Posterior perilunate carpal dislocation associated with a multifragmentary distal radius fracture.

Dorsal perilunate wrist dislocations are rare. Associated carpal bones or radial styloid process fractures can occur and be included in the current classifications. The authors report an unusual association of dorsal perilunate wrist dislocation with a multifragmentary distal radius fracture. Such traumatic entity has never been previously described. Poor functional outcome may justify the inclusion of associated complex forearm bone fractures in wrist dislocation classifications.

Clinics in diagnostic imaging (124). Multiple enchondromatosis in Ollier disease.

A three-year-old girl presented with slow-growing swellings at the left foot and upper right humerus. Radiographs show multiple enchondromas in both feet, proximal humeri and scapulae, as well as at multiple sites in the hands, distal forearm bones and pelvis, in keeping with multiple enchondromatosis in Ollier disease. The clinical presentation and imaging features of enchondromas and the different types of enchondromatosis are discussed.

Cold sensitivity as a new side effect after sympathicotomy for hyperhidrosis.

Extremity cold sensitivity is a previously unreported side effect of thoracoscopic sympathicotomy. We report a case of a young man who underwent thoracoscopic sympathicotomy for hyperhidrosis, after which cold sensitivity developed in his sympathicotomized right hand and arm. When exposed to cold, the right hand and forearm become numb and the skin temperature drops to uncomfortable levels, while the left extremity remains comfortably warm.

[Parathyroidectomy for advanced renal hyperparathyroidim in hemodialysis].

Treatment of advanced secondary hyperparathyroidism should be shifted from to avoid progression of bone disease to protection of cardiovascular complications induced by ectopic calcification. Patients who suffer from advanced secondary hyperparathyroidism with uncontrollable hypercalcemia or/and hyperphosphatemia by medical treatment should be referred to surgical treatment at relatively early time. Total parathyroidectomy with forearm autograft is adequate operative procedure especially in patients who require long-term hemodialysis.

Independent function in a split flexor carpi radialis transfer.

In a patient requiring tendon transfer after radial nerve palsy, the flexor carpi radialis, a bipennate muscle, was split longitudinally into 2 compartments along the length of the aponeurosis extending proximally from the distal tendon to provide independent finger and thumb extension. This case report shows that the 2 compartments of a bipennate muscle in the forearm may have separate innervation, allowing transfer for independent functions.

[Cervical spondylosis revealed after general anesthesia].

We present a case of complicated postoperative nerve injury. A 70-year-old man complained of partial paralysis and numbness of the left forearm 1.5 hour after operation for femoral artery bypass graft. Postoperative cervical spine MRI revealed cervical spondylotic radiculopathy. The MRI findings could account for his symptom. Not only peripheral nerve injury but the cervical radiculopathy may play an important role in perioperative neuropathy.

[Surgical treatment of children with brachial plexus paralysis].

A variety of surgical procedures exist for early repair of the nerve injury in obstetrical brachial plexus palsy, including neuroma excision and nerve grafting, neurolysis and neurotization. Secondary deformities of the shoulder, forearm, and hand can similarly be reconstructed using soft tissue and skeletal procedures. This review describes our surgical approach to maximize the ultimate functional outcome in infants and children with obstetrical brachial plexus palsy.

A personal experience.

David Bailey, who is 14 years old, has a left transverse total forearm deficiency and bilateral longitudinal femur partial deficiencies. The latter fall into Torode and Gillespie's Group I, with marked femoral shortening. A wedge osteotomy was performed for his gross right coxa vara, which was successful, but an intractable fixed flexion deformity of the right knee led eventually to a knee disarticulation being performed.

Microsurgical facial reconstruction of a xeroderma pigmentosum patient after skin tumor resection: case report.

Microsurgical reconstruction of the face completely destroyed after malignant skin tumor excisions in a xeroderma pigmentosum patient is reported. The patient's left cheek was reconstructed using a free latissimus dorsi flap and the nose using a free forearm flap with a base of iliac bone graft. There has been no recurrence of tumor at or surrounding the transferred flap site over two years postoperative follow-up.

Multiple skeletal familial abnormalities associated with balanced reciprocal translocation 2;8(q32;p13).

A father and three of his offspring had skeletal abnormalities consisting of a short forearm, cubitus valgus, fusion of first and second cervical vertebrae, and cleft of L5 and S1. All four had a reciprocal, apparently balanced, translocation 2;8(q32;p13). Normal sibs had normal chromosomes. We conclude that this may be a rare instance of an autosomal dominant condition associated with a balanced chromosome translocation.

When Time Is Not on Your Side: Delayed Recognition of Hand Compartment Syndrome After a Fracture.

Compartment syndrome of the forearm is a well-described clinical entity, but only a few case reports on hand compartment syndromes have been reported due to its rarity. The early recognition and treatment of this potential threat to the limb are important in order to prevent permanent disability and other life-threatening complications. This is a documented case report on the late presentation of compartment syndrome of the hand following a fall.

Convergent type proximal radioulnar translocation complicating posterior elbow dislocation: watch out for dual dislocations in children's elbows.

Proximal radioulnar translocation associated with posterior dislocation of the elbow is an extremely rare combination of trauma to the elbow in children. We report on the successful and sequential closed reduction of both these dislocations in a 10-year-old girl in which the translocation of the forearm bones was initially missed. A concomitant fracture would have probably precluded an ideal outcome as we note from the literature review on this problem.

Depot-glucagon in the treatment of McArdle's disease.

Glucagon has been reported to improve exercise tolerance in some patients with myophosphorylase deficiency (McArdle's disease) but a placebo effect was not excluded in previous trials. A single-blind controlled trial of glucagon in a patient with myophosphorylase deficiency is described. Although an improved exercise endurance of the forearm muscles was demonstrated with both short-acting and depot-glucagon, this was not statistically significant when compared with placebo or no treatment.

[Restorative operations using microsurgical technics in abruption of large segments of the upper limb].

Ten observations of replantations of the hand, forearm and shoulder after their traumatic abjunction are analyzed. Seven patients had complete reimplantation. Failures resulted from prolonged anoxia of the abjuncted segment or from extensive lesions to soft tissues of the replant. Success of the operation was mostly related with the quality of the vascular sutures (microtechnique), careful primary treatment of the wound and organization of the urgent surgical aid.

A Case of Recurrent Chromoblastomycosis Treated with Multiple Surgical Management Options.

Chromoblastomycosis is a chronic mycosis of the skin and subcutaneous tissue typically caused by traumatic inoculation of dematiaceous fungi of the Herpotrichiellaceae. A 59-year-old male presented with a 12-month history of an asymmetrical, scaly plaque on the left forearm that has been slowly increasing in size. Past medical history included atrial fibrillation on apixaban, hypertension and a cardiac stent. A 4 mm punch biopsy of the left forearm revealed superficial dermal fibrosis with mild pseudoepitheliomatous hyperplasia and granulomatous inflammation with scattered multinucleate histio…

An Approach to Using Electrical Impedance Myography Signal Sensors to Assess Morphofunctional Changes in Tissue during Muscle Contraction.

This present work is aimed at conducting fundamental and exploratory studies of the mechanisms of electrical impedance signal formation. This paper also considers morphofunctional changes in forearm tissues during the performance of basic hand actions. For this purpose, the existing research benches were modernized to conduct experiments of mapping forearm muscle activity by electrode systems on the basis of complexing the electrical impedance signals and electromyography signals and recording electrode systems' pressing force using force transducers. Studies were carried out with the involvem…

CCN1 expression is regulated by mechanical stimuli in tendons.

Tendon overuse injuries are common, but the processes that govern tendon response to mechanical load are not fully understood. A series of e … nical stimuli. Then, baseline Ccn1 gene expression in different murine tendons (Achilles, patellar, forearm, and tail) was examined. Finally, changes in Ccn1 expression after in vivo unloading experiments were examined. It was found that CCN1 expression significantly increased in both human and murine tenocytes at 5 and 10% cyclical uniaxial strain, while …

Curettage and external fixation for displaced pathological fracture through non-ossifying fibroma in the radius: A case report.

Non-ossifying fibromas are extremely rare in the upper extremity, such as those in the radius and humerus. The treatment of completely displaced fractures following non-ossifying fibromas in the radius has not been reported. We present the case of a pathological fracture caused by a non-ossifying fibroma in the radius treated using external fixation. The patient was a 10-year-old girl who presented to our hospital with right forearm pain after an accidental fall. She had no remarkable medical history. Radiographs and computed tomography showed a pathological fracture through osteolytic lesions…

Tuberculous flexor tenosynovitis of wrist and hand.

A woman in her 40s presented with a swelling over her left distal forearm and hand since 7 months, progressively increasing in size. She had history of difficulty in moving her wrist and fingers with no associated pain. She had no immune or chronic conditions except for hypothyroidism for which she was on regular medication. On examination, there was an 8×7 cm swelling on the radial side of the volar surface of her left distal forearm extending till the thenar eminence. MRI of the left upper limb was suggestive of a soft tissue swelling arising from the flexor tendon. The patient was planned fo…
