RNA splicing. The human splicing code reveals new insights into the genetic determinants of disease.

To facilitate precision medicine and whole-genome annotation, we developed a machine-learning technique that scores how strongly genetic variants affect RNA splicing, whose alteration contributes to many diseases. Analysis of more than 650,000 intronic and exonic variants revealed widespread patterns of mutation-driven aberrant splicing. Intronic disease mutations that are more than 30 nucleotides from any splice site alter splicing nine times as often as common variants, and missense exonic disease mutations that have the least impact on protein function are five times as likely as others to

Sensorimotor polyneuropathy in patients with SMA type-1: electroneuromyographic findings.

INTRODUCTION: Generally, spinal muscular atrophy (SMA) is believed to be a pure motor neuron disease. We retrospectively evaluated our electrodiagnostic findings in SMA type 1 patients to demonstrate co-existence of sensorimotor neuropathies. METHODS: Electroneuromyographic (ENMG) studies in 15 patients (11 boys, 4 girls) were reviewed independently by 2 neurophysiologists. Upper extremity findings were compared with normal right arm controls. RESULTS: Patient ages ranged from 1.5 to 26 months. Four SMA patients (26.7%) had decreased sensory nerve action potentials (SNAPs) or sensory nerve c

Daily salbutamol in young patients with SMA type II.

The aim of this open pilot study was to establish the profile of tolerability and clinical response of salbutamol (albuterol) in a cohort of young children affected by type II spinal muscular atrophy (SMA). Twenty-three children between 30 months and 6 years of age were treated with salbutamol (2 mg three times a day) for 1 year. All children were longitudinally assessed using the Hammersmith motor functional scale 6 months before treatment started (T0), at baseline (T1) and 6 and 12 months later. There was no significant change in function between T0 and T1 assessments, but the functional sco

Local Translation in Glial Cells of the Brain.

Local protein synthesis is a conserved mechanism that allows cells with intricate architectures to perform compartment-specific functions. By translating messenger RNAs (mRNAs) at distinct subcellular locations, cells can respond swiftly and precisely to localized stimuli. This strategy is crucial in neurons, whose long processes extend far from the cell body. Disruptions in neuronal local translation have been implicated in neurological disorders, including fragile X syndrome, amyotrophic lateral sclerosis, and spinal muscular atrophy. While much of the spotlight has been on neurons, glial ce

Balancing RNA processing and innate immune response: Possible roles for SMN condensates in snRNP biogenesis.

Biomolecular condensates like U-bodies are specialized cellular structures formed through multivalent interactions among intrinsically disordered regions. U-bodies sequester small nuclear ribonucleoprotein complexes (snRNPs) in the cytoplasm, and their formation in mammalian cells depends on stress conditions. Because of their location adjacent to P-bodies, U-bodies have been considered potential sites for snRNP storage or turnover. SMN, a chaperone for snRNP biogenesis, forms condensates through its Tudor domain. In fly models, defects in SMN trigger innate immune responses similar to those o

Splicing inhibition of U2AF65 leads to alternative exon skipping.

U2 snRNP auxiliary factor 65 kDa (U2AF(65)) is a general splicing factor that contacts polypyrimidine (Py) tract and promotes prespliceosome assembly. In this report, we show that U2AF(65) stimulates alternative exon skipping in spinal muscular atrophy (SMA)-related survival motor neuron (SMN) pre-mRNA. A stronger 5' splice-site mutation of alternative exon abolishes the stimulatory effects of U2AF(65). U2AF(65) overexpression promotes its own binding only on the weaker, not the stronger, Py tract. We further demonstrate that U2AF(65) inhibits splicing of flanking introns of alternative exon i

Yeast Augmented Network Analysis (YANA): a new systems approach to identify therapeutic targets for human genetic diseases.

Genetic interaction networks that underlie most human diseases are highly complex and poorly defined. Better-defined networks will allow identification of a greater number of therapeutic targets. Here we introduce our Yeast Augmented Network Analysis (YANA) approach and test it with the X-linked spinal muscular atrophy (SMA) disease gene UBA1. First, we express UBA1 and a mutant variant in fission yeast and use high-throughput methods to identify fission yeast genetic modifiers of UBA1. Second, we analyze available protein-protein interaction network databases in both fission yeast and human t

Lymphoproliferative disorders and motor neuron disease: an update.

We studied 26 patients with both motor neuron disease and lymphoproliferative disease (LPD). Twenty-three patients had definite or probable upper motor neuron signs; none had electrophysiologic evidence of motor neuropathy. LPD syndromes comprised Waldenström's macroglobulinemia, multiple myeloma, chronic lymphocytic leukemia, follicular cell lymphoma, and Hodgkin's disease. In all but one patient, the cause of disability or death was neurologic. LPD was confined to bone marrow in 14 patients; eight of 14 had monoclonal paraproteinemia. One patient had LPD discovered at autopsy. Treatment of L

Short-term and long-term outcomes of transcutaneous spinal cord stimulation in SMA: findings from consecutive courses.

Objective.Spinal muscular atrophy (SMA) is a genetic disorder that causes the progressive degeneration of spinal motor neurons, which can result in premature death. Restoring motor function in people with SMA is a serious issue. These individuals require lifelong motor rehabilitation to maintain their quality of life. A recent study showed that a two-week course of transcutaneous electrical spinal cord stimulation (tSCS) combined with physical therapy led to clinically significant improvements in Revised Upper Limb Module (RULM) and Hammersmith Functional Motor Scale Expanded (HFMSE), improved

Effect of subthalamic deep brain stimulation on posture in Parkinson's disease: A blind computerized analysis.

INTRODUCTION: We sought to assess the effect of subthalamic deep brain stimulation (STN DBS) on Parkinson's disease (PD)-associated postural abnormalities. METHODS: A computerized analysis of posture was used to quantify the thoracolumbar, thoracic, and cervical-occipital ventral angles, as well as the thoracolumbar and cervical-occipital lateral angles from the video-repository of three specialized movement disorder centers (n = 158 patients). Data was extracted from frames from video-recordings in the pre-surgical medication-ON (dopaminergic therapy) and post-surgical stimulation-ON/medicat

Snake-Eyes Appearance on MRI Occurs during the Late Stage of Hirayama Disease and Indicates Poor Prognosis.

PURPOSE: Because Hirayama disease is stereotyped as a self-limited disease in the absence of a definite pathology, we investigated the potential relationship between snake-eyes appearance (SEA) and Hirayama disease to bring a new perspective in the pathological process of Hirayama disease based on relevant radiological and clinical evidence. METHODS: A total of 30 cases observed SEA were selected from 293 patients with Hirayama disease to constitute the SEA group, and an equal number of cases were randomly selected from the remaining patients to form the non-SEA group. Age-dependent SMN expression in disease-relevant tissue and implications for SMA treatment.

BACKGROUNDSpinal muscular atrophy (SMA) is caused by deficient expression of survival motor neuron (SMN) protein. New SMN-enhancing therapeutics are associated with variable clinical benefits. Limited knowledge of baseline and drug-induced SMN levels in disease-relevant tissues hinders efforts to optimize these treatments.METHODSSMN mRNA and protein levels were quantified in human tissues isolated during expedited autopsies.RESULTSSMN protein expression varied broadly among prenatal control spinal cord samples, but was restricted at relatively low levels in controls and SMA patients after 3 mo

Camptocormia in Chinese patients with Parkinson's disease.

To investigate the frequency and clinical characteristics of camptocormia in Chinese PD patients, we included 705 patients with PD and studied the clinical features and prevalence of camptocormia. Forty-six (6.5%) patients presented with camptocormia at the time of evaluation. The mean disease duration of PD patients with camptocormia was significantly longer than that without camptocormia (P < 0.01). After adjusted for age and disease duration, compared with patients without camptocormia, PD patients with camptocormia presented with higher score of UPDRS part III (P < 0.01), higher H&Y stage

Morphological changes of the lower cervical spinal cord under neutral and fully flexed position by MRI in Chinese patients with Hirayama's disease.

We examined MR images of the spinal cord from the neutral neck to the fully flexed neck position in Chinese patients with Hirayama's disease (HD). MRI was performed on 25 HD patients and 31 healthy controls in the neutral and fully flexed neck positions. The mean anterior-posterior diameter (APD) and transverse diameter (TD) of the cervical cord at C6 level in the different positions was measured. Localized lower cervical cord atrophy and asymmetric cord flattening in a neutral neck position were highly suggestive of HD. The mean APDs were 5.6+/-0.7 mm in HD patients and 6.7+/-0.5 mm in contro

Charcot-Marie-Tooth's disease with severe trophic and sensory disorders. Study of a case following along a half a century with anatomical studies.

Plantar ulcers of neurological origin are known to be associated with several disorders, some of which may be familial. We present the case report together with the clinical and post-mortem data of a patient with peroneal muscular atrophy of the Charcot-Marie-Tooth type and plantar ulcers as a prominent feature. At least three other members of her family had Charcot-Marie-Tooth disease. The family pedigree is included. The discussion stresses the difference between patients with Charcot-Marie-Tooth disease with associated manifestations, bony destruction of the feet and plantar ulcers and pati

Surgical management of camptocormia in Parkinson's disease: systematic review and meta-analysis.

OBJECTIVE: Camptocormia is a potentially debilitating condition in the progression of Parkinson's disease (PD). It is described as an abnormal forward flexion while standing that resolves when lying supine. Although the condition is relatively common, the underlying pathophysiology and optimal treatment strategy are unclear. In this study, the authors systematically reviewed the current surgical management strategies for camptocormia. METHODS: PubMed was queried for primary studies involving surgical intervention for camptocormia in PD patients. Studies were excluded if they described nonsurg

Body height loss characterizes camptocormia in Parkinson's disease.

Axial deformities such as camptocormia or Pisa syndrome in people with Parkinson's disease (PwP) are poorly understood. The scarcity of information may result from the shortage of reliable and responsive evaluation instruments. We evaluated the body height loss (BHL) as a new measure for PwP with axial deformities. 50 PwP with axial deformity defined by an UPDRS item 28 value of at least 2 were included in this mono-center study. We measured body height while lying supine and after 1 min of standing, providing a percentage value of BHL, and compared this measure to other clinical variables. BH
