Morbidity and mortality from pharmacosedation and general anesthesia in the dental office.

Morbidity and mortality (M&M) statistics have been used to determine the safety of pharmacosedation and general anesthesia for dental procedures. Although relevant, these data often do not describe what actually caused the problems. Descriptive data are needed to understand etiologic factors and to accurately set malpractice insurance rates, establish legislative regulations, and determine means of prevention. The purpose of this study was to characterize the factors involved in causing M&M in a national data base of dental patients who received either pharmacosedation or general anesthesia. L

Intraoperative Acute Cardiac Tamponade as a Result of Intracardiac Perforation Requiring Emergency Continuous Pericardiocentesis and Open Sternotomy: A Case Report and Literature Review of a Rare but Fatal Complication.

Intraoperative acute cardiac tamponade associated with iatrogenic intracardiac perforation from percutaneous interventional cardiac procedures is a rare but potentially catastrophic complication. We report a case of intraoperative acute hemopericardium caused by a left atrial (LA) perforation resulting in cardiac tamponade in a patient undergoing a baffling procedure for the correction of two anomalous pulmonary veins draining into her superior vena cava (SVC) that required continuous pericardiocentesis with autologous blood transfusion via the femoral vein and an emergency intraoperative tran

Consensus practice guidelines on interventions for cervical spine (facet) joint pain from a multispecialty international working group.

BackgroundThe past two decades have witnessed a surge in the use of cervical spine joint procedures including joint injections, nerve blocks and radiofrequency ablation to treat chronic neck pain, yet many aspects of the procedures remain controversial.MethodsIn August 2020, the American Society of Regional Anesthesia and Pain Medicine and the American Academy of Pain Medicine approved and charged the Cervical Joint Working Group to develop neck pain guidelines. Eighteen stakeholder societies were identified, and formal request-for-participation and member nomination letters were sent to those

Endoscopic ultrasound-guided transoesophageal pericardiocentesis: a case report on a therapeutic solution for pericardial tamponade with malignant posterior pericardial effusion.

BACKGROUND: Treatment options for malignant loculated pericardial effusions are limited and effusions often reaccumulate following surgical management. For posterior effusions, access percutaneously is challenging, hence alternative approaches may be needed. Here, we report a rare case of transoesophageal endoscopic ultrasound (EUS)-guided pericardiocentesis for recurrent loculated posterior malignant pericardial effusion causing haemodynamic compromise. CASE PRESENTATION: A 65-year-old male was diagnosed with metastatic lung adenocarcinoma following an emergency admission with malignant card

Intravenous patient-controlled analgesia versus epidural anesthesia in the treatment of patients undergoing total hip arthroplasty: a retrospective comparative study.

BACKGROUND: Total hip arthroplasty is a common surgical procedure used to reduce pain and improve hip joint function in patients with advanced hip inflammation. Patient-controlled epidural analgesia is more effective in reducing postoperative anxiety. In addition, combined spinal and epidural anesthesia is associated with a lower incidence of respiratory complications like atelectasis. This retrospective comaprative study compared the post operative analgesic effect and postoperative recovery of intravenous patient-controlled analgesia and patient-controlled epidural analgesia. METHODS: A tot

Consensus on Upper Gastrointestinal Endoscopy Key Performance Indicators to Reduce Post Endoscopy Upper Gastrointestinal Cancer.

BACKGROUND: Upper gastrointestinal (UGI) endoscopy lacks established key performance indicators. Up to three-fold variation in post endoscopy upper gastrointestinal cancer rates has been observed among endoscopy providers in England, highlighting the need for standardisation of UGI endoscopy practices. OBJECTIVE: We aimed to achieve consensus on evidence-based key performance indicators to reduce post endoscopy upper gastrointestinal cancer. METHODS: Modified nominal group technique was employed in two consensus workshops, with representation from clinicians, patients and relatives, moderate

Inhaled nitrous oxide for painful procedures in children and youth: a systematic review and meta-analysis.

OBJECTIVES: The objective of this study was to synthesize indication-based evidence for N2O for distress and pain in children. STUDY DESIGN: We included trials of N2O in participants 0-21 years, reporting distress or pain for emergency department procedures. The primary outcome was procedural distress. Where meta-analysis was not possible, we used Tricco et al.'s classification of "neutral" (p ≥ 0.05), "favorable," or "unfavorable" (p < 0.05, supporting N2O or comparator, respectively). We used the Cochrane Collaboration's Risk of Bias tool and the Grading of Recommendations Assessment, Devel

Dynamic electrophysiological mechanism in patients with long-standing persistent atrial fibrillation.

BACKGROUND: Improved understanding of the mechanisms that sustain persistent and long-standing persistent atrial fibrillation (LSpAF) is essential for providing better ablation solutions. The findings of traditional catheter-based electrophysiological studies can be impacted by the sedation required for these procedures. This is not required in non-invasive body-surface mapping (ECGI). ECGI allows for multiple mappings in the same patient at different times. This would expose potential electrophysiological changes over time, such as the location and stability of extra-pulmonary vein drivers an

[Flexible endoscopic treatment for Zenker's diverticulum: preliminary results in a single center experience in a public hospital in Argentina.].

UNLABELLED: Zenker's diverticulum (ZD) is an acquired protrusion of the esophageal wall. Treatment is reservedfor symptomatic cases. In Argentina, ZD has historically managed exclusively by surgical intervention. In 1995, Ishioka, et al and Mulder, et a1published their experience using flexible endoscopes for diverticulotomy. Since then, a wide variety ofdifferent techniques have been published. Objetive. To present technique modifications and results ofour center. Motocross morbidity: economic cost and injury distribution in children.

BACKGROUND: Motocross is a nationally organized sport that is growing in popularity. The distribution and severity of motocross injuries in the pediatric population is not known. We hypothesize a high rate of musculoskeletal injuries requiring hospitalization and/or surgical intervention. METHODS: All patients 17 years of age or younger with injuries sustained while using off-road 2-wheeled motorcycles were identified through surgical, diagnostic, and trauma registries at a level 1 regional trauma center. Type, severity, and mechanism of injury were assessed, as well as charges billed for med

Postoperative nausea and vomiting. Its etiology, treatment, and prevention.

In a recent editorial, Kapur described perioperative nausea and vomiting as "the big 'little problem' following ambulatory surgery."257 Although the actual morbidity associated with nausea is relatively low in health outpatients, it should not be considered an unavoidable part of the perioperative experience. The availability of an emesis basin for every patient in the postanesthesia recovery unit is a reflection of the limited success with the available therapeutic techniques.257 There had been little change in the incidence of postoperative emesis since the introduction of halothane into cli

Comparison of Postoperative Analgesic Efficacy Between Opioid-free Anaesthesia Using Intravenous Magnesium, Lidocaine, and Erector Spinae Plane Block, and Opioid-based Anaesthesia in Laparoscopic Abdominal Surgeries: A Randomised Controlled Trial.

OBJECTIVE: Opioid-based anaesthesia is a standard practice in laparoscopic surgeries. However, it is associated with adverse effects that can limit its use. To address these issues, opioid-free anaesthesia has been introduced as an alternative. This study aims to compare the analgesic efficacy of opioid-free anaesthesia, incorporating magnesium, lidocaine, and an erector spinae plane block (ESPB), with traditional opioid-based anaesthesia during laparoscopic procedures. METHODS: Seventy patients aged 18-60 years undergoing laparoscopic surgery were randomly divided into two groups of 35 each.

Qualitative evaluation of a video-based tool to support informed consent in colonoscopy.

BACKGROUND: Endoscopy unit efficiency is critical to provide timely and quality care, despite limited resources. In previous work, obtaining informed consent was the most common documented cause of delay. We developed a 4-minute animated video to facilitate the consent process, describing colonoscopy, its purpose, and potential risks and benefits. AIMS: In this study, we aim to assess (1) the ability of the video to support the informed consent process and (2) the extent to which the video is attractive, clear, acceptable, actionable and usable, from the perspective of the audience. METHODS:

Reinforcement learning based automated anesthesia system for gastrointestinal endoscopy with a multicenter randomized trial.

The increasing demand for gastrointestinal endoscopic procedures, coupled with a global shortage of anesthesiologists, underscores the need for intelligent automation in anesthesia care. Reinforcement learning (RL) offers a promising strategy for autonomous anesthesia control, yet prospective clinical validation remains limited. We developed an RL-based automated anesthesia system for gastrointestinal endoscopy (AAS-GE) for automated ciprofol delivery and conducted a prospective, multicenter, randomized controlled trial across four centers in China between January 8 and August 27, 2025. Discharge After Transcatheter Aortic Valve Implantation: Observational Study of Self-Reported Experiences and Health Status in the Very Early Recovery Period.

BACKGROUND: Early discharge home is the standard of care after transcatheter aortic valve implantation (TAVI). Transition home can be challenging for this population. We lack evidence of patients' experiences during their immediate recovery to develop targeted and effective interventions. We aimed to describe patients' self-reported readiness; supports for early discharge; and very early experience of physical health status, activities of daily living, and procedure-related events after TAVI. METHODS: We conducted a prospective observational study at five Canadian hospitals; registered nurses
