Left arial (LA) function, defined according to conduit, reservoir and booster functions, is closely linked to left ventricular (LV) mechanics, particularly during diastole. Right ventricular pacing (RVP) is thought to impair LA diastolic restoring forces through alteration of ventricular activation. The aim of this study was to determine whether the LA functional reservoir estimated as the change in mean LA ejection fraction (EF) immediately after RVP, and for the second and for the third beats after RVP, predicts clinical outcome in patients with paroxysmal atrial fibrillation (AF) who have u

We report the case of a 62-year-old woman who presented with an acute inferior wall myocardial infarction complicated by cardiogenic shock and refractory ventricular fibrillation. Following prolonged resuscitation in the emergency room, she was transferred to the cardiac catheterization laboratory where, as a first step, mechanical circulatory support with venoarterial extracorporeal membrane oxygenation (ECMO) was established. Next, a right heart catheterization study was performed, followed by coronary angiography and angioplasty of the infarct-related artery. Promptly on transfer to the int

Patients with end-stage renal disease (ESRD) show characteristic abnormalities in cardiac structure and function. We evaluated the influence of these abnormalities on adverse cardiopulmonary outcomes after living donor kidney transplantation in patients with valid preoperative transthoracic echocardiographic evaluation. We then observed any development of major postoperative cardiovascular complications and pulmonary edema until hospital discharge. In-hospital major cardiovascular complications were defined as acute myocardial infarction, ventricular fibrillation/tachycardia, cardiogenic shock

Thirteen children (age range 6 h-11 years, median 4 months) operated upon for aortico-left ventricular tunnel since 1971 are reported. All patients had systolic and diastolic murmurs, 4 of 5 neonates were in severe congestive heart failure. Twelve children had left ventricular hypertrophy on ECG with an ischaemic pattern in the neonates. Chest X-ray revealed cardiomegaly with a broad upper mediastinal shadow from the enlarged aorta. Echocardiography (2DE) revealed a tubular communication between the aorta and the left ventricle bypassing the aortic valve and massive regurgitation through this

INTRODUCTION: Transcatheter pulmonary valve replacement (TPVR) is increasingly used to treat complications related to dysfunctional right ventricular outflow tracts (RVOT). Candidacy is limited by RVOT diameter and coronary artery anatomy. We present the case of a branch pulmonary artery (BPA) that was too large to accommodate a single TPVR and underwent placement of 2 TPVR.
CLINICAL CASE: The patient is a 54-year-old with a history of pulmonary valve stenosis who underwent a Brock procedure at 8 years-old. He presented due to NYHA class 3 heart failure symptoms. He underwent an extensive wor

Functional tricuspid regurgitation (TR) is a common etiology of TR. Functional TR results from geometrical distortion of the normal spatial relationships of the tricuspid leaflets, annulus, chords, papillary muscles, and right ventricular (RV) walls. Functional TR results most commonly from left-sided heart disease, including mitral valve abnormalities and cardiomyopathy and RV dysfunction secondary to pulmonary disease (Cor pulmonale). The tricuspid annulus, which has a normal bimodal or saddle shape, becomes larger, flatter, and more circular with the development of functional TR. RV dilatio

BACKGROUND: Transcatheter tricuspid valve replacement (TTVR) is an option for selected patients with severe tricuspid regurgitation (TR) who are at high surgical risk, but experience remains limited in those with extreme right-heart anatomy, including a giant right atrium and a markedly enlarged elliptical tricuspid annulus.
CASE SUMMARY: A 62-year-old man was admitted with progressive dyspnoea and lower-extremity oedema (NYHA class IV). He had previously undergone mechanical mitral valve replacement and transcatheter aortic valve replacement. Echocardiography and computed tomography angiogra

BACKGROUND: Left ventricular ejection fraction (LVEF) and global longitudinal strain (GLS) remain recommended for cardiotoxicity surveillance during cancer therapy, yet it changes late and overlooks injury to the other cardiac chambers.
PURPOSE: We systematically reviewed echocardiographic and cardiac magnetic resonance (CMR) studies that assessed left atrial (LA), right atrial (RA), and right ventricular (RV) function in patients exposed to potentially cardiotoxic oncological treatment.
METHODS: PubMed, Scopus, Web of Science, and the Cochrane Central Register of Controlled Trials were sear

Failure of tricuspid valve prostheses is an increasingly encountered and complex clinical problem. Patients often present with advanced comorbidities, progressive right ventricular dysfunction, and prohibitive surgical risk. Despite growing burden, high-quality evidence and clear guideline-directed strategies remain limited. This narrative review summarizes contemporary approaches to the management of failed tricuspid valve prostheses, which include traditional redosurgical re-intervention, minimal invasive and beating-heart surgical techniques, and transcatheter valve-in-valve and valve-in-ri

Large ascending aortic aneurysms (AscAAs), defined as exceeding 8 cm, are exceedingly rare and carry high mortality due to the combined risks of rupture and mediastinal compression. Their management demands precise anesthetic, surgical, and perfusion coordination to prevent hemodynamic collapse and neurologic injury. We report an 83-year-old woman with an 8.9 × 7.7 cm AscAA producing marked right ventricular and pulmonary artery compression. Perioperative management emphasized advanced monitoring, preservation of venous return during induction, and readiness for immediate cardiopulmonary bypas

BACKGROUND Noonan syndrome is a clinically diagnosed genetic disorder frequently associated with congenital and structural cardiac abnormalities such as valvular dysplasia and cardiomyopathy, which may predispose patients to infective endocarditis. Delayed recognition, particularly in underserved populations, can lead to advanced cardiac complications and complex presentations. CASE REPORT A 29-year-old woman from a rural area with no previous medical history presented with severe respiratory distress, weight loss, and newly detected cardiac murmurs. Physical examination revealed dysmorphic fe

Pulmonary arterial hypertension (PAH) is a progressive and life-threatening disease characterized by pathological remodeling of the pulmonary vasculature. The serotonin 2A receptor (5-HT2aR, encoded by Htr2a), a G protein-coupled receptor expressed in the pulmonary vascular wall, has been implicated in the pathogenesis of PAH. However, the effect of 5-HT2aR deficiency has not been investigated in rat models relevant to human disease and the underlying mechanisms remain unclear. In this study, we generated Htr2a knockout rats using the CRISPR/Cas9 system and established PAH models using monocro

BACKGROUND: Current prediction tools for acute mountain sickness (AMS) are mainly limited by single-altitude designs. This study aimed to evaluate the cross-altitude predictive value of pre-ascent echocardiography and investigate altitude-specific cardiac mechanisms.
METHODS: This prospective study assigned 238 healthy young males to 3800 m (n = 106), 4300 m (n = 60), or 4500 m (n = 72) groups. Baseline echocardiography was performed at 200 m before ascent. AMS was assessed (days 1-7) using the Lake Louise Questionnaire. Candidate predictors were selected via a multi-stage process (univariate

BACKGROUND: Right ventricular (RV) dysfunction negatively affects mitral valve transcatheter edge-to-edge repair (M-TEER) outcomes in patients with ventricular secondary mitral regurgitation (vSMR). However, RV dysfunction occurs in the late phase of heart failure, when it may not respond to interventions. The pulsatile component of RV afterload, pulmonary artery (PA) compliance, is a sensitive parameter that decreases before RV dysfunction occurs. We explored the utility of PA compliance in predicting cardiac events after M-TEER.
METHODS AND RESULTS: We analyzed 107 patients with vSMR who un

PURPOSE OF REVIEW: The unique pathophysiologic considerations of severe tricuspid regurgitation (TR) have led to advancements in surgical and transcatheter treatments. The purpose of this review is to highlight the current surgical and transcatheter tricuspid valve interventions (TTVI) to functional TR.
RECENT FINDINGS: Surgical repair with ring annuloplasty consistently demonstrates better outcomes than surgical replacement or other repair approaches. However, surgical uptake of TR correction remains relatively low, and operative mortality rates are still high owing to multiple comorbidities

In 1992, the Brugada brothers published a patient series of aborted sudden death, who were successfully resuscitated from ventricular fibrillation (VF). These patients had a characteristic coved ST-segment elevation in the right precordial leads on their 12-lead electrocardiogram with no apparent structural heart abnormality. This disease was referred to as "right bundle branch block, persistent ST-segment elevation, and sudden death syndrome." The term Brugada syndrome (BrS) was first coined for this new arrhythmogenic entity in 1996. BrS is more prevalent in Southeast Asian ethnic groups and
