Evidence from a recent cadaveric study has revealed that the thickness of the distal interosseous membrane (DIOM) of the forearm shows substantial individual variations, and a thick fibrous tissue known as the distal oblique bundle (DOB) was found in 40% specimens in the study. Every third child sustains a fracture before adulthood. Forearm fractures comprise one third of all fractures and their incidence has significantly increased during the past decades. Hand and foot fractures are also common but their number has decreased. A small proportion of children sustain multiple long-bone fractures or vertebral fractures. They should be referred to a pediatric evaluation to detect and treat potential underlying bone fragility.

We present a case of Ehlers-Danlos Syndrome (EDS) type IV with associated neuromuscular manifestations. The patient presented with nonspecific myopathic-type symptoms and later developed a spontaneous median mononeuropathy in the forearm. We believe that the case presented here complements current published neuromuscular manifestations occurring in EDS IV patients and illustrates the urgency of making the diagnosis, given the increased mortality associated with EDS IV.

The forearm distal bone fracture is the most com-mon fracture in children. Scaphoid frac-ture is rare because the scaphoid is largely cartilaginous in children. Some cases associating both distal radius and scaphoid fractures were reported in the literature. Scaphoid fracture often occurs without displacement and can be easily overlooked. We reported a case of children wrist injury combining scaphoid fracture with ipsilateral distal fracture.

We present a case of a 2-year-old girl who had a lawn mower accident with subtotal gut evisceration, multiple ischemic intestinal lesions, hepatic and gastric wounds, amputation of the left forearm, and hypovolemic shock. Prompt and adequate management was carried out in tertiary level institution, based upon quick evaluation of the lesions, fluid resuscitation, surgical repair, and postoperative admission to the pediatric intensive care unit.

This technical report discusses a subgroup of rheumatoid patients who have minimal ulnar drift but a severe fixed metacarpophalangeal joint flexion contracture for whom conventional metacarpophalangeal joint arthroplasty alone was insufficient to correct the deformity. We describe a surgical technique to deal with this clinical problem that uses fractional flexor tendon lengthening in the forearm to correct the severe flexion deformity at the metacarpophalangeal joint.

A case of bilateral forearm localization of multiple hereditary osteochondromata and unilateral Kienböck's disease is reported. Ulnar minus variance is frequent in both diseases. Carpal slip is often found in multiple hereditary osteochondromata. In this case, the extremity having both multiple hereditary osteochondromata and Kienböck's disease had no carpal slip. This might have produced an excess load on the lunate, which might have provoked Kienböck's disease.

A 40-year-old patient presented with traumatic amputation of the thumb and index finger. The second metacarpal bone fragment was transposed to lengthen the first metacarpal bone. A free flap from the lateral forearm was used to reconstruct the thenar region. Extravasation of vesicant agents into the soft tissues can lead to painful and indolent ulcerations. Precautions for avoiding extravasation include the use of fresh intravenous lines. The line should be placed in the forearm whenever possible. Hand veins, joint surfaces and extremities with impaired circulation should be avoided as infusion sites. If evidence of extravasation appears, the infusion must be stopped immediately and appropriate measures initiated.
