Integrity and characteristics of secondary oesophageal peristalsis in patients with gastro-oesophageal reflux disease.

Secondary peristalsis contributes to oesophageal acid clearance. The aim of the study was to evaluate the integrity and characteristics of secondary peristalsis in patients with gastro-oesophageal reflux disease. Studies were performed in 22 patients with reflux disease and 20 age matched controls. Oesophageal motility was recorded at 3 cm intervals along the oesophageal body. Primary peristalsis was tested with 5 ml water swallows. Secondary peristalsis was stimulated with 10 ml boluses of air and water injected in the mid-oesophagus and by 5 second distensions with a 3 cm balloon at the same…

[Speech therapy guidelines in patients with amyotrophic lateral sclerosis].

Bulbar involvement is usual in patients with amyotrophic lateral sclerosis (ALS), leading to a progressive loss of speech, phonation and swallowing abilities. The goal of the speech therapy is to maintain, as long as possible, those abilities and to devise alternative communication strategies when oral communication is ineffective. This paper presents guidelines for speech therapy during disease progression.

Foreign body in the throat migrating through the common carotid artery.

We present a 55-year-old lady who swallowed a 3 cm pointed metal foreign body whilst eating a sardine salad. The foreign body migrated from the hypopharynx through the parapharyngeal space and traversed the common carotid artery over a period of 12 days. The foreign body was removed by exploration of the neck.

Infant apnea and gastroesophageal reflux.

The value of a combination of ultrasound studies and barium swallow examination in the analysis of temporal relationships between apnea and reflux is demonstrated. The two techniques allow acute apneic spells induced by gastroesophageal reflux to be documented and underlying specific digestive tract disorders demonstrated. The high incidence of digestive tract disorders in this area has been identified.

Reversible esophageal motor dysfunction in botulism.

Two cases of botulism with autonomic and neuromuscular system involvement are presented. In both patients, dryness of the mouth and difficulties in swallowing were predominant symptoms. Esophageal manometry revealed a marked decrease in peristaltic amplitude, which was most pronounced in the upper third of the esophagus. These functional abnormalities returned to normal following recovery from the acute disease.

One man's poison.

In an unrandomised uncontrolled study on boats and trains, capsules of flurazepam (30 mg) and temazepam (20 mg), when chewed, but not when swallowed whole (even with the ends pricked), gave a better night's sleep than 'Mandrax' (methaqualone 250 mg/diphenhydramine 50 mg) and a better night's sleep than nitrazepam 7.5 mg.

Peritonsillar, peripharyngeal, and deep neck abscesses.

Abscesses of the peritonsillar and peripharyngeal regions typically present as a sore throat and as difficulty in swallowing. One of the most ?striking clinical features is difficulty in controlling saliva. Deep neck abscesses usually are secondary to disease in the pharynx, tonsils, or teeth. Initially, attention should be devoted to finding the primary source of infection. Treatment of these abscesses is incision and drainage.

The esophagus in radiologic problems.

Examination of the esophagus must go considerably beyond observation of "a barium swallow"; it must include a thorough study of the hypopharynx, the cervical esophagus and the thoracic esophagus. Dysphagia, which is almost always the presenting complaint, may be due to a number of lesions originating outside the esophagus and thus cause no sign of mucous membrane destruction or of filling defect.

The Inpatient with Dysphagia: An Approach to Diagnosis and Treatment.

The inpatient with dysphagia can present a unique challenge in medicine. We present a review of normal swallowing function and relevant pathophysiology followed by an approach to evaluation and treatment. For the inpatient clinician, we have developed an algorithm and diagnostic guide based on history that will help advise the need for further workup, arrive at an accurate diagnosis, and facilitate collaborative intervention.

[RIGHT AORTIC ARCH].

A 66-year-old male developed dysphagia due to external compression of the esophagus caused by an aberrant left subclavian artery, as part of a rare congenital anomaly of a right aortic arch. This represents an incomplete vascular ring-an anatomical variant of the right aortic arch - which is typically asymptomatic but may present with swallowing difficulties in adults, as demonstrated in this case.

Medical Management of Eosinophilic Esophagitis in Pediatric Patients.

Eosinophilic esophagitis is an immune-mediated allergic disease of the esophagus that affects pediatric patients of all ages. The diagnosis is made by esophagogastroduodenoscopy demonstrating eosinophilic infiltrate of the esophagus. Approaches to treatment involve proton pump inhibitors (PPIs), swallowed topical steroid preparations, as well as dietary elimination. In this review we discuss the evidence and efficacy of each of these approaches.

It's not just a heartburn and reflux disease: a case report of distal oesophageal spasm and review of literature.

Distal oesophageal spasm is a rare condition that affects the motility of the oesophagus. It can be diagnosed by highresolution oesophageal manometry and the diagnosis is supported by other modalities such as barium swallow and esophagogastroduodenoscopy examinations. Treatment options include pharmacological therapy, endoscopy and surgical interventions. We described a case of distal oesophageal spasm in an elderly patient who presented with chronic dyspepsia.

Case for diagnosis. Palate perforation due to cocaine use.

We report a 42-year-old cocaine addicted female patient referred for evaluation of hard palate ulceration resulting in oro-sinus communication with difficulties in swallowing and phonation, an rhino-sinusitis. Aberrant insertion of the right subclavian artery: an unusual cause of dysphagia in an adult.

INTRODUCTION: Within this report, we present a patient with difficulty of swallowing caused by an aberrant right subclavian artery. DISCUSSION: This is a congenital anomaly with the right subclavian artery originating from the dorsal part of the aortic arch and coursing through the mediastinum between the esophagus and the vertebral column. The diagnosis and treatment of this disorder is discussed based on the findings from the literature.

Side effects and complications of velopharyngeal surgery.

Although velopharyngeal surgery is generally safe, complications are not uncommon. This article reviews the major complications occurring in the postoperative period and the severe late complications such as velopharyngeal incompetence and nasopharyngeal stenosis. Most of these complications can be prevented by a careful operative technique and preoperative evaluation. We also emphasise the frequency of minor modifications of voice, swallowing, taste and velopharyngeal function.

Small bowel obstruction caused by snail's shell: radiographic and CT findings.

We report a case of small bowel obstruction in a 74-year-old woman presenting with abdominal pain due to the accidental swallowing of a snail shell. A diagnosis of obstruction was made by abdominal radiograph, and its etiology was found after abdominal CT was performed. This is an unusual case of small bowel obstruction due to a foreign body that was preoperatively diagnosed with imaging.
