Procedure and step-based analysis of the occupational radiation dose during endovascular aneurysm repair in the hybrid operating room.

OBJECTIVE: This study measured the cumulative occupational X-ray radiation dose received by support staff during endovascular aortic procedures and during additional intraoperative steps in the hybrid operating room. METHODS: Radiation dose measurements were performed during interventions on 65 patients receiving 90 stent grafts during endovascular aneurysm repair (EVAR), bifurcated EVAR, thoracic EVAR, iliac branched device deployment, aortouni-iliac stenting, and fenestrated/branched EVAR (F/BrEVAR). X-ray imaging was acquired using the Philips Allura FD20 Clarity System (Philips Medical Sy

Percutaneous deep venous arterialization in patients with critical limb ischemia.

BACKGROUND: Critical limb ischemia (CLI) is the presentation of end stage peripheral arterial disease and typically presents with rest pain, ulceration and gangrene. The outcome of conservative treatment is poor and often leads to amputations. Arterial revascularization plays an important role in amputation prevention. Unfortunately, a significant percentage of CLI patients cannot be revascularized and subsequently end up with a palliative amputation. This has led to the need and exploration of new treatment options in this no option subgroup of CLI. Deep venous arterialization (DVA) is one of

Distinguishing Chronic Thromboembolic Pulmonary Hypertension From Other Causes of Pulmonary Hypertension Using CT.

OBJECTIVE: The purpose of this study was to discern imaging findings that separate chronic thromboembolic pulmonary hypertension (CTEPH) from other causes of pulmonary hypertension (PH). MATERIALS AND METHODS: A total of 143 patients with proven PH (group 1, pulmonary arterial hypertension; group 2, PH due to left heart disease; group 3, PH due to lung disease; group 4, CTEPH; and group 5, PH due to unclear or multifactorial mechanisms) underwent MDCT angiography. The CT images were assessed for the presence of chronic pulmonary embolism (PE), disparity in segmental vessel size, mosaic perfus

Transnasal Penetration of a Ballpoint Pen: Case Report and Review of Literature.

BACKGROUND: Transnasal penetration by a nonmissile foreign body is a rare injury. Consequently, appropriate management remains controversial. We report a case of transnasal penetration by a ballpoint pen and review the literature. To our knowledge, this is the first living patient who sustained carotid artery damage from a transnasal penetrating intracranial injury. CASE DESCRIPTION: A 56-year-old female presented with a ballpoint pen lodged through her left nostril. She exhibited right cranial nerve palsies (III, IV, VI, and V1). A computed tomography (CT) scan of the head revealed a foreign

Combined transmesenteric and transhepatic recanalization of chronic portal and mesenteric vein occlusion to treat bleeding duodenal varices.

Two patients presented with bleeding duodenal varices secondary to mesenteric and portal vein chronic occlusion. After a failed transhepatic recanalization, a combined transmesenteric and transhepatic approach was used to recanalize the chronic portal and mesenteric venous obstruction. The occluded segment was treated with transmesenteric stent placement in one patient and stent placement and coil embolization of varices in the second patient. Follow-up imaging and endoscopy showed decompression of the duodenal varices in both patients and absence of further bleeding episodes.

Alternative treatment for bleeding peristomal varices: percutaneous parastomal embolization.

PURPOSE: To describe how peristomal varices can be successfully embolized via a percutaneous parastomal approach. METHODS: The medical records of patients who underwent this procedure between December 1, 2000, and May 31, 2008, were retrospectively reviewed. Procedural details were recorded. Median fluoroscopy time and bleeding-free interval were calculated. RESULTS: Seven patients underwent eight parastomal embolizations. The technical success rate was 88 % (one failure). All embolizations were performed with coils combined with a sclerosant, another embolizing agent, or both. Of the seven

Endovascular repair of external carotid artery disease.

PURPOSE: To review the literature on the endovascular management of external carotid artery (ECA) disease. METHODS: A review was conducted of the English-language medical literature from 1985 to 2011 using PubMed, OVID, and EMBASE databases to find all studies involving endovascular management of ECA stenosis. The search identified 21 reports describing endovascular repair of ECA lesions encompassing 56 patients (35 men; mean age 62.6±10.5 years) and 58 arteries. RESULTS: The majority of patients (91.1%) were reported to have ipsilateral ICA occlusion, while 55.6% of patients were symptomati

Perioperative approach in the surgical management of carotid body tumors.

BACKGROUND: Now, surgical resection still remains the gold standard for the treatment of carotid body tumors (CBTs). Although advances in surgical techniques and the introduction of sensitive imaging modalities have significantly reduced mortality, the incidence of perioperative neurovascular complications, especially cranial nerve deficit and intraoperative hemorrhage, remains considerable. To solve these problems, preoperative embolization has been suggested; the reported benefits of preoperative embolization performed <48 hours before surgery include a reduction in tumor size, decreased blo

Upper extremity ischemia treated with tissue repair cells from adult bone marrow.

BACKGROUND: Unreconstructable critical ischemia with gangrene of the upper extremity is rarely due to atherosclerosis alone, and few treatment options exist. We describe a patient with gangrene of both hands as a result of unreconstructable atherosclerotic disease of both upper extremities who was successfully treated with tissue repair cells (TRCs) produced from the patient's bone marrow. METHODS: A patient with type 1 diabetes was referred with bilateral upper extremity digital gangrene due to unreconstructable forearm and hand atherosclerosis. He was evaluated for therapeutic angiogenesis

Definitive test to diagnose median arcuate ligament syndrome: injection of vasodilator during angiography.

The diagnosis of median arcuate ligament (MAL) syndrome and its correlation with symptoms has been controversial since the disease entity was described. The authors describe a technique that will identify patients who will benefit from intervention. Eight patients with isolated celiac artery compression from MAL were identified by the authors. Their technique involved selective cannulation of the superior mesenteric artery (SMA) and injection of a vasodilator during angiography. Symptom reproduction and loss of collateral filling of the celiac territory represented a positive test: 4 of the 8

[Guidelines of the German Radiological Society for percutaneous vertebroplasty].

In many countries, percutaneous vertebroplasty became a widely accepted therapeutic option in osteoporotic and neoplastic fractures of the vertebral bodies which is employed in more and more patients. Under image guidance the method can be safely performed and provides a high success rate regarding stabilization of vertebral fractures and pain relief. Due to the increasing interest in vertebroplasty in Germany the German Radiological Society-Working Group on Interventional Radiology decided to produce guidelines for percutaneous vertebroplasty. This guidelines are based on the recent standard

Surgical treatment for aortic regurgitation caused by Takayasu's arteritis.

Aortic regurgitation (AR) caused by Takayasu's arteritis is relatively rare. We investigated the surgical results in patients with AR caused by Takayasu's arteritis. Between 1978 and December 1997, 65 patients (5 males, 60 females) with AR secondary to Takayasu's arteritis underwent surgery. The patients' ages ranged from 19 to 70 years (mean, 48 years). Takayasu's arteritis was diagnosed by clinical examination confirmed by intraoperative pathological examination of the aortic wall. The surgical procedures for AR were aortic valve replacement in 45 patients, Bentall-type operation in 19, and

The varied manifestation of pulmonary artery agenesis in adulthood.

OBJECTIVE: Unilateral pulmonary artery agenesis (UPAA), a rare congenital anomaly that is frequently associated with other cardiovascular abnormalities, is usually diagnosed in childhood. Most patients who have no associated cardiac anomalies have only minor or absent symptoms and survive into adulthood. The conditions of such patients are frequently misdiagnosed in adulthood. In this report, we describe six patients with UPAA in whom the diagnosis was first established in adulthood. The varied clinical presentation of these patients is reviewed and the relative effectiveness of a variety of d

The anatomy and clinical importance of the collateral circle between the external carotid arteries through an anastomosis between the superior thyroid arteries.

Three cases of obstruction of a single common carotid artery were incidentally discovered by Doppler ultrasonography. Interestingly, anterograde blood flow was demonstrable in the ipsilateral internal carotid artery (ICA), even though it was distal to the obstruction Anterograde blood flow was also observed in the intracranial portion of the internal carotid artery and in its branches that were explored (the ophthalmic and middle cerebral arteries). The blood flow was comparatively reduced and irregular with respect to that of the corresponding contralateral artery, but nonetheless anterograde

Coronary steal syndrome.

A patient who suffered recurrent myocardial ischaemia and cerebrovascular symptoms 56 months after a quadruple coronary bypass is reported. Three coronary arteries had been bypassed using reversed saphenous vein and the other using the left internal mammary artery (IMA). Coronary angiography demonstrated patency of al bypasses but the presence of an obstruction of the left subclavian artery proximal to the origin of the left IMA, with angiographic criteria of the steal syndrome. The patient's symptoms were relieved by bypass from the left common carotid artery to the distal left subclavian art
