Outcome measures after medial ulnar collateral ligament reconstructions in a military population.

BACKGROUND: Medial ulnar collateral ligament (MUCL) reconstruction outcomes are well described in competitive throwers but not in nonthrowers. This investigation elucidated epidemiologic variables, functional outcomes, and prognostic factors after MUCL reconstruction in young active patients. METHODS: United States military service members undergoing MUCL reconstruction were isolated using the Management Analysis and Reporting Tool (M2) database from 2009 to 2016. Demographics, injury characteristics, and surgical variables were extracted. Multivariate analysis was performed, discerning varia

The role of cultured Schwann cell grafts in the repair of gaps within the peripheral nervous system of primates.

With recent advances in cell culture techniques it is possible to isolate human SCs from adult peripheral nerves, expand and purify their number in cell culture, and construct a cellular prosthesis from the cultured cells. The current study was designed to ascertain whether these techniques could be used to repair nonhuman primate nerve injuries. In 12 adult female cynomologous monkeys, the musculocutaneous (msk) nerve was divided and prevented from regenerating and the brachioradialis nerve (brach) was exposed bilaterally (n = 24 nerves) and injured so that a 15-mm gap existed within the nerv

Technical Trick: Coronoid Fracture "Lasso" Repair Using Arthroscopic Instrumentation in Terrible Triad Injuries With Fixable Radial Head Fractures.

A single-stage operative repair of terrible triad injuries through a laterally-based approach using arthroscopic instrument-assisted reduction of the coronoid fracture in cases in which the radial head is deemed appropriate for repair rather than arthroplasty is described in this technical trick. Using an arthroscopic suture lasso, adjustable drill guides, cannulated guide-pins with nitinol shuttling wires, and a suspensory cortical button allow for a more precise and facile technique of "lasso" fixation of coronoid fractures and anterior capsular injuries with intact radial heads in terrible

Osteochondritis Dissecans of the Capitellum: Influence of Activity on Lesion Location Within a Zone of Propensity.

BACKGROUND: There may be an association between repetitive stress on the elbow and capitellar osteochondritis dissecans (COCD). The influence of activity on the characteristics or specific location of the lesion within a capitellar zone of propensity remains unclear. PURPOSE/HYPOTHESIS: The purpose of this study was to evaluate COCD in gymnasts and baseball players compared with a non-upper extremity (non-UE) sport COCD cohort. It was hypothesized that activity would influence presentation and lesion location and characteristics. STUDY DESIGN: Cross-sectional study, Level of evidence, 3. ME

Surgical Outcomes, Trends, and Risk Factors of Distal Triceps Repairs.

BACKGROUND: Distal triceps ruptures are rare, and complete ruptures are commonly treated with surgery. Studies of patients in small cohorts with distal triceps tear have reported outcomes and risk factors; however, large-scale data are scant. This study seeks to determine current trends, outcomes, and risk factors of distal triceps tears. METHODS: Within a large insurance claims database, distal triceps repair patients were identified through Current Procedural Terminology coding with concomitant distal triceps International Classification of Diseases, 9th Revision/10th Revision diagnosis cod

Cadaveric investigation on radial nerve strain using different posterior surgical exposures for extraarticular distal humeral ORIF: merits of nerve decompression through a lateral paratricipital exposure.

OBJECTIVE: To determine whether the type of posterior surgical approach for distal humeral fracture open reduction and internal fixation influenced radial nerve strain during simulated operative retraction in a cadaveric model. METHODS: Three different posterior surgical exposures: triceps splitting, lateral paratricipital, and paratricipital with release of the lateral intermuscular septum were used. Radial nerve strain was measured using a microDVRT, while traction was applied with a digital force gauge at forces 0.1-0.3 kg. RESULTS: The lateral paratricipital with nerve decompression was

Deficiencies in pitching biomechanics in baseball players with a history of superior labrum anterior-posterior repair.

BACKGROUND: Baseball pitchers who undergo superior labrum anterior-posterior (SLAP) repair often have trouble returning to their previous level of performance. While the reason is often assumed to be diminished shoulder range of motion or other mechanical changes, differences in pitching biomechanics between baseball pitchers with a history of SLAP repair and pitchers with no injury history have not been studied previously. HYPOTHESIS: The primary hypothesis was that compared with the control group, the SLAP group would exhibit compromised shoulder range of motion (external rotation and horiz

Repaired distal biceps magnetic resonance imaging anatomy compared with outcome.

BACKGROUND: This study examined the magnetic resonance imaging (MRI) appearance of an anterior incision distal biceps tendon repair and evaluated the association between appearance and outcome. MATERIALS AND METHODS: Nineteen patients were randomly recruited to undergo an elbow MRI from a single-surgeon series of distal biceps repairs using an anterior approach. Tendon healing was evaluated by the integrity of the repair, the amount of heterogeneity within the tendon substance, and the presence of heterotopic bone. The angle of tendon insertion on the tuberosity was used to quantify the tendo

Are psychosocial factors, risk factors for symptoms and signs of the shoulder, elbow, or hand/wrist?: A review of the epidemiological literature.

BACKGROUND: In 1993, an extensive review on the role of psychosocial factors in the development of musculoskeletal problems was published by Bongers et al (1993). Since then, additional reviews on this topic have been published; however, none of these focussed on upper limb problems. METHODS: In this systematic review, the methodological quality of all studies was assessed and levels of evidence were apriori defined. RESULTS: The large majority of the studies reported an association between at least one work-related psychosocial factor and adverse upper extremity symptoms or signs. High-perc

[Use of pedicled radial forearm flaps for covering of extensive soft tissue defects in the upper extremity].

Results of treatment of 13 patients with extensive defects of soft tissues in the upper extremities treated with pedicled radial forearm flaps are presented (11 fascio-cutaneous flaps and 2 adiposo-cutaneous). The group included 5 females and 8 males aged 9-85 years (mean age: 37 years). The defects followed trauma in 12 cases and in one case it was the result of oncological resection. The flap was used as a primary procedure in 4 cases after amputation of the finger or degloving of the hand. The flap appeared very useful in delayed reconstructions in older patients with extensive defects of s

Severe fractures of the distal radius: effect of amount and duration of external fixator distraction on outcome.

Although severe intra-articular fractures of the distal radius are often treated with external fixation/distraction, little attention has been devoted to the amount and duration of fixator distraction required to maximize outcome. To determine these effects, we conducted a retrospective study of 26 patients whose primary treatment was external fixation/distraction. Patients were evaluated by chart review, questionnaire, x-ray films, and physical examination an average of 104 weeks after injury. The carpal height index was used to quantify distraction. Scores for pain, function, wrist motion, a

MR imaging of the elbow.

Of 27 patients who underwent magnetic resonance (MR) imaging of the elbow, 11 underwent elbow arthroscopy and/or an open surgical procedure. Surgical findings were compared with those from MR imaging. Five healthy volunteers also underwent MR imaging to demonstrate anatomic relationships. Transchondral fracture (osteochondritis dissecans) was identified in three of the 11 patients and was proved at surgery. Loose bodies were suspected at MR imaging in the three patients but were found in only two. One complete avulsion of the ulnar collateral ligament (UCL) and four cases of intact, thickened

Operative management of intra-articular fractures of the distal humerus.

Over a 5-year period (1979-1984), 33 patients with intra-articular fractures of the distal humerus underwent open reduction and internal fixation of their fractures. Twenty-two (67%) of the patients had multiple injuries. Fourteen (42%) of the fractures were open (1 was Grade I, 8 were Grade II and 5 were Grade III). The mean age was 32 years (range: 15-61 years), which is significantly younger than in other series. A subgroup of 25 of the patients was evaluated clinically at a mean of 18.3 months postoperatively. "Excellent" and "good" functional results occurred in 23 patients (92%). "Poor"

Epidemiology of Medial Ulnar Collateral Ligament Reconstruction: A 10-Year Study in New York State.

BACKGROUND: Despite an increase in the prevalence of medial ulnar collateral ligament (UCL) reconstruction of the elbow in professional baseball and popularity within the media, there are no population-based studies examining the incidence of UCL reconstruction. PURPOSE: To examine the epidemiological trends of UCL reconstruction on a statewide level over a 10-year period. The primary endpoint was the yearly rate of UCL reconstruction over time; secondary endpoints included patient demographics, institution volumes, and concomitant procedures on the ulnar nerve. STUDY DESIGN: Descriptive epi

Simulation of distal tendon transfer of the biceps brachii and the brachialis muscles.

Rupture of the distal tendons of the biceps brachii and the brachialis often consists of a clean avulsion of the end of the tendons from their tuberosities. In most of the reattachment procedures these tendons are reinserted to the same tuberosities. The purpose of this study was to examine the kinetic activity in the upper limb when the insertion locations of the two prime elbow flexors are altered. The right upper limb was modeled as a two-bar linkage moving in the vertical plane of the scapula. Our Hill-type musculo-tendon actuation system was modeled in terms of five muscles moving in thre
