fracture table. The authors describe a technique to determine the skin-pin entry point for percutaneous pinning of the hip on a regular radiolucent operating table. The pin entry point determined by this modified method was reliable in 15 SCFEs in 13 patients. Pinning on a regular radiolucent table was much easier, without the need to transfer obese patients to a fracture table. It was also useful when a

With regard to injuries of the extremities caused by birth trauma, ultrasound allows simultaneous direct evaluation of the surface of osseous elements and of cartilaginous and soft-tissue structures as well. Thus, not only fractures, but also concomitant articular fluid collections, respectively haematomas and/or dislocation of an epiphysis are demonstrated in their initial extent and in their course of healing. Additionally, closed repositions, for instance of a displaced epiphysis, can be exactly monitored by ultrasound. A reduced number of roentgenograms as well as renunciation of other ima

The article describes the various opinions on the etiology of Legg-Calvé-Perthes disease: 1. The theory of double infarction. 2. The theory of venous stasis. 3. The latent prolonged ischemic phase. Due to the varying development of the vasculature of the proximal end of the femur, with a diminishing number of arteries ensuring blood supply and the subsequent reduction in the speed of flow in the myeloic sinuses, we describe here the origin of the latent ischemic phase of the proximal epiphysis of the femur, which constitutes the predisposition for the development of Perthes' disease. In the di
