Changes in clinical management and diagnosis following DaTscan SPECT imaging in patients with clinically uncertain parkinsonian syndromes: a 12-week follow-up study.

BACKGROUND: An accurate diagnosis is important for timely and adequate treatment in patients with clinically uncertain parkinsonian syndrome (CUPS). OBJECTIVE: The objective of this study was to assess safety and changes in clinical management, diagnosis and quality of life (QoL) at 4 and 12 weeks following DaTscan (ioflupane [(123)I] injection) imaging in patients with CUPS. METHODS: This randomized, open-label, single-dose, multicenter trial was carried out in patients with CUPS who were randomized to either a DaTscan imaging group or to a control group without imaging. The main outcome me

Understanding Degeneration and Healing Pathways for Tissue-Engineered Treatment Strategies in Tendinopathy.

BACKGROUND: The multidisciplinary nature of the tendon tissue engineering field brings challenges to move from our basic understanding of tendon biology towards engineering strategies for tendon disorders. Such pathologies present a high risk of inflammation for young patients and limited tissue regeneration capability in ageing patients, leading to painful symptoms and impaired quality of life. SUMMARY: Among the complications observed in tendinopathy are degenerative changes in the extracellular matrix and aberrant tissue remodelling, accounting for a huge burden in musculoskeletal disorder

Oxylipins as canine sepsis indicators in vivo and in ex vivo skin organ culture model.

Sepsis, a life-threatening condition characterized by a dysregulated immune response to infection, remains a significant cause of mortality in both humans and veterinary patients. This study explores oxylipins as potential indicators of sepsis in dogs through in vivo plasma analysis and an ex vivo lipopolysaccharide (LPS)-treated skin organ culture model. By employing a robust analytical platform, 52 oxylipins and 4 polyunsaturated fatty acids were profiled in plasma and skin cultures. Results revealed distinct biochemical and morphological changes, with LPS triggering capillary vasodilation a

Platelet-mediated activation of perivascular mast cells triggers progression of sepsis to septic shock in mice.

The critical events that trigger sepsis progression into life-threatening septic shock remain unclear. In agreement with reports that link a drop in platelet count to a complicated clinical course in sepsis patients, here we report that, during sepsis, mouse platelets become activated, deposit systemically on vascular walls, and stimulate perivascular mast cells (MC) by releasing platelet activating factor (PAF). In mouse models and patient samples, MC activation correlates with the development of shock in sepsis and is mechanistically linked to shock by inducing systemic hypotension, vascular

Psychosocial and Behavioral Impacts of the Mpox Outbreak among People with and without HIV in the United States.

Over 32,000 mpox cases were identified in the United States (US) within one year of the start of the global outbreak in 2022. Research on how the mpox outbreak affected individuals' lives is limited. We analyzed qualitative and quantitative cross-sectional survey data collected from 60 participants in August 2022 to (1) describe mpox-related psychosocial and behavioral impacts among adults in the US and (2) explore differences in mpox-related impacts by HIV status. Results indicate that the outbreak affected both people with HIV (PWH) and people without HIV (PWOH) in our sample, while also hig

Tongue graft-versus-host disease: remission with ruxolitinib.

Graft-versus-host disease (GvHD) is a potentially life-threatening and commonly encountered event of allogeneic haematopoietic stem cell transplantation. Here, we present a young adult male with primary refractory Hodgkin's lymphoma who received a transplant and developed cutaneous GvHD after donor lymphocyte infusion, which was managed with cyclosporine and steroids. However, while the patient was under immunosuppressive treatment, diffuse confluent whitish patches on the patient's tongue were observed. A biopsy of the tongue lesions revealed lichenoid, hyperkeratotic tissue changes and intra

Nutrition in Advanced Thyroid Cancer Patients.

In the last decade, multikinase inhibitors (MKIs) have changed the paradigm of treatment of advanced and progressive thyroid cancer. Compared with the traditional treatment with chemotherapy and radiotherapy, these new drugs have shown a good efficacy in controlling the neoplastic disease, and also a different toxicity profile compared to traditional chemotherapy, milder but still present and involving mainly the nutritional profile. Weight loss, nausea, anorexia, stomatitis, diarrhea may be associated with malnutrition and cancer-related cachexia. The latter is characteristic of the advanced

Dysfunction of the Blood-brain Barrier in Cerebral Microbleeds: from Bedside to Bench.

Cerebral microbleeds (CMBs) are a disorder of cerebral microvessels that are characterized as small (<10 mm), hypointense, round or ovoid lesions seen on T2*-weighted gradient echo MRI. There is a high prevalence of CMBs in community-dwelling healthy older people. An increasing number of studies have demonstrated the significance of CMBs in stroke, dementia, Parkinson's disease, gait disturbances and late-life depression. Blood-brain barrier (BBB) dysfunction is considered to be the event that initializes CMBs development. However, the pathogenesis of CMBs has not yet been clearly elucidated.

How to Cope with Psoriasis: Data from Patient Tests and Surveys.

There are a limited number of studies that have addressed coping strategies in patients with psoriasis, so the aim of this study was to examine how patients with psoriasis cope with their illness. We included 122 participants in the study: 56 patients with psoriasis vulgaris and 66 healthy subjects. They filled out the Recent Life Changes Questionnaire (RLCQ), blinded to their stress and clinical status, and the Brief COPE questionnaire with 28 questions measuring 14 coping strategies that are either adaptive or maladaptive. We statistically analyzed the self-reported magnitude of each stressf

Addressing the Consequences of School Closure Due to COVID-19 on Children's Physical and Mental Well-Being.

Prolonged school closures are one of the most disruptive forces in the COVID-19 era. School closures have upended life for children and families, and educators have been forced to determine how to provide distance learning. Schools are also an essential source of nonacademic supports in the way of health and mental health services, food assistance, obesity prevention, and intervention in cases of homelessness and maltreatment. This article focuses on the physical and emotional toll resulting from school closures and the withdrawal of nonacademic supports that students rely on. The COVID-19 pan

Monitoring functional status using a wearable real-time locating technology.

Sensor technologies enable real-time, continuous, and objective monitoring of activity and functioning in later life. In long-term care, timely assessment of functional status is needed to prevent falls and other acute events. However, the electronic forms and paper and pencil tools currently used are time-consuming and conducted too infrequently (e.g., every 6 months) to provide the sensitivity and specificity required. Staff are also unable to detect subtle changes in functioning through observation alone. The purpose of this paper is to discuss the use of a wearable real-time locating syste

Syncope in a patient with minimal change disease without nephrotic-range proteinuria.

Introduction Pulmonary embolism is a potentially life-threatening complication of nephrotic syndrome. Syncope is rarely reported as an initial presentation of pulmonary embolism in nephrotic patients. Case presentation We describe a 35-year-old man who was taking steroids and diuretics for relapse of minimal change disease who presented after a syncopal event. The patient was hypotensive and had distended neck veins. The major laboratory findings were hypoalbuminemia with mild proteinuria. The findings on electrocardiography, chest radiography, and echocardiography and the elevated plasma D-di

Atrial Fibrillation Ablation in Adults With Repaired Congenital Heart Disease.

The incidence of atrial fibrillation (AF) in congenital heart disease (CHD) adults has increased in the past decades due to a longer life expectancy of this population where the subjects are exposed to cardiac overflow, overpressure and structural changes for years. The literature regarding AF ablation in repaired CHD adults emphasizes the importance of intracardiac echocardiography (ICE) to perform the transseptal puncture and the ablation procedure in the left atrium (LA), both effectively and safely. In small case control studies, where the predominant congenital cardiomyopathy was the atri

Sex differences in the pharmacokinetics of antidepressants: influence of female sex hormones and oral contraceptives.

Women are twice as likely to develop depression as men. Moreover, the symptoms they experience also show sex differences: women tend to develop depression at an earlier age and show more severe symptoms than men. Likewise, the response to antidepressant pharmacotherapy appears to have sex differences. These differences can partially be explained by differences in pharmacokinetic properties (i.e., absorption, distribution, metabolism, and excretion) of drugs in males and females. More recent research has shown that sex hormones may influence all these previously named pharmacokinetic processes.

Peripheral immune challenge with viral mimic during early postnatal period robustly enhances anxiety-like behavior in young adult rats.

Inflammatory factors associated with immune challenge during early brain development are now firmly implicated in the etiologies of schizophrenia, autism and mood disorders later in life. In rodent models, maternal injections of inflammagens have been used to induce behavioral, anatomical and biochemical changes in offspring that are congruent with those found in human diseases. Here, we studied whether inflammatory challenge during the early postnatal period can also elicit behavioral alterations in adults. At postnatal day 14, rats were intraperitoneally injected with a viral mimic, polyinos

Effects of immune system diversity and physical variation of immunotypic mixing on the dynamics of rabies in bats.

The ecology and life history of rabies viruses in bats suggest a need for an integrated modelling treatment that extends beyond traditional epidemiological approaches. We modify our adaptive modelling approach to investigate the effects of immune system structure and immunological mixing events on the disease profile of the bat colony and subsequently on the dynamics of rabies viruses in bats. Our theoretical framework, which is based on individual (intra-host) models of the response of the host to a viral challenge and virus-specific disease mechanisms, integrates the individual components to

Marfan syndrome-diagnosis and management.

Marfan syndrome (MFS) is the most common inherited disorder of connective tissue that affects multiple organ systems. This autosomal-dominant condition has an incidence of 2-3 per 10,000 individuals. Although genetic testing is available, the diagnosis is still primarily made using the Ghent criteria. Early identification and appropriate management is critical for patients with MFS who are prone to the life-threatening cardiovascular complications of aortic dissection and rupture. Advances in the understanding of the cause of MFS, early recognition of the disorder, and subsequent institution o

Recent advances in women's sexual function and dysfunction.

Current reconceptualization of women's sexual response acknowledges that women have many reasons or incentives for engaging in sex over and beyond sexual desire. Normative changes in their sexuality across the life span, with reproductive events, and with duration of relationship are recognized. Psychophysiological and preliminary functional magnetic resonance imaging data clarify that women's subjective experience of arousal may correlate poorly with signals reflective of genital congestion and also correlate poorly with activation of areas of the brain involved in organizing the reflexive ge

How Interpersonal Psychotherapy Changes the Brain: A Study of fMRI in Borderline Personality Disorder.

Background: Recent guidelines and systematic reviews suggest that disorder-specific psychotherapeutic interventions are the first choice in the treatment of borderline personality disorder (BPD). The aim of this study is to examine brain activity changes in BPD patients (DSM-5) who received a revised BPD-adapted interpersonal psychotherapy (IPT-BPD-R) compared with patients on the waiting list. Methods: Forty-three patients with a BPD diagnosis (DSM-5) were randomly assigned to IPT-BPD-R (n = 22 patients) or the waiting list with clinical management (n = 21 patients) for 10 months.
