Comparison of primary analysis strategies of randomized controlled trials with multiple endpoints with application to kidney transplantation.

Relying on a single primary endpoint in randomized controlled trials (RCTs) is often infeasible, for example due to rare or heterogeneous events. Regulatory guidance therefore allows multiple endpoints, but different analytical strategies address different scientific questions and null hypotheses, even when applied to the same set of variables. We explored three approaches to consider multiple endpoints in the primary analysis of RCTs, as stated in the FDA and EMA guidelines on multiplicity: (i) a composite endpoint (CE), (ii) multiple testing and multiplicity correction (MTMC), and (iii) a hi

From challenge to innovation: a narrative review on innovation in nephrology and hemodialysis randomized trials.

Randomized controlled trials (RCTs) are the cornerstone of evidence-based medicine, yet their distribution across clinical disciplines is far from uniform. Nephrology and kidney failure are a striking example of fields where the number, quality, and applicability of trials remain suboptimal, despite the high disease burden and cost of care. The dearth of high-quality trials persists due to numerous systemic and practical barriers, including the unwillingness of patients and clinicians to participate, ethical constraints, logistical complexity, and financial limitations. These barriers are comp

Self-management strategies for menstrual cycle symptoms: a systematic review.

PURPOSE: Menstrual cycle symptoms, such as discomfort, fatigue, and mood swings, are prevalent among women and can significantly diminish quality of life. Many women employ non-clinical, self-management strategies to mitigate symptoms, yet the supporting evidence for these strategies is equivocal. This systematic review sought to identify, evaluate and synthesise existing evidence from randomised controlled trials (RCTs) for non-clinical, self-management strategies for menstrual cycle symptoms. METHODS: A systematic literature search was performed in PubMed, Scopus, Web of Science, and Cochra

A Comprehensive Scientometric Analysis of Placebo and Nocebo Effects in Pain Research: From Placebo Mechanisms to Clinical Trials.

Placebo and nocebo phenomena are central in neuroscience, psychology, and clinical research, yet comprehensive bibliometric analyses remain limited. This study mapped placebo and nocebo research in pain, distinguishing mechanistic studies from randomised controlled trials (RCTs), using data from the Web of Science Core Collection up to March 2025, which yielded 1246 mechanistic studies and 35,134 RCTs. Analyses performed with the Bibliometrix R package included descriptive statistics, co-citation and keyword co-occurrence networks, and local citations to assess internal impact. Mechanistic stu

Intranasal oxytocin for alcohol use disorder: A systematic review and multilevel, bayesian, and variance meta-analyses of randomized clinical trial data.

BACKGROUND: Intranasal oxytocin (OT) has been proposed as a promising adjunctive treatment for Alcohol Use Disorder (AUD), yet randomized controlled trials (RCTs) have yielded mixed and inconclusive findings. To clarify its therapeutic potential, we conducted a comprehensive meta-analysis using frequentist, Bayesian, and variability-based approaches. METHODS: We performed a multilevel random-effects meta-analysis of six eligible RCTs comparing intranasal OT with placebo for alcohol-related outcomes. Hedges' g values were calculated and winsorised at |g| = 3 to limit leverage from extreme smal

Efficacy of transcranial alternating current stimulation on cognitive function in patients with dementia: a systematic review and meta-analysis of randomized controlled trials.

BACKGROUND: Transcranial alternating current stimulation (tACS) has shown potential for improving cognitive function in dementia, but evidence remains inconsistent. This systematic review and meta-analysis aims to evaluate the efficacy of tACS on cognitive function in dementia. METHODS: We searched seven databases for randomized controlled trials (RCTs) assessing tACS effects on cognitive function in dementia. Two reviewers independently screened the literature, extracted data, and assessed study quality. A random-effects model was used to calculate standardized mean differences (SMDs) with 9

Routine abdominal drainage versus no drainage after elective hepatectomy: A systematic review and meta-analysis.

INTRODUCTION: The value of routine abdominal drainage after hepatectomy remains controversial. Although drains have traditionally been used to detect bile leakage or postoperative bleeding and to evacuate intra-abdominal collections, their routine use after elective liver resection has increasingly been questioned. MATERIALS AND METHODS: A systematic review and metaanalysis of randomized controlled trials comparing routine abdominal drainage with no drainage after elective hepatectomy was performed. PubMed/MEDLINE, Embase, Web of Science, and the Cochrane Library were searched from inception

Barriers and Strategies for Recruiting Care Partners During the Hospitalization of People Living with Dementia: Lessons from a Hospital-Based Randomized Controlled Trial.

Background: Recruiting care partners (CPs) of hospitalized people living with dementia is challenging due to rapid discharge timelines and complex inpatient workflows. This study identified barriers to CP recruitment encountered during the first year of a hospital-based randomized controlled trial and the strategies implemented in response to them. Methods: A qualitative descriptive study using thematic analysis of twelve study coordination and implementation meeting transcripts was conducted. Recruitment outcomes were examined to assess changes before and after implementation of the enhanced

SPIRIT-Children and Adolescents (SPIRIT-C) 2026 extension statement: enhancing the reporting and usefulness of paediatric randomised trial protocols.

Randomised controlled trial (RCT) protocols contain essential details needed to understand and evaluate the trial's planned aims, design, data collection methods, monitoring, data analysis, and participants' safety. However, key information is often omitted from paediatric RCT protocols, including details on dose adjustments of interventions based on age, body surface area, or weight; developmental appropriateness of trial outcome measures and processes; or strategies to minimise participants' anxiety and pain. These deficiencies impair the planning and implementation of potentially impactful

Effectiveness of motor imagery techniques in the treatment of chronic musculoskeletal pain: A scoping review.

Recent neurophysiological studies provide evidence that motor imagery techniques, as a non-pharmacological intervention, reduce the perception of pain intensity at the cortical level and can therefore be used in clinical practice in patients with chronic musculoskeletal pain (CMP). The aim of this scoping review was to evaluate recent randomised controlled trials on the effect of motor imagery techniques for the relief of CMP. Four databases (PubMed, Scopus, Proquest, Cochrane Clinical Trials) were searched to identify randomised controlled trials on the effect of motor imagery techniques on t

Trustworthiness and transparency features were less frequent in randomized trials presenting large effects for continuous outcomes in abstracts.

OBJECTIVES: Large effect sizes (ESs), especially when prominently presented in trial abstracts, draw large attention, but it is important to understand whether they are trustworthy. We aimed to assess indicators of transparency and trustworthiness in randomized controlled trials (RCTs) reporting some large ES for continuous outcomes in their abstract, in comparison with RCTs presenting only nonlarge ESs in their abstract. STUDY DESIGN AND SETTING: We included RCTs indexed in MEDLINE between January 1, 2024 and March 18, 2025, presenting at least one standardized mean differences of absolute v

A randomized controlled trial of metastasis-directed therapy for oligometastases in breast cancer: OLIGAMI trial (JCOG2110).

Oligometastasis is a concept that refers to the condition between localized cancer and widespread distant metastases; however, a consensus on the definition has yet to be reached. Metastasis-directed therapy (MDT) has recently been established to incorporate local therapy, such as radiotherapy and surgery, for distant metastases. Previous trials have proposed the possibility of long-term survival with the administration of MDT for patients with oligometastases. However, the efficacy and safety of MDT have not been sufficiently validated. We are conducting this clinical trial to confirm the sup

A critical analysis of the methodological limitations in trials of low-intensity shockwave therapy for erectile dysfunction.

INTRODUCTION: Low-intensity shockwave therapy (LiSWT) is a novel treatment option for patients with erectile dysfunction (ED). To date, numerous randomized controlled trials (RCTs) and cohort studies have been published. OBJECTIVES: To critically evaluate the methodology and limitations of the published studies on LiSWT for ED. METHODS: A literature search was performed. We excluded studies that compared and/or combined LiSWT to other therapies. In the context of multiple studies from the same center with identical populations and study design, we selected the studies with the largest patien
