Pre- and postmatch hop test outcomes in soccer players returning to performance after lower extremity injury.

BACKGROUND: Most soccer injuries concern the lower extremity with a higher injury rate during the second half of matches. In advising safe return to sport, hop tests are usually assessed at the point of return to sport under non-fatigued conditions. No studies exist investigating hop test outcomes before and after a match in soccer players returning to performance after lower extremity injury and non-injured teammates. The objective is to assess differences in hop test outcomes before and after a match in and between soccer players returning to performance after lower extremity injury and thei

Insidious intraoperative ureteral injury as a complication in oblique lumbar interbody fusion surgery: a case report.

BACKGROUND: Oblique lumbar interbody fusion surgery is a recently introduced minimally invasive lateral interbody fusion surgery for degenerative lumbar disease. There have been no reports of associated ureteral injury. CASE PRESENTATION: A 77-year-old Japanese woman underwent oblique lumbar interbody fusion surgery for lumbar spondylolisthesis with refractory low back pain and pain in both legs. The patient experienced abdominal pain 2 days after surgery. Delayed contrast-enhanced computed tomography and retrograde urography revealed leakage of contrasted urine from the ureter into the retro

Epidemiology of Pediatric Musculoskeletal Injuries and Their Pattern in a Tertiary Care Center of North India.

BACKGROUND: Trauma is emerging as an epidemic and a leading cause of morbidity and mortality in children. Children <15 years of age comprise about 32.8% or about 1/3rd of the total Indian population. In India, up to one fourth of hospital admissions and approximately 15% of deaths in children are due to injury. This study presents the epidemiology, various causes and pattern of musculoskeletal injuries in pediatric population of North India. MATERIALS AND METHODS: This is an observational, prospective hospital-based study conducted in a tertiary care center of North India for 6 months from Ju

Etiology, pathophysiology, and most common injuries of the lower extremity in the athlete.

The majority of injuries during sport are to the lower extremity (more than 50%), most of which occur in children and young adults younger than 25 years and involve fractures, ligament injuries, or tendon injuries. This article discusses the etiology of some of the most common foot and ankle sports-related injuries. The authors focus on clinical findings, associated injuries, pathophysiology, and current trends. Many of these factors are discussed in detail elsewhere in this issue.

A rare case of nephrotic syndrome and arterial thrombosis following long-term rifampicin therapy.

Although rifampicin is one of the main drugs in the treatment of tuberculosis, its potential for causing nephrotoxic reactions may lead to renal injury. In this paper, we present a 23-year-old man who was treated for pulmonary tuberculosis and who developed a nephrotoxic syndrome caused by rifampicin. This syndrome was defined by abdominal distension due to fluid retention, swelling in both legs, nephrotic range proteinuria and biopsy findings showing tubular atrophy and interstitial fibrosis. The nephrotoxicity developed after a prolonged, uninterrupted course of treatment with rifampicin. Th

A Low-Cost Tensionable Quadriceps Tendon Rupture Repair Using Transosseous Tunnels.

A quadriceps tendon rupture is a traumatic injury of the quadriceps insertion on the patella leading to a disruption in the knee extensor mechanism. Diagnosis is usually made clinically with a palpable defect proximal to the superior pole of the patella coupled with the patient's inability to perform a straight leg raise and confirmed on radiographs, ultrasonography, or magnetic resonance imaging scan. When the extensor mechanism is disrupted, operative repair is indicated. Two common techniques have been described for repair: the transosseous) repair and the suture anchor repair, and both hav

Orthopedic Challenges in Avascular Necrosis Management: A Case Report of Surgical Complications and Multidisciplinary Care.

A 52-year-old woman, with a multifaceted medical background encompassing spinal cord injury, pneumonia, and recurrent hospitalizations, presents with enduring left hip and leg discomfort ultimately diagnosed as avascular necrosis (AVN). She previously underwent intraosseous direct anterior arthroplasty (DAA) of the left hip during the removal of orthopedic artifacts. Despite enduring hypertension, severe trochanter dislocation, and prosthesis fracture, she recovered and required additional surgery to address the dislocation and fracture. This case underscores the challenges in diagnosing and t

Rare Mucor circinelloides and Fusarium infection in latissimus free flap reconstruction after devastating foot injury in non-neutropenic patient.

Nowadays, free flap reconstruction in devastating lower limb trauma is a standard procedure in reconstructive surgery. The greatest factor directly affecting limb salvage is still the risk of infectious complications, whether local or systemic. Fungal wound infections are not among the most common infection complications in surgery, but their low incidence is compensated for by their fulminant and serious course, as well as severe local tissue destruction and strong angio-invasive potential together with the possibility of dissemination. In this case study, we present an example of a devastati

Semimembranosus muscle herniation: a rare case with emphasis on muscle biomechanics.

Muscle herniations are rare and most reported cases involve muscles of the lower leg. We use a case of muscle herniation involving the semimembranosus muscle, presenting as a painful mass in an adolescent male after an unspecified American football injury, to highlight a simple concept of muscle biomechanics as it pertains to muscle hernia(s): decreased traction upon muscle fibers can increase conspicuity of muscle herniation(s)-this allows a better understanding of the apt provocative maneuvers to employ, during dynamic ultrasound or magnetic resonance imaging, in order to maximize diagnostic

Threatened limb from stingray injury.

We present the case of a 43-year-old female who suffered a stingray injury to her left ankle. The sting caused occlusion of the dorsalis pedis artery, causing dry gangrene of the medial forefoot. A below knee amputation was recommended but she was transferred for a second opinion. A Prostaglandin E1 infusion was commenced, resulting in alleviation of pain and improvement in perfusion. Amputation of great and second toes was performed, with the head of the first metatarsal preserved and covered via a cross-over skin flap raised from the contralateral leg. Achilles tendon lengthening was then pe

Effect of surgical approach on the early outcome of total hip replacement for femoral neck fractures.

We reviewed the short-term outcome of 171 patients treated with total hip replacement for femoral neck fractures using the lateral (94 patients) or posterior approach (77 patients). The Sernbo score and the Abbreviated Mental Test Score were used to assess patients' pre-injury functional and cognitive status respectively. Patients were uniformly treated with regard to the type of prosthesis, size of femoral head and rehabilitation. They were followed up to a mean of 25 months (range : 13-42 months). The dislocation rate in the lateral group was 2.1% versus 0% in the posterior group. There was

Nonoperative management of a partial patellar tendon rupture after bone-patellar tendon-bone graft harvest for ACL reconstruction.

This is a case report of a young athlete who sustained a partial tear of the patellar tendon after anterior cruciate ligament (ACL) reconstruction with a bone-patellar tendon-bone (BPTB) autograft. The injury, diagnostic workup, and decision-making process that lead to the choice of nonsurgical treatment are described. Furthermore, the rehabilitation process is described in detail. The patient returned to his previous level of sports activity and had a good clinical outcome as measured by range of motion, isokinetic quadriceps muscle strength testing, single leg hop testing, and the modified N

Palm needle induced pseudo-tumours of bone.

Two children presented with recurrent swelling and pain, one in the forearm the other in the leg, without a history of a penetrating injury. It was difficult to arrive at a precise clinical diagnosis. X-ray films showed periosteal thickening in the diaphysis of the adjacent long bone with some unusual features but were suggestive of infection or tumour. Other extensive investigations did not contribute to a diagnosis. This was only achieved after surgical exploration. Both children had a 3 cm length of brittle palm needle within a soft tissue cavity adjacent to the long bone. The needles were

Not All Leg Swelling Is Volume Overload: Point-of-Care Ultrasound and Sirolimus-Associated Lymphedema After Combined Heart-Kidney Transplant.

A 65-year-old man, one year post combined heart-kidney transplant, presented with acute kidney injury, dyspnea, and progressive leg edema unresponsive to escalating diuretics. Despite suspicion for volume overload, multi-organ point-of-care ultrasound (POCUS) revealed bilateral A-lines without B-lines or effusions, a small collapsible inferior vena cava, and preserved biventricular function, all inconsistent with cardiogenic congestion. The edema was attributed to sirolimus-associated lymphedema. Diuretic cessation and transition from sirolimus to everolimus improved renal function. This case

Complications with recombinant human bone morphogenetic protein-2 in posterolateral spine fusion associated with a dural tear.

BACKGROUND CONTEXT: Potential complications related to ectopic bone formation, as seen with transforaminal lumbar interbody fusion, have always been a concern with the use of bone morphogenetic proteins (BMPs). Although less clearly anticipated, complications related to the proinflammatory effects of recombinant human bone morphogenetic protein-2 (rhBMP-2), such as swelling and edema in the cervical spine, have been observed as well. Until recently, risks related to intradural exposure to BMP have not been widely considered. However, a recent animal study reports "in the presence of a SCI and/

The Optimal Timing of Traumatic Lower Extremity Reconstruction: Current Consensus.

Marko Godina in his landmark paper in 1986 established the principle of early flap coverage for reconstruction of traumatic lower extremity injuries to minimize edema, fibrosis, and infection while optimizing outcomes. However, with the evolution of microsurgery and wound management, there is emerging evidence that timing of reconstruction is not as critical as once believed. Multidisciplinary care with a combined orthopedic and reconstructive approach is more critical for timely and appropriate definite treatment for severe lower extremity injuries.
