[PECULIARITIES OF DIAGNOSIS AND SURGICAL TREATMENT OF PATIENTS, SUFFERING ATYPICAL FORMS OF AN ACUTE APPENDICITIS].

Results of diagnosis and surgical treatment of 852 patients, suffering an acute appendicitis (AA) with atypical course, were analyzed. Retrocecal localization of appendix was noted in 61.2% of observations, a pelvic one — in 24.3%, medial — in 11.2%, and subhepatic — in 3.4%. Destructive forms of atypical AA were diagnosed in 92.5% patients, and various kinds of peritonitis — in 77.7%. Some diagnostic (rectal thermometry, test with ethanol) and operative methods (including laparoscopic) in destructive forms of AA, complicated by typhlitis, were improved and tested. Diagnostic—treatment algorit

Predictive value of abnormally raised serum bilirubin in acute appendicitis: a cohort study.

UNLABELLED: Appendicitis is a common clinical diagnosis aided by biochemical, haematological and radiological investigations. The role of some investigations, such as bilirubin, is controversial but could indicate complicated appendicitis. Accurate diagnosis enables prioritisation of patients on operating lists and a possible reduction in unnecessary investigations. METHODS: This is a retrospective cohort study of 1347 patients who underwent appendicectomy. Statistical analysis of serum bilirubin levels was performed according to histological classification of appendicitis. RESULTS: Mean ser

Sacro-iliac osteomyelitis in a 13 year old boy following perforated appendicitis.

Appendicitis is a common cause of acute abdominal pain in children and is treated by an open or laparoscopic appendicectomy. Well documented post-operative complications include wound infection, intra-abdominal collection, and adhesional bowel obstruction. We present the rare case of right sacro-iliitis and iliac bone osteomyelitis in a 13 year old boy following an open appendicectomy for a perforated appendicitis.

Diagnostic laparoscopy for chronic right iliac fossa pain: a pilot study.

BACKGROUND: The aim of this study was to determine the value of diagnostic laparoscopy in patients with chronic right iliac fossa pain. METHODS: A retrospective study at Echuca Hospital involving case-note review and telephone questionnaire of patients who had undergone diagnostic laparoscopy for chronic right iliac fossa pain at least 12 months earlier (September 1992 to August 1995) was carried out. RESULTS: Forty-one cases were identified and followed up 12-40 months postoperatively (median 21 months). Eleven cases had positive findings at laparoscopy, of whom eight obtained lasting relie

Peritoneal cultures and antibiotic therapy in pediatric perforated appendicitis.

We retrospectively reviewed 70 cases of perforated appendicitis in children to examine the relationship between postoperative antibiotic selection and culture results from the peritoneal cavity. Initial antibiotic therapy chosen for the children consisted of a three-drug combination in 54 (77%) patients. Peritoneal cultures were performed in 58 (83%) patients. Escherichia coli and Bacteriodes fragilis were the most common bacterial isolates from the peritoneal culture. Only 7 (10%) patients had their antibiotic regimen changed after the culture results were available, of which 2 changes brough

Appendicitis in cystic fibrosis.

Appendicitis is said to be uncommon and difficult to diagnose in cystic fibrosis. The clinical and radiological features in nine patients with cystic fibrosis who had appendicitis were studied. All but one of the patients had an appendiceal abscess at surgery. Four patients had a delay in diagnosis of greater than three days before the correct diagnosis was made. This delay may have been due to a more indolent presentation or because these patients were initially considered to have distal intestinal obstruction syndrome. Appendicitis should be considered in the differential diagnosis if a cont

Imipenem therapy for perforated and gangrenous appendicitis.

A matched patient control study of imipenem therapy of patients with perforated or gangrenous appendicitis was performed. Thirty-three patients treated with imipenem and cilastatin were compared with 66 control patients treated with clindamycin and gentamicin. Patients were matched for age and surgical pathologic factors. Twenty-five of the 33 imipenem treated patients had perforated (two with abscesses) and nine had gangrenous appendicitis. Anaerobes were recovered from 23 of 32 (72 per cent), anerobes from 31 of 32 (97 per cent) and Pseudomonas from six (26 per cent) of the 23 with perforate

[Laparostomy in children].

Laparotomy means leaving the abdomen open after a laparotomy. This enables the surgeon to revise the abdominal cavity regularly and to check the efficacy of the drainage without repeated damages to the abdominal wall. In adults, this method is used in cases of abdominal pathology with necrotic or infectious pathogenesis. We describe the method of laparotomy in 3 cases of paediatric surgery. It was used in cases of postoperative peritonitis and we think that laparotomy may be used in these situations, especially because there are almost no sequelae.

Pediatric appendicitis. A 20-year study of 1,640 children at Cook County (Illinois) Hospital.

Of 1,640 children with acute appendicitis treated at Cook County (Illinois) Hospital between Jan 1, 1957, and Dec 31, 1976, 35% had appendiceal perforation. Overall morbidity was 12.8% and mortality was 0.24%. Antibiotics, transperitoneal drainage, and delayed wound closure were used routinely in children with appendiceal perforation. Antibiotics and transperitoneal drains did not appreciably alter the incidence of intraabdominal abscess formation. Delayed wound closure in patients with appendiceal perforation reduced the incidence of wound infection by 75%.

One year negative appendicectomy rates at a district general hospital: A Retrospective Cohort Study.

INTRODUCTION: There is no defined 'acceptable' Negative Appendicectomy Rate (NAR) in the UK. Despite advances in radiology and predictive scoring systems, appendicitis remains a clinical diagnosis but inevitably some patients will have an entirely normal appendix removed. We sought to define our local practice and results. METHODS: A one year retrospective observational study was performed in our institution on all appendicectomies performed on an emergency basis. Cases were identified with the hospital electronic theatre record system and histopathology reports were retrieved and analysed.

de Garengeot hernia: a unique surgical finding.

Femoral hernias occur in about 3% of all abdominal wall hernias. Herniation of the appendix through a femoral hernia (called de Garengeot hernia) is a much rarer finding, occurring in 0.8% to 1% of all cases. This unique surgical finding is most often found intraoperatively due to the similarities it shares with an irreducible inguinal or femoral hernia that requires emergent surgical intervention. When evaluating a patient with a persistent groin lump, consider de Garengeot hernia in the differential diagnosis.

Too attractive: the growing problem of magnet ingestions in children.

BACKGROUND: Small, powerful magnets are increasingly available in toys and other products and pose a health risk. Small spherical neodymium magnets marketed since 2008 are of particular concern. OBJECTIVE: The objective of this study was to determine the incidence, characteristics, and management of single and multiple-magnet ingestions over time. METHODS: Magnet ingestion cases at a tertiary children's hospital were identified using radiology reports from June 2002 to December 2012. Cases were verified by chart and imaging review. Relative risk regressions were used to determine changes in

Children with spinal dysraphism: transversus abdominis plane (TAP) catheters to the rescue!

Transversus abdominis plane (TAP) block catheters may offer an alternative to a central neuraxial blockade for patients undergoing major abdominal surgery (Paediatr Anaesth 19: 2009; 296). This case series consists of two children with spinal dysraphism in whom bilateral TAP block catheters were placed for postoperative pain relief. Administration of low dose, low concentration local anesthetic provided both children with excellent postoperative analgesia.

Appendiceal mucinous adenocarcinoma presenting as an enterocutaneous fistula in an incisional hernia.

A 68-year-old woman with a history of bone-graft harvesting from the right iliac crest presented with an incisional hernia and abscess at the graft donor site. Following incision and drainage of the abscess, CT demonstrated an enterocutaneous fistula between the appendix and bone-graft incision with appendicitis assumed to be the original cause of the abscess. At laparoscopy, the appendix was adherent to the hernia sac with mucinous material at the superficial orifice of the fistula site but not in the peritoneal cavity. Laparoscopic appendicectomy with fistula track excision was performed. Hi

[The role of laparoscopy in the diagnosis and treatment of early postoperative complications].

Results of 276 laparoscopic operations performed for diagnostic and therapeutic purposes in the early postoperative period in 198 patients are analyzed. All the patients had been previously operated on the organs of the abdominal cavity and retroperitoneal space: in 131 patients the investigations were performed with a diagnostic purpose; in 67 patients a laparoscopic method was used for treatment of complications of the early postoperative period. This problem can be solved by using the methods allowing diagnosing the complication as early as possible, before the development of irreversible a
